Abstract

Background: Osteoarthritis is a degenerative disease which attacks all the joint parts, including articular cartilage, subchondral bone, ligament, meniscus, capsule, synovium, and periarticular tissue. Among various joints in human body, knee joint is the most affected by osteoarthritis. There are several established risk factors for knee osteoarthritis, including age, female gender, and obesity.Aim: This study aimed to describe age, gender, and body mass index (BMI) profile in knee osteoarthritis patients.Material and methods: This was a retrospective descriptive study with total sampling of 292 medical records of patient with knee osteoarthritis from Physical Medicine and Rehabilitation outpatient clinic, Universitas Airlangga Hospital, Surabaya.Results: Of all the data, there were 130 medical records included in this study. The majority characteristic of the subjects were older than 60 years old (54.6%), 98 (75.4%) were females and 32 (24.6%) were males. The highest male-female ratio was on 45-59 years old (1:4). Most patients were obese (58.5%) with the highest percentage was on 45-59 years old (51.3%).Conclusion: The majority of osteoarthritis patients in Universitas Airlangga Hospital Surabaya were elderly, females, and obese people.

Abstract

ABSTRACTObjective: Stroke could cause upper extremity motor function disturbances that may affect to quality of life. The affected brain has an ability to get recovery by neuroplasticity process. Transcranial direct current stimulation (tDCS) is a non-invasive modality that could induce brain neuroplasticity Goal: To determine the effect of tDCS stimulation on recovery of upper extremity motor function in subacute ischaemic stroke patient. Methods: 22 stroke patients included in inclusion criteria, divided into 2 groups, control group and intervention group. Control group had occupational therapy for 5 days consecutively and intervention group had occupational therapy and tDCS stimulation simultaneously for 5 days consecutively. Upper extremity motor function was evaluated with Fugl-Meyer Assessment (FMA) before and after the treatment. Results: Significant improvement of FMA score on control group (p=0,018) and intervention group (p=0,000). Comparison of the result after the treatment revealed that the intervention group showed more significant improvement in FMA score than the control group (p=0,000). Conclusion: Application of tDCS stimulation on occupational therapy simultaneously showed improvement of upper extremity motor function in subacute ischaemic stroke patient better than only occupational therapy. Abstract

One of the basic goals of palliative care is to reduce the suffering of the patient, which includes relieving the pain. Some facts show that sometimes a doctor is afraid to give opioid analgesic mono-therapy to his patients because she is worried on one hand about the effect of addiction. In addition, in some cases, NSAID analgesics mono-therapy can also cause a series of side effects or even the patient may be contraindicated against the use of NSAID or paracetamol class drugs. Actually, this can be prevented by giving a combination of opioids and non-opioids analgesic. By providing this combination of therapy, it is hoped that this can accelerate the onset of work, extend the working period which can reduce the dose of drugs consumed, and more importantly minimize the side effects of each drug. The fixed dose combination of Tramadol-Paracetamol (Tramadol 37.5 mg–Paracetamol 325 mg) is an example of a drug combination preparation that has been shown to be effective in the treatment of pain. In this case report, a patient who is currently undergoing palliative therapy for cancer is described. On the way, the patient felt spinal pain as a result of the spread of the disease. The combination of Tramadol-paracetamol was administered twice a day with a dose that adjusted to the VAS Score observations before and after drug administration. And in this case, there was a decrease in the VAS score after being given Tramadol-Paracetamol combination.

Abstract

Carpal Tunnel Syndrome (CTS) is the most common entrapment neuropathy of median nerve. The current development of global COVID-19 pandemic urges medical rehabilitation services to deliver a comprehensive approach amidst the physical distancing period. This case report aims to investigate the feasibility and efficacy of implementing telerehabilitation to the conventional therapy in the treatment of CTS. A 51-year old female presented with post carpal tunnel release of the left hand, and chronic-severe CTS on right hand. We maintained 5 weeks rehabilitation programs of combined telerehabilitation and face-to-face rehabilitation therapy and evaluation. The patient showed good compliance with the programs and achieved the intended goals within 5 weeks of therapy. Patient showed improvement (in pain, range of motions, hand functions, and ADLs). Telerehabilitation combined with conventional therapy were effective and feasible in treating patient with bilateral (left post-release and right chronic-severe) CTS. The use of telerehabilitation may add accessibility while reducing physical contact in order to minimize the risk of transmission of COVID-19.

Abstract

Background: Six-minute walking test (6-MWT) is one of the submaximal exercise testing that is commonly used. The 6-MWT has limited use in the condition of individuals with limited limbs. Therefore, a six-minute arm ergometer test (6-MAT) submaximal exercise testing was developed. The presence of 6-MAT for assessing VO2max prediction is still relatively small.Aim: To compare the predicted VO2max of 6-MWT and 6-MAT, and to see if there are differences in each predicted VO2max value.Material and Methods: This was a cross-sectional study with a consecutive sampling, involving 31 untrained healthy young adult males, aged 26-40 years old. Each subject underwent two exercise testing 6-MWT and 6-MAT, which were carried out on different days with a minimum washout period of 24 hours. VO2max prediction value was evaluated for each exercise testing on each subject.Results: There are significant differences between the predicted VO2max values of 6-MWT and 6- MAT (p = 0.00). The mean VO2max prediction values of 6-MAT is higher than 6-MWT with 2288.43 mL.min-1 and 1573.72 mL.min-1, respectively.Conclusion: The 6-MAT could be used for assessing VO2max prediction of individuals, although, there is a difference value between the predicted VO2max of the 6-MWT and 6-MAT submaximal exercise testing, with the VO2max value of the 6-MAT higher than the 6-MWT submaximal exercise testing.

