Critical Medical and Surgical Nursing Journal (CMSNJ) (p-ISSN: 2355-1992), (e-ISSN: 2656-470X) was established at the end of 2012 under Universitas Airlangga. The CMSNJ was prepared in response to the development of nursing/health science and technology to contribute to the development of sustainable quality nursing/health resources. Abstract

Introduction: Triage is the process of sorting patients according to their severity. However, the phenomena that occur in the emergency room of some hospitals are different, triage is not carried out according to the standards. This is due to many factors, one of which is the triage model used. The purpose of this study was to look at the relationship between the use of Standard Operations Procedure for admitting new patients using ATS triage which was modified with the speed of response of nurses in the Emergency Installation. Method: This study used a correlative analytical design with a case study approach. The total population is 31 people and a sample of 28 people was obtained through inclusion and exclusion criteria. The independent variable in this study was the use of ATS (Australasian Triage Scale) and the dependent variable in this study is the nurse's Response Time. Data obtained by means of sample observation and analyzed using the Spearman correlation test rho. Results: There was no association between the use of ATS (Australasian Triage Scale) modification and the response time of nurses in the morning shift (p = 0.720), day shift (p = 0.866) and night shift (p = 0.173).Discussion: From the results of the study there was no relationship between the use of ATS (Australasian Triage Scale) and the response time of nurses in the morning, afternoon and night shift. The use of ATS modification and the response time of nurses that are appropriate and fast are found in morning service. This is caused by several factors both from the nurses themselves and from the hospital. Most nurses do not get training in Triage and this affects nurses' skills in sorting patients.

Abstract

Introduction: Stroke is a disease that needs attention, especially for patients and their families. Not all families have the motivation to do a ROM for various reason. The aimed of this study was to examine the influence of health education to the family motivation to do ROM for the patients with infarction stroke in Seruni A Ward, Dr.Soetomo Hospital Surabaya.Methods: This study was used Pre Experimental Design. Population in this study were all family of the patients with infarction stroke in Seruni A Ward, Dr.Soetomo Hospital Surabaya. Purposive sampling technique was used in this study. There were 10 respondents as the sample who met to the inclusion criteria. The independent variable was helath education about Range Of Motion (ROM) and the dependent variable was the patient's family motivation to do ROM. Data were collected and analyzed using Wilcoxon Signed Ranks test.Results: The results showed that there was an effect of health education to family motivation to do ROM for the patients with infarction stroke in Seruni A Ward, Dr.Soetomo Hospital Surabaya (p=0.01).Conclusions: Health education about the ROM influence family motivation in carrying out physiotherapy. Nurses and other health professionals should provide health education in order to motivate families to increase and accelerate the healing process of patients. The results of this study can be further investigated by comparing the extension method, which one is more effective to motivate patients and it can associated with a cure rate of patients.

Abstract

Introduction: Aging process affects physical and mental changes that lead to lower resistance, resulting in the onset of various diseases and most commonly found is hypertension. Therapeutic gardening is hobby that can be used as a medical therapy to lower blood pressure. The purpose of this study was to determine the effectiveness of therapeutical gardening in blood pressure changes in elderly with hypertension in UPT PSLU Magetan.Methods: This study was a pre-experimental study using one-group pre-post test design. The population was elderly with hypertension at UPT PSLU Magetan. Sampling technique used was purposive sampling with 10 respondents who were selected based on inclusion criteria. The independent variables in this study was therapeutical gardening and the dependent variable was blood pressures, which were measured with sphygnomanometer and stethoscope. Data were analyzed using paired t test with significance value p < 0.05.Result: The average of systolic blood pressure pre-test was 161 mmHg and post-test was 149 mmHg with paired t test showed p = 0.013 and the average of diastolic blood pressure showed pre-test was 83 mmHg and post-test was 79 mmHg and paired t test showed p = 0.037, that showed therapeutical gardening is effective to change blood pressure in elderly with hypertension.Discussion: Therapeutic gardening effectively changes blood pressure in elderly withhypertension, so that the elderly are expected to perform therapeutical gardening as one of nonpharmacological alternative to lowering blood pressure, because it can be done independently or together and does not cause side effects.

