Abstract

Background : Surgical wound infection is a problem that is often found today in surgery. The Indonesian Ministry of Health reported that in 2011, 55.1% of surgical wound infections were found in government hospitals. This is in line with WHO data in 2010, where the sectio caesarean rate in Indonesia was recorded at 6%. Skin preparation can be used to prevent surgical wound infection by reducing the number of germs on the skin prior to incision.Objective : To see the relationship between skin preparation and post sectio caesarea surgical wound infection at Achmad Mochtar Bukittinggi Hospital.Methods : This study was an analytical study using a cross sectional study design by looking at the subject's medical records according to the time and place of the study. Samples were all medical records of mothers giving birth by sectio caesarea in the Obstetrics and Gynecology Section of Achmad Mochtar Bukittinggi Hospital in the period 1 January 2017 to 31 December 2017. Samples were taken from populations that met the inclusion and exclusion criteria. The sample was taken using simple random sampling technique which was taken from the medical records of Achmad Mochtar Hospital Bukittinggi. Statistical analysis to assess significance used the chi-square test.Results : The sectio caesarean group that carried out skin preparation without using chlorhexidine gluconate had the highest number (59.8%), with post sectio caesarean wound infection of 70.7%.Conclusion : There was a significant relationship between skin preparation and post sectio caesarean surgical wound infection (p <0.021) at Achmad Mochtar Bukittinggi Hospital.Keywords: Sectio caesarea, skin preparation, surgical wound infection

Abstract

There are changes of eutopic endometrium molecular fenotipe in endometriosis such as changes in gene expression, steroid hormone response, increase of inflammation marker and cellular adhesion molecule, decrease of apoptotic index and decidualization capacity, increase of oxidative stress marker, increase activity of angiogenesis and neurogenesis. This study was conducted to analyze the differences in the expression of angiogenic factors (VEGF A) and neurogenesis factors (PGP 9.5) eutopic endometrium of menstrual bleeding among patients with endometriosis and non endometriosis and then the correlation of the two factors. This study is a cross sectional that examines the relationship between the incidence of endometriosis with risk factors such as VEGF expression and PGP 9.5 in menstrual blood. There were significant differences in the expression of VEGF A eutopic endometrium menstrual blood between endometriosis and non endometriosis group (p=0.002). There are significant differences expression of VEGF A and PGP 9.5 eutopic endometrial of menstrual blood between endometriosis and non endometriosis groups. There is a positive correlation between the expression of VEGF A with PGP 9.5 eutopic endometrial of menstrual blood on endometriosis patients.Keywords: eutopic endometrium, menstrual blood, VEGF A expression, PGP 9.5 expression, immunocytochemistry

Abstract

Premature rupture of membranes (PROM) is defined as rupture of membranes before contraction (his). PROM is the case of 5-10% of all deliveries. 70% of cases of PROM occur in term pregnancies. So that PROM in aterm pregnancies is suggested to induce labor to reduce the incidence risk of mother and fetus complications. This study aimed to compare the effectiveness of cervical ripening by using Misoprostol 25 µg peroral and without using Misoprostol 25 µg at term premature rupture of the membrane with Bishop Score ≤ 4. This study was conducted at the Departement of Obstetrics and Gynecology, Dr. M. Djamil Central General Hospital, and Reksodiwiryo Hospital Padang from November 2013 – August 2014 Simple Random Sampling design. The subject selected based on inclusion criteria, then assess the state of the cervix and laboratory examination and nonstress test, then recapitulated and processed data is computerized using the computer software and statistical test. Significant differences increase bishop scores on 25 mcg misoprostol orally with those not given to premature rupture of membranes at term with less than bishop scores or equal to (≤) 4. The p-value evidence this was 0.000 <0.05. And there is no side effects in this study.Keywords: misoprostol effectivity, premature rupture of membrane