Abstract

Background: Weakness of upper extremity can affect the ability to perform daily activities in post-stroke patients. Power-Assisted Functional Electrical Stimulation (PAFES) may give motor reeducation and sensory feedback to improve motor recovery through neuroplasticity.Aim: To evaluate the effect of PAFES and occupational exercise on post-stroke patient hand dexterity and motor unit activity of extensor digitorum communis muscle.Material and methods: Twenty post-stroke patients enrolled in the study were divided intervention group whom received PAFES and occupational exercise, and control group whom received occupational exercise only. Each group underwent 30 minutes per session of treatment, 5 times a week, for 3 weeks. The Root Mean Square (RMS) for measuring motor unit activity of extensor digitorum communis muscle; Box and Block Test (BBT) and Nine Hole Peg Test (NHPT) for measuring hand dexterity were evaluated before and after intervention.Result: There were significant improvements of RMS (p<0.001), BBT (p<0.001) and NHPT (p=0.002) in intervention group after receiving PAFES and occupational exercise. The improvement of BBT in intervention group was significant compared with control group (p=0.028), however, no significant improvement of NHPT was found between groups.Conclusion: PAFES and occupational exercise could improve motor unit activity of the extensor digitorum communis muscle and hand dexterity in post-stroke patient.

Abstract

Background: Mechanical neck pain is defined as neck pain due to biomechanical dysfunction in the neck or upper back. Exercise can reduce muscle tension and strengthen deep cervical flexor (DCF) muscles by helping to relieve pain. The prevalence of neck pain is considered high in helicopter crews compared to the general population. Aim: To investigate the effectiveness of DCF exercise on neck functional scores (study of helicopter crews with mechanical neck pain).Material and Methods: Experimental one pre and post-test group design. The samples were 14 squadron-31/serbu helicopter crew performing conventional DCF muscle exercise 12 times for 4 weeks with a frequency of 3 times each week. Neck function was assessed using the Neck Outcome Score (NOOS) which consists of domains of mobility, stiffness, symptoms, sleep disturbances, everyday activity and pain, participating in everyday life, and quality of life.Results: This study showed that the mean NOOS score before intervention was 30.64 (SD: 9.44) and the mean NOOS score after intervention was 8.57 (SD: 5.33). There was a significant difference in the NOOS score before and after deep cervical flexor exercise (p = 0.001).Conclusion: DCF exercises are effective for improving neck functionality in helicopter crew with mechanical neck pain.

Abstract

Background: Fencing is one of many sports that are held in major sports events, both nationally and internationally. Fencing also has a high risk of injury, and knee injury is the most common injury site according to the authors' observations. Even so, studies on knee injuries in fencing athletes are very scarce, especially in Indonesia.Aim: To ascertain the prevalence of knee injuries and its intrinsic risk profile in fencing athletes at East Java Puslatda (regional training center).Material and Methods: The method used in this study was descriptive observational based on athletes’ health screening data, which were obtained from Sport Clinic Dr. Soetomo Academic General Hospital and athletes’ fitness data obtained from KONI.Results: This research involved 14 fencing athletes that were active in the East Java regional training center. Out of the 14 athletes who were included in this study, it was found that 1 athlete (7.1%) had a knee injury and 13 athletes (92.9%) had no knee injury. The athlete who had injured was a senior overweight female, had history of knee injury, average VO2max, normal knee ROM and alignment, and did 1-2 times leg strengthening exercises every week.Conclusion: The prevalence of knee injuries in fencing athletes at East Java regional training center is 1 in 14 athletes.

Abstract

Background: The American College of Sport Medicine (ACSM) recommends cardiorespiratory fitness training in people with diabetes mellitus (DM) for 3-7 days a week, total duration 150 minutes a week with moderate intensity using large muscle groups. Treadmill training is the best cardiorespiratory fitness training because it involves large muscle group compared to static cycle and arm crank.Aim: To determine the effect of moderate intensity treadmill exercise with gradual increase of speed and inclination on VO2max in men with T2DM.Material and Methods: Randomized group design in 22 men with T2DM, suitable with inclusion criteria was divided into 2 groups, the experimental and control group. The experimental group received moderate intensity treadmill exercise, 3 times a week with gradual increase of speed and inclination for 4 weeks. The control group underwent standard therapy. Measurement of VO2max was performed before and after program in both groups.Results: A significant increase of VO2max in the experimental group (p = 0.003). Compared to the control group, alteration of VO2max also significant among the experimental group (p = 0.000).Conclusion: Moderate intensity treadmill exercise with gradual increase on speed and inclination for 4 weeks increase VO2max in men with T2DM.

Abstract

The objective of this study is to report comprehensive physical medicine and rehabilitation management of post-palatoplasty in an adolescent patient. The cleft palate repair is usually done in the first year of life in order to gain optimal speech function. To achieve normal communication development, normal hearing, normal oral and pharyngeal structures, adequate stimulation, and reinforcement from the environment for communication efforts are needed. Patient was assessed comprehensively using The International Classification of Functioning, Disability and Health (ICF)  and by perceptual assessment for speech. The results showed that after one month intervention, the perceptions of hypernasality in plosive /b/ and affricates /c/ were found reduced significantly. There was improvement in articulation after one month of intervention.