Abstract

Introduction: Sampang is one of the districts in East Java that has many problems faced, especially in the health sector. With a population of 947,614 inhabitants, Sampang facing the issue of professionalism of health workers, nurses 'career paths, high prevalence of Tuberculosis, and high incidence ofdisasters (floods) in Sampang Regency that require nurses' readiness and ability in an emergency situation. Nurses as the frontline of health services are required to be able to demonstrate their professionalism through the effectiveness and success of the actions taken. This activity aimed to increase professionalism and encourage nurses to improve their competence through continuing education and training.Method: Faculty of Nursing Universitas Airlangga in collaboration with the Sampang Health Departement held conference and workshop on the topics: (1) nurse professionalism: assessment of legal aspects and nursing ethics; (2) nurse career paths; (3) comprehensive treatment of tuberculosis includes bio-psycho-socio-spiritual; (4) wound care; and (5) update on basic life support based on the 2015 AHA Guidelines. The target was 152 nurses at the primary health care in the working area of the Sampang District Health Office who are selected and identified according to the competencies. The activities were carried out during the period of July-August 2018.Result: A series of education and training programs produced output in the form of increasing knowledge and attitudes about nurse professionalism and career paths, increasing knowledge and attitudes about handling Tuberculosis patients, and increasing nurses' competence in wound care and BLS (Basic Life Support).Discussion: In the future, sustainable education and training activities are needed for nurses and other health workers in accordance with the latest developments in science and health technology to create professional nurses/health workers who encourage the achievement of the Healthy Indonesia program.

Abstract

Introduction: There are many drugs in the effort to cure hypertension for the old that in fact have bad influence because they cause dependence and are risky with misestimated doses. Communication back massage and back massage are two of complementary therapies recommended to lower blood pressure due to each relaxation effects are good. This study aims to clarify the difference of communication back maassage and back massage to reduce blood pressure. Method: This study was used quasy experimental design. The total samples in this study were 14 old patients who got hypertension. They were divided into 2 groups, one of the groups was treated with communication back massage and the other one was only treated with back massage. The independent variable was communication back massage and back massage. The dependent variable was blood pressure, which was observed for 5 days. Data were collected using aneroid sphygmomanometer and observation paper then analysis was implemented using Paired t-test with level significance α≤0.05 and Independent t-test with level significance α≤0.05. Result: The result showed that blood pressure decrease in significantly both in group one and group two (p=0.04 in group one and p=0.046 in group two). Between the group one and the group two, there was no difference on blood pressure reduction (p=0.315). Discussion: Communication back massage and back massage have a same effect on blood pressure reduction in old patient who get hipetension. The possible explanation for these findings is that communication back massage and back massage have a relaxation effect that can decrease norepinefrin, decrease ADH and increase endorphine. The decrease of norepinephrine can widen blood vessel. The decrease of ADH can reduce excessive fluid in the blood circulation. The increase of endhorphine can reduce sympathetic nerve activity. Therefore, blood pressure level will be decreased.

Abstract

Introduction: Diabetes mellitus is a degenerative disease with high prevalence that happens in many countries and high risk of causing death. The objective of this study is to know the effect of decoction by Allium cepa L on blood glucose level regulation in mice with diabetes mellitus.Method: This study was used a true experimental pre post test design with 30 samples, 8-12 weeks old-male mice, and 20-30 grams in weight. The mices were divided into five groups (normal control group, diabetes control group without treatment, treatment group with 3 gram/kg of decoction, treatment group with 4 gram/kg of decoction and treatment group with 5 gram/kg ofdecoction). The independent variable was Allium cepa L decoction (1cc) as a treatment, which is given per oral. Furthermore, the dependent variable was blood glucose level after 7 days treatment with Allium cepa L decoction.Result: The result showed that there were some differences between the treatment group and the diabetes control group without treatment for blood glucose level after giving treatment with Allium cepa L decoction (p<0.005). in other hand, there was no significant result between the normal group and the treatment group for blood glucose level after giving treatment with Allium cepa L decoction 5 gram/kg bb (p>0.005). Allium cepa L contains many substances which has many function to regulate blood glucose level.Discussion: It can be concluded that Allium cepa L decoction (5 gram/kg bb in 1 cc) is adequate for regulating blood glucose level in diabetes mellitus mice after 7 days treatment

Abstract

Introduction: Asthma is a disease that often found in the community, according to the data from the World Health Organization in 2006, as many as 300 million people suffered from asthma and 225 thousand people died from asthma worldwide.Patients with asthma often use asthma exercise, pranayama yoga, deep breathing, Buteyko and upper body exercise as a way to help reduceasthma. Effectiveness of each of these exercises to improve expiratory lung capacity has not been identified.Methods: This study used quasi-experimental pre-post test design. The population consisted of 21peoples divided into 9 individuals each in Buteyko breathing exercises and upper body exercise. Samples were collected with purposive sampling. The independent variables were Buteyko exercises and upper body exercise, and the dependent variable was the peak expiratoryflow. Data were collected through the observation of peak expiratory flow using a peak flow meter. Data processing used T test with significance <0.05 and comparison of two techniques used independent T test with significance <0.05.Result: The result of T test analysis showed that p=0.000 indicating there was correlation between the two techniques in improving peak expiratory flow inbronchial asthma, and independent T test obtained p=0.078, indicating there was no difference between the two techniques in improving peak expiratory flow in bronchial asthma.Discussion: It can be concluded that Buteyko breathing exercises and upper body exercise has no different function in increasing the value of peak expiratory flow. In patients with bronchial asthma,can useone or both of the breathing techniques, because it has the same effectiveness to increase peakexpiratory flow in assisting the prevention of bronchial asthma.