Abstract

Background : Cervical agenesis is an extremely rare congenital anomaly of female reproductive system with manifestation on the absence of uterine cervix with normal development of uterine and secondary sex signs. According to the American Fertility Society, cervical agenesis is classified as type IB Müllerian anomaly. Symptoms are include primary amenorrhea, cyclic abdominal lower pain and infertility.Objective : To report the treatment of cervical agenesis.Case Report : Reportedly, a woman, 28 years old, came with complaints of primary amenorrhea, cyclic abdominal lower pain and 3 years of primary infertility. From speculum and transvaginal ultrasonography examination, the uterine was slightly smaller than normal, without cervical uterine, found adhesion, both ovarian were normal. Laparoscopic utero-vaginal anastomosis was performed and catheter condom was maintained in the cervical canal for 3 months. Two weeks after catheter was removed, a hysteroscopy was performed, but the canal was found closed.Conclusion : In this case, after the laparoscopic utero-vaginal anastomosis was performed, the cervical canal which has been made, is found closed. Abstract

Objective?: This study looked at the incidence of overactive bladder (OAB) in KIA poly patients at Pauh Health Center in Padang City using Overactive Bladder Symptomps Score (OABSS).Method?: This research is descriptive. The sampling technique was purposive sampling by considering inclusion and exclusion criteria. The inclusion criteria were women of ideal reproductive age (20-35 years) who had given birth spontaneously, were not pregnant, did not suffer from neurological disorders, diabetes, post bladder surgery and or urinary tract infections and were not treated with Overactive Bladder (OAB), while the exclusion criteria are not willing to take part in the study. The study was carried out at the KIA Poly of Pauh City Health Center in Padang during January 2019. The variable in this study was Overactive Bladder (OAB).Result?: In this study, 97.22% of respondents experienced complaints of overactive bladder (OAB), (97.14%) experienced an urgent complaint, and only a small percentage (2.86%) of respondents experienced urinary incontinence. The results of this study indicate that respondents who did not experience complaints of overactive bladder (OAB) were respondents with the smallest parity (parity 1).Conclusion?: Most respondents experienced complaints of overactive bladder (OAB) and urgency, and only a small proportion of respondents experienced urinary incontinence. Abstract

There will be multiple organs changes in preeclampsia and eclampsia. One of them is a change in hemostasis system which is platelet activation, extrinsic and intrinsic cascade reaction and increasing of fibrinolytic activation. This is a cross sectional study conducted at Obstetric and Gynecologic Departement of Medical Faculty of Andalas University/ M Djamil Central Hospital in Padang on July 2014 with the number of samples are 44 persons. Samples are divided into 3 groups: Severe preeclampsia, eclampsia, and normal pregnancy. Platelet, PT,APTT, and D-Dimer counting were conducted and statistic analyzed was done with Anova dan Post Hoc Bonferoni. The more severe pregnancy, the lower platelet count and PT, but the dif- ference is not statistically significant between three groups: severe preeclampsia, eclampsia, and normal pregnancy (p < 0,05). Mean of APTT and D-Dimer is statistically significant due to condition of pregnancy. Post Hoc Bonferroni analysis showed a significant difference of APTT mean in the eclampsia, severe pre-eclampsia, and normal pregnancy (p <0,05 ). D-Dimer Mean shows a significant difference between normal pregnancy, severe preeclampsia, and eclampsia (p <0,05).Keywords: Pregnancy condition, severe preeclampsia, eclampsia, normal pregnancy, platelet, PT, APTT, and D-Dimer

Abstract

Objective : To find the difference in mean hemostatic factors in severe preeclampsia and eclampsia.Method : The study was analytic descriptive using a cross sectional study design by looking at the subject's Medical Record according to the time and place of research. The sample is the entire medical record of pregnant women who suffer from preeclampsia and eclampsia in the obstetric and gynecology departments of Dr. M. Djamil Padang Hospital in the period 15 January 2016 to 31 December 2017. The assessment was in the form of assessed platelet levels, PT, APTT and D-Dimer. Samples were taken from populations that met the inclusion criteria and did not have exclusion criteria. Sampling using consecutive sampling techniques taken from the Medical Record Dr. M.Djamil Padang. Statistical analysis to assess significance using the T-Test.Result : The severity the condition of pregnancy the lower the platelet mean and PT. Significant differences were obtained between platelets in PEB and eclampsia (p> 0.05). The mean PT, APTT and D-Dimer showed no significant differences in pregnancy conditions. Statistical test with T-Test did not show significant differences in mean PT, APTT, and D-dimer between eclampsia and PEB (p> 0.05).Conclusion : There were significant differences in mean in platelets, whereas in PT, APTT, and D-dimers in PEB and eclampsia there were no significant differences.Keywords: Pregnancy, severe preeclampsia, eclampsia, platelets, PT, APTT, D-Dimer.

