Abstract

Background: Laparoscopic cholecystectomy is the gold standard for cholecystectomy and can be performed by trained general surgeons. However, the percentage of complications that occur from cholecystectomy laparotomy is approximately 20 percent. So, it is necessary to research risk factors for complications. Methods: This research is an analytic observational study with a cross-sectional design using preoperative data and intraoperative findings of laparoscopic cholecystectomy. Age, gender, body mass index, history of cholecystitis, number of stones, and location of stones are identified as predictors that affect the difficulty of laparoscopic cholecystectomy. The data collection was performed at the Mohammad Hoesin Hospital Palembang Department from July 2019- July 2020. Results: Of the 67 research samples, a Chi-square test was performed, and the factors that had a significant relationship were the body mass index (P-Value 0,000), history of cholecystitis (P-Value 0,000), number of stones (P-Value 0.049), and stone location. (P-Value 0.002). Conclusion: The backward stepwise logistic regression using the four variables showed that the history of cholecystitis had an independent effect (OR-adjusted 28.889 and P-Value 0.000).

Abstract

Background: Decompression craniectomy is a surgical method used for immediate reduction of intracranial pressure. Repair of cranial bone defects to protect the brain and reconstruct the original cranial brain compartment is called a cranioplasty. Methods: In this study researchers will conduct research and compare storage of bone graft in the freezer with a temperature of -20 ° C and subcutaneous to the risk of infection. The research design used was paired clinical trials. Clinical tests were in open label form. Results: The analysis of significance using the Mann Whitnay test showed that the value of p = 0.381. This means that there is no significant difference between the storage group in the subcutie and the storage group in the Cryopreservation (p > 0.05). The analysis of significance using the McNemar test showed that the p value = 0.003. Conclusion: This means that there is a significant relationship between the storage group in the subcutie and the storage group in the Cryopreservation (p <0.05).

Abstract

Background: The breast cancer progression and chemotherapy side effects not only cause problems in mental status, but also the patient's physical status. This is related to a deteriorating quality of life and a worse prognosis. Physical exercise can improve physical status, mental status, and quality of life. Methods: Experimental pre- and post intervention research with a control group was conducted on breast cancer patients to determine the effect of progressive muscle relaxation on the improvement of body composition, depression status, and quality of life for breast cancer patients during chemotherapy at RSMH on February 2019 - February 2020. Results: Progressive muscle relaxation (PMR) can improve body composition (p = 0.000), reduce the degree of depression (p = 0.000), improve global health status (p = 0.013), physical functioning (p = 0.002), emotional functioning (p = 0.009), social functioning (p = 0015), body image (0.000), sexual function (p = 0.05), future perspective (p = 0.006), and decreased fatigue (p = 0.00), nausea and vomiting (p = 0.002 ), pain (0.000), insomnia (0.002), appetite loss (p = 0.000), breast symptoms (p = 0.000), arm symptoms (p = 0.000), hair loss (p = 0.000), and therapy side effects (p = 0.000). Conclusion: Progressive muscle relaxation has a significant effect on improving body composition, depression status, and quality of life for breast cancer patients during chemotherapy.

Abstract

Background: Hemodialysis is a therapy for end-stage cornic kidney disease (ESRD) which is most often used as renal replacement therapy. Vascular access for hemodialysis is essential in the management of patients with chronic kidney disease. One type of vascular access is av fistula. A well-matured AVF is needed in order to function optimally during the hemodialysis process. The study of Srivastava et al. In India examined a predictor parameter of the maturation of AVF using Doppler ultrasound. The parameters studied were Spiral Laminar Flow (SLF), Spectral Broadening and Murmur / thrill. The results of this study state that SLF is the most important and early predictor of AVF maturation.5 Research on the accuracy of spiral laminar flow, spectral broadening and murmur / thrill as predictors of av-shunt maturity has never been carried out in Indonesia. This encourages this research to be carried out. Methods: This type of research is the prognostic test. The study was conducted in the vascular and endovascular subdivisions from July - September 2020 or until the number of research samples is met. Results: There were 30 samples in this study, Murmur / thrill had high sensitivity and specificity values ​​on day 21 while SLF and spectral broadening had very high sensitivity and specificity values ​​in predicting AVF maturase from day 7th and 21th, also had a significant association with AVF maturase (p < 0.001). Conclusion: Murmur / thrill on 21th day can be a perdictor for maturity of AVF