Abstract

Introduction: One of the causes of death in patients with chronic kidney disease on hemodialysis was due to the problem of uncontrolled fluid intake. The prevalence at Dr. Soetomo reported hospital as much as 80% increase weight more than 6%. Nursing education in fluid restriction was expected to improve adherence to fluid intake, and therefore contributes to the achievement of the ideal IDGW was less than 2%.Method: Design in this study was pre-experiment. The population in the study were all patients with chronic kidney disease undergoing hemodialysis. The samples obtained were 28 respondents who met to the inclusion and exclusion criteria. Purposive sampling technique was used in this study. The independent variabel was education. The dependent 2 variabel was Inter Dialitic Weight Gain, Qb and systolic blood pressure. Body weight was observered before and after hemodialysis. Blood pressure was observered during hemodialysis. Data were analyzed using Wilcoxon Signed Rank test with significant level of p=0.005.Result: The results showed that IDWG (p=0.157); Qb (p=0.007) and systolic blood pressure (p=0.021). IDWG patients did not undergo significant changes before and after education. Qb and systolic blood pressure of patients experiencing significant changes before and after education. Nursing educations provides significant effect on the patient's fluid intake compliance with indicator Qb and systolic blood pressure.Conclusions: Nursing educations would provide significant effect on compliance if supported by many factors such as family support advocates, social support and motivation of the patients.The provision of education should be carried out continuously and sustainably to obtain optimal results in patients undergoing regular hemodialysis , especially in terms of compliance with hemodialysis therapy and fluid therapy regimen , in order to achieve adequate dialysis. In a subsequent study can be developed not only limited to the provision of education to the patient but also involve the patient's family .

Abstract

Introduction: People with diabetes mellitus were at 15% risk of developing diabetic foot ulcers. Diabetic foot ulcers can be prevented if people with DM have the knowledge, positive attitude and thus able to perform the diabetic foot care. The aim was to determine the effect of health education on knowledge, attitudes and preventionof diabetic foot ulcers of people with diabetes mellitus. Method: quasi-experiment research design pretest-posttest control group design. Sample was DM patients who visited the Bangkalan City Health Center, recruited by purposive sampling technique with sample size of 35 people in the treatment group and 35 people in the control group. Independent variable was the health education and the dependent variables were knowledge, attitudes and of diabetic foot ulcers. Data were collected with questionnaires andthen analyzed by Wilcoxon Sign Rank Test and Mann-Whitney Test.Result: The results showed that there was an effect of health education on knowledge (p=0.000), attitudes (p=0.000) and preventive actions (p=0.000) of diabetic foot ulcers.Conclussion: Health education was important as an effort to persuade DM patients to take charge on their knowledge and attitude foot care by firstly enhancing.

Abstract

Introduction: Rest and sleep were human base necessary. Sleep quality was human satisfied for sleep, so that human not felt tired, not easier stimulated and nervous, not weak and apathies, no blackness around eyes, no swollen eyelid, no red conjunctiva, no poignant eyes, no difficulties to concentrate, no headache, or often felt sleepy. The aimed of this study was to explain the dominant correlation factors of sleep quality in Faculty of Nursing student Airlangga University.Methods: This research was used Descriptive Analyze Design. Population was Faculty of Nursing student Airlangga University stayed in Mulyorejo-Surabaya in Juni 2012. The samples were 50 taken with purposive sampling technique. Data were collected with questionnaire and were analyzed with spearman’s rho with significance level was p<0.05.Result: The result showed that some factors had significance correlation but some were not. The factors were had significance correlation were between stress and sleep quality (p=0.024), tired with sleep quality (p=0.001), illness with sleep quality (p=0.022). Some factors were had no correlation were between environment with sleep quality (p=0.497), diet with sleep quality (p=0,201), drug with sleep quality (p=0.731), and lifestyle with sleep quality (p=0.816).Discussion: stress, tired, and illness were dominant factorswhich were influence sleep quality for Faculty of Nursing student Airlangga University. Further research should occupied larger amount of sample to make more representative population.
