Abstract

Highlights: 1. There are differences in the result of the use of glucosamine and the increase of intraocular pressure.2. There are many other factors that may contribute to the increase in the intraocular pressure other than the use of glucosamine such as races, genetics, different dose, and duration of glucosamine use. Abstract: Background: Glucosamine is an amino monosaccharide that can directly stimulate the synthesis of glycosaminoglycans in the cartilage. It has been widely used as an osteoarthritis treatment. However, several literatures show the possible side effects of glucosamine, such as increased intraocular pressure (IOP). Objective: The objective of this study was to determine if there was any correlation between the use of glucosamine and the increase in IOP. Material and Method: This was a descriptive qualitative study that implied a systematic review design. The study sample consisted of patients with osteoarthritis (OA) and glaucoma in Iran, Indonesia, Thailand, the USA, and India between 2013 and 2018. The literature search was conducted on a database (PubMed and Google Scholar) and selected using inclusion and exclusion criteria. Discussion: The research identified 5 studies on the use of glucosamine and the increase of IOP. Two articles provide significant results on the correlation between the use of glucosamine and the increase of IOP (P < 0.05). In addition, two studies showed significant IOP reduction outcomes after discontinuation of glucosamine (P < 0.05). A case series indicated an increase in IOP during the 6th month of glucosamine use but still at normal value. Conclusion: Many other factors contribute to IOP growth, other than the use of glucosamine. Therefore, a large-scale randomized clinical trial or a multicentre cohort study using the same parameters is still needed to improve the quality of the subsequent systematic review

Abstract

Highlights: 1. The Child-Pugh score is an instrument to assess the degree of severity and prognosis of chronic liver disease. 2. the rise in APRI score may predict the advance of cirrhosis stages. Abstract: Background: Simple diagnostic and screening methods are needed to assist with liver biopsy, which is not always available in local health facilities. Objective: To assess the AST-to-Platelet Count Index (APRI) as a means to predict the advance of cirrhosis stages as indicated in Child-Pugh classification. Material and Method: This research employed an observational descriptive study design to examine laboratory values such as serum AST and platelet count, which were then compared to Child-Pugh profiles among cirrhosis patients at an inpatient facility in a tertiary hospital of Dr Soetomo General Academic Hospital, Surabaya, Indonesia on 1 January – 31 December 2015. The sampling method for this research was total sampling from medical histories. Result: Data obtained were then analyzed and tabulated using the descriptive technique. As many as 191 samples were obtained, consisting of male (N=129, 67.54%) and female patients (N=62, 32.46%) with a ratio of 2.08:1. APRI score showed an increase in age, with a peak frequency of 68 patients (35.6%) in the range 50-59 years old. Male patients had a higher average APRI score (2.11) than their female counterparts (1.93). Patients from Child C were the majority in this research, consisting of 105 patients (54.9%), followed by Child B (N=65, 34.03%) and A (N=21, 10.9%). Conclusion: The proportion of patients with APRI score > 1.5 is increasing along with the degree of severity according to the Child-Pugh classification. This indicates that the rise in APRI score may predict the advance of cirrhosis stages.

Highlights: 1. Most malaria infections found in East Kutai Regency were found within the male group and had high contact with forest areas.2. Malaria infections were found predominantly in the male group aged 25 to 34 years old.3. Most of the malaria infections found were caused by Plasmodium vivax. Abstract: Background: Malaria is endemic in Indonesia and knowing the factors that are involved in malaria infection will help the development of surveillance, prevention, and disease control. Objective: This study aimed to identify the profile of malaria infection and the vectors known in East Kutai Regency, Indonesia, in 2016. Material and Method: This research was a descriptive observational study with a cross-sectional design. The sample used was taken from the 2016 annual report of The Health Office, East Kutai Regency. We evaluated malaria indicators, disease distribution, incidence (age, sex, and occupation), plasmodium species found and known vectors. Result: In 2016, there were 55 confirmed cases of malaria infection in East Kutai. The Annual Parasite Index was 0.16 per 1,000. Malaria patients were dominated by males (85.5%), aged between 25-34 years old with a mean age of 29.56 years. Most of the patients lived in Rantau Pulung (41.8%). Most of the patients had work that had high contact with forest areas (70.9%). Most of the patients were infected with Plasmodium vivax (58.2%). Identified vectors were Anopheles balabacensis which was found in Kaliorang District. Conclusion: The East Kutai Regency was classified as Low Case Incidence Area with most cases found in males of 24 to 34 years old whose work had high contact with forest areas. The identified plasmodium species were Plasmodium vivax and Plasmodium falciparum while the identified vector was Anopheles balabacensis.