Abstract

Background: Imperforated anus is a congenital abnormality which is better known as anorectal malformation. The incidence of anorectal malformations is 1 in 4000 to 5000 live births. The incidence of anorectal malformations in Europe varies from 1.14 to 5.96 per 10,000 people and can change each year.1-3 The surgical approach in the management of anorectal malformations depends on the classification of anorectal malformations based on low, intermediate and high location. The distal loopogram or distal colostrography / colostrogram examination is an examination that is very important to determine the location of the distal rectum before definitive repair, but this examination has the disadvantage of using contrast and x-ray radiation. One alternative radiological examination that can be used for faster initial management is sonographic.7 Methods: This study was a diagnostic test study to assess the diagnostic accuracy of the distal loopogram / colostrography examination and sonographic examination with the transperineal approachon post-colostomy Anorectal Malformations patients at Mohammad Hoesin Hospital, Palembang in August 2019-October 20120 or until the number of research samples is met. Results: There were 21 samples in this study, 11 (52.4%) samples were dominated by clinical appearance without fistula. On the results of ultrasound examination, the majority of samples of rectouretral fistulas were 7 (33.3%). Loopogram examination found the majority of samples without fistulas as many as 10 (47.6%) samples. On intraoperative examination, 7 (33.3%) samples were obtained for rectouretral fistulas. The result of pouch-perineum measurement on ultrasound examination was 1.15 ± 0.6 cm, loopogram was 0.7 ± 1.5 cm, intra-operasionem was 1.8 ± 1.6 cm. In this study, USG has a very high accuracy value with a sensitivity of 100% and a specificity of 93.8% while the Loopogram has a sensitivity of 100% and a specificity of 68.8%. Conclusion: Transperineal ultrasound has a very high accuracy value with a sensitivity of 100% and a specificity of 93.8% while the Loopogram has a sensitivity of 100% and a specificity of 68.8%. Although there is a significant difference between the distal rectum -perineal distance calculation with a difference of -0.38 ± 0.70 cm, these two methods are equally accurate in assessing the distal rectal pouch -perineal distance and the location of the fistula.

Abstract

Background: In 2018, breast cancer was the most common malignancies in women. Which main modality for the management is surgery. The most frequently-used incision design is Stewart incision. Its disadvantages were difficult to access axillary, surgical scar, lateral ear dog and the loss of anterior axillary fold. Another design used is Songket design which consists of crescent incision, rule of half buried mixed mattress suture technique and axillary anker suture. This study aims were to compare the total drain volume of post-MRM breast cancer patients between Songket and Stewart incision design. Method: Clinical randomized control trial (cRCT) research is the most robust design for evaluating the interventions used. Samples were all post-Modified Radical Mastectomy Ca Mammae patients with Songket and Stewart incision design who were hospitalized in the Department of Surgery at Dr. Mohammad Hoesin Hospital Palembang in the time of June - November 2019. The inclusion criteria were Ca Mammae patients who underwent the procedure and agreed to participate for this study. The patients who had post-operative wound dehiscence, under-documented drains, or the history of axillary lymph node surgery were excluded from this study. Results: Total drain volume for post-MRM using Stewart incision was 613.85 ± 215.93 ml, while Songket Incision stood was 116.15 ± 139.95 ml. The mean age of the study subjects was 48.69 ± 9.57. BMI was 23.65 ± 3.51 on average. Total drain volume of Stewart incision design was 497.7 ± 75.98 higher than Songket incision design. Conclusion: There was found significant difference in total drain volume between the two groups with p value of <0.001.

Abstract

Introduction: Hemangioma is a form of vascular anomaly that has various characteristics. Characteristics data, region, size of lesion, complications, and choice of treatment both operative and non-operative in hemangioma patients, especially at the RSUP Dr. Mohammad Hoesin, Palembang. This study aims to collect data and evaluate it. Methods: This study used secondary data which is medical records of all hemangioma cases treated at RSUP Dr. Mohammad Hoesin Palembang on January 1, 2019 to December 31, 2019. Data analysis used descriptive analysis, where the data that has been collected will be grouped and presented in the form of a frequency distribution table for each characteristic. Results: There were 61 medical records that meet the study criteria. The age at diagnosis was 2 (range 0-8) weeks. The number of female patients (n = 34; 55.74%) was found to be more than men (n = 27; 44.26%). Infantile hemangiomas (n = 39; 63.9%) were found to be more common than congenital hemangiomas (n = 22; 36.1%). Most of the hemangiomas were in the face (n = 36; 59%). The median size of hemangioma lesions in this study was 3 (range 0.5-10) cm. There were 18 hemangioma lesions that had complications (29.5%). The majority of hemangioma patients underwent operative management (n = 47; 77%), others underwent non-operative management (n = 14; 23%). Conclusion: Hemangiomas are found commonly in female patients. The location of the lesion is predominantly in the facial area. Nearly a third of hemangioma lesions have complications with the most common complication were ulcers. Hemangioma management was dominated by operative management which may be associated with a large number of hemangioma lesions in sensitive locations and complications.