Abstract

In preeclampsia occurring carbohydrate and fat metabolism disorders. Components of the metabolic syndrome such as insulin resistance, obesity, and dyslipidemia contributes to the occurrence of preeclampsia. This was an obser-vational analytic study with Kohort design and has been performed in Obgyn Department of M. Djamil Hospital Padang, primary health care in Koto Berapak, Private Practice Midwive in Lintau from July 2013 to May 2014. 60 samples of second trimester of pregnancy with positive Roll over test. Each subject was examined BMI, fasting blood glucose, LDL, HDL, triglycerides, and insulin levels. Then divided into two groups, positive metabolic components group and negative metabolic components group. Statistical analysis to assess significance using the unpaired t test and chi square on SPSS 18.0 for windows. There was no significant association between metabolic components and preeclampsia (p> 0.05). Other metabolic components such as HDL level and insulin resistance were not statistically significant with preeclampsia (p> 0.05). HOMA IR examination also showed no significant association with the in-cidence of preeclampsia. However, the subgroup analysis showed a mean insulin levels higher in preeclampsia patients compared with normal pregnancies (p <0.05). Abstract

Using Visual Inspection with Acetic Acid (VIA) as one examination for cervical cancer screening not well-known among health workers in Padang. This study was conducted to know the level of knowledge and attitudes of midwives about cervical cancer screening using Visual Inspection with Acetic Acid (VIA) examination in Primary Health Center in Padang. This was a cross-sectional analytic study and has been performed in the Primary Health Center in Padang from July –August 2014. The selection of the research subjects was performed to a midwife who worked in a gynecological clinic that meets inclusion criteria. We found 48 midwives that met inclusion criteria, guided interviews were conducted with a questionnaire, data recapitulated, and computerized processed using computer software and performed statistical tests. There was a weak correlation between the level of knowledge (X) and attitude (Y) about Visual Inspection with Acetic Acid (VIA) in cervical cancer screening in Padang primary health center. Abstract

Objective: To report cases of hydrops fetalisMethod: Case reportResults: The case was a female patient aged 36 years, with a diagnosis of G3P1A1H1 gravid 23-24 weeks + Hydrops Fetalis + 1x SC former. On ultrasound examination at 23-24 weeks of age, fetal biometry was found; BPD: 58.9 mm, HC: 211.0 mm, AC: 202.5 mm, FL: 44.4 mm, HL: 40.7 mm, EFW: 417 gr, SDP: 12.79 cm, FHR: 162x / minute, shows anasarcoma edema (+), hydrothoric (+), ascites (+), impression: gravid 23-24 weeks according to biometry, live fetus, Hydrops fetalis, polyhydramnios. Then amniocentesis was carried out followed by a chromosome analysis examination carried out by the G-Banding technique. The chromosomes from 18 cells from 3 different cell culture preparations were carried out and obtained the number of chromosomes in each cell studied was 46, XY, which means that the number of chromosomes is 46. fruit with the fetal sex chromosome is XY. No major structural abnormalities were seen. At 25-26 weeks of gestation, the baby was born by SC with BBL: 2100 gr, PB: 32 cm, maceration degrees 2- 3, Hydrops Fetalis.Conclusion: Hydrops Fetalis is an abnormal accumulation of fluid in 2 or more compartments of the fetal body. The prenatal diagnosis of Hydrops Fetalis can be confirmed by fetal imaging, maternal hematology, amniocentesis.Keywords: Hydrops Fetalis, polyhydramnios