Abstract

Background: Cancers with esterogen receptor negative (ER-) have a worse prognostic than estrogen receptor positive (ER +). Research by Cooper at al and Huiyan Ma et al have found strong association between history of hormonal contraceptive use and breast cancer with receptor negative estrogen (ER -). However, another study from Cotterchio et al found no difference. Methods: This study investigated breast cancer patients who went to dr. Moh Hoesin Palembang. This study involved 200 respondents with data obtained using patient immunohistochemical data and interviews using a questionnaire. This study uses a cross sectional analysis method. Data were obtained from the history of use of hormonal contraception, the type of hormonal contraception, and the status of hormonal receptors (Esterogen Receptors (ER) and Progesterone receptors (PR). 2x2 tables were used to obtain the Prevalence Ratio (PRR) and chi quare to obtain p values. Results: The results of the Chi square analysis showed that there was no significant relationship between the history of hormonal contraception and negative receptor expression with the prevalence ratio of hormonal contraceptives and ER (-) 1.48 times and PR (-) 1.43 times. There was no significant relationship between type of contraception and negative ER PR. The highest relative risk is in implant contraception with 1.54 times for ER (-) and 1.8 times for PR (-) Conclusion: There was no relationship between the history of hormonal contraception and negative hormonal receptors

Abstract

Introduction: Incidence of lung cancer highest in Indonesia and the fifth most abundant in female after breast cancer. Lung cancer is first cause of cancer related mortality in male (21.8%) and second cause of cancer related mortality in female (9.1%) after breast cancer (21.4%). The result of study in 100 hospitals in Jakarta showed that lung cancer was the most abundant in male and forth in female. The sputum cytology in lung cancer is an initial examination for diagnosis. The sputum cytology examination is a simple, accurate, cheap and non-invasive for initial diagnosis of lung disease including lung cancer. Methods: This study targeted lung malignancy suspect patient with sputum cytology examination in Dr. Mohammad Hoesin Hospital Palembang. The inclusion criteria were patients who will have a surgery and histopathology examination, patient >17 years old, patient that willingly take part in this research. The exclusion criteria were patient with sputum that can’t be retrieved. This study was using Cross Sectional method. The measurement of sensitivity and specificity of sputum cytology to histopathology examination calculated with Thornier-Remain formula. Results: The lung cancer patient found most in category of > 40 years (63.6%), male and smoking (77.3%). The sputum cytology’s result highest in non-malignancy (86.4%) and the histopathology examination’s result highest in malignancy (72.7%). Sensitivity of sputum cytology in this study is 28.75% and the specificity 100% Conclusion: The sensitivity of sputum cytology in 28.75% shown accuracy of sputum cytology method and specificity in 100% shown how often the sputum cytology method diagnoses the malignancy

Abstract

Introduction: Hydrocephalus may occur when the production of cerebrospinal fluid (CSF) is excessive or, the most common cause when, the absorption of CSF is decreased. The VP Shunt is most commonly performed worldwide as the primary treatment for hydrocephalus. The complications of VP Shunt are shunt infection, mechanical malfunction and over or under-drainage. This study aims to see the characteristics of patients affected by post VP shunt infection who are treated at dr. Mohammad Hoesin Palembang Methods: This research was a cross-sectional descriptive study. Using secondary data from the medical records of Mohammad Hoesin Hospital, Palembang. Performed in the from May 2018 to July 2019. Samples were all patients diagnosed with malfunction and infection with VP Shunt. Results: There were 23 subjects participated who met study critera. VP shunt infections were mostly male (82.6%). The largest age distribution is less than 1 year old (39.1%). The most incidence occured at the age of 1-6 months (60.9%). The most common type of surgery was emergency surgery (60.8%). The most common malfunctions were caused by shunt malfunctions (69.6%). Conclusion: Male patients, patients age <1 year old with onset of 1-6 months, and shunt malfunctons were the most common characteristic of patients diagnosed with malfunction and infection with VP Shunt.