Highlights: 1. The major risk factors for type I endometrial cancer, namely age and obesity, play a major role in the increase in this case and are thought to be related to the grade in these cancer patients.2. Type I endometrial cancer patients at Dr. Soetomo General Academic Hospital 2019-2020 are dominated by patients diagnosed at the age of 56-65 years, having an overweight body mass index (23-24.9 kg/m2).3. The patient's age and body mass index did not correlate with endometrial cancer type I grade. Abstract Background: The increase in obesity and life expectancy has contributed to type I endometrial cancer cases worldwide. Increased risk factors play a role in the increase of these cases. Objectives: To determine the relationship between age and BMI with the cancer grade. Material and Method: This research was a cross-sectional study. The data were obtained from the patient’s medical records. The sampling technique was total sampling. The analysis used was Spearman Rho correlation test for the relationship between age and BMI with cancer grade. Results: This study recorded 54 patients with type I endometrial cancer in Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, from 2019 to 2020. The patients were from the age group 56-65 years 25 patients (46.30%), 46-55 years 17 patients (31.48%), 36-45 years 8 patients (14.81%), 26-35 years 2 patients (3.7%), and >65 years 2 patients (3.7%). For BMI, the patients were overweight (23-24.9 kg/m2) 21 patients (38.89%), normal (18.5-22.9 kg/m2) 14 patients (25.93%), obesity (25-29.9 kg/m2) 12 patients (22.22%), and obesity II (≥30 kg/m2) 7 patients (12.96%). For grade, grade III were 22 patients (40.75%), grade II  20 patients (37.04%), and grade I 12 patients (22.22%). There was a weak, insignificant positive correlation between age with grade (ρ=0.116, 0.405>α=0.05) and a weak, insignificant negative correlation between BMI with grade (ρ=-0.206, 0.135>α=0.05). Conclusion: A total of 54 patients with type I endometrial cancer at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, from 2019-2020 aged 56-65 years and overweight (23-24.9 kg/m2) with grade III cancer. The patient's age and BMI did not correlate with the patient's grade.

The administration of goat bile for 28 days had a toxic effect on the liver of the mice based on histopathological findings Abstract: Background: Bile consumption by Indonesians is believed to have therapeutic effects, especially goat bile. Goat bile is thought to contain harmful ingredients that can cause toxic effects on the liver. However, the 28-days oral toxicity study of goat bile has not been performed. Objective: To analyze the hepatotoxic effect of subchronic administration of goat bile on the liver of mice (Mus musculus). Material and Method: This was an experimental research with a post-test-only control group design. The samples used were 32 Balb/C mice (Mus musculus), which were grouped into 4 groups. The samples were administered with goat bile orally (3.2, 6.4, or 12.8 mL/kg/day) for 28 days. The liver was taken for histopathological examination and the hepatocytes injury score was performed. The scoring results were analyzed using Kruskal-Wallis, Mann-Whitney, and Spearman correlation tests (p<0.05). Result: Goat bile administration was associated with hepatocyte injury (p= 0.004). Groups with goat bile administration of 6.4 and 12.8 mL/kg/day had significant differences with the control group (p= .015 and .029 respectively) and the 3.2 mL/kg/day administered group (p= 0.006 and 0.009 respectively). Moreover, the increased administration of goat bile had a positive correlation with the level of hepatocyte injury (p= 0.004 and r_s= 0.504) Conclusion: Goat bile administration for 28 days had a significant toxic effect on the liver of mice at a dose of 6.4 mL/kg/day.

Highlights1. The occurrence of macrovascular and microvascular problems is strongly associated with high mortality in type 2 diabetes mellitus (T2DM).2. Combinations of rapid-acting and long-acting insulin were the most frequent kind of insulin. AbstractBackground: High mortality in type 2 diabetes mellitus (T2DM) has a strong correlation with the presence of macrovascular and microvascular complications. The risk of these complications can be reduced through good glycemic control. As the disease progresses, patients may need to add or switch to insulin therapy rather than oral antihyperglycemic drugs to achieve desirable glycemic control. Objective: To determine the pattern of insulin therapy and glycemic control in T2DM patients. Material and Method: Data for this study were obtained from the medical records of the Diabetes Outpatient Clinic at the Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. This study was an observational descriptive. In June-July 2016, total samples were collected from 124 patients who received insulin therapy between February 2015 and March 2015. Result: According to the data obtained, the most commonly used insulin was a combination of rapid-acting and long-acting insulin (75.8%). While basal bolus insulin (75.8%) accounts for the most common regimen used to administer insulin, only 27.4% and 45% of total patients achieved good glycemic control based on FPG and PPG testing, respectively. HbA1c testing was done on 44 out of 124 people, with 9 people (20.5%) met the target of HbA1c <7%. Conclusion: Glycemic control in T2DM patients who received insulin therapy was relatively low.

AbstractBackground: Heart failure preserved ejection fraction (HFpEF) is a complex disease associated with metabolic disease as a risk factor. A previous study found an association between iNOS and PDE-9 with inflammation and myocardial fibrosis in HFpEF. Objective: This study aimed to identify the potential phytochemicals of Crocus sativus (Saffron) that can inhibit protein iNOS and PDE-9 based on a molecular docking study. Material and Method: A total of fifty phytochemicals were obtained from Dr. Duke’s Phytochemical Database and IJAH IPB. Those phytochemicals were screened by using the PyRx application and followed with Lipinski’s Rule of Five screening by using SWISS Adme. Ten phytochemicals with lowest binding energy for each protein were docked and visually analyzed using Autodock 4.2 and BIOVIA Discovery Studio Visualizer 2016. Results: The best binding energy between protein-phytochemicals were -9.17 kcal/mol and -8.55 kcal/mol for iNOS and -9.17 kcal,/mol and -9.08 kcal/mol for PDE-9. Conclusion: Delphinidine and malvidin are the recommended inhibitors against iNOS and PDE-9 and must be investigated in further research.

Highlights1. In Indonesia, 18.8% of all suicides were the result of chemical poisoning, such as using hydrochloric acid (HCl).2. Hyperbaric Oxygen Therapy (HBOT) can be used as an emergency therapy for acute pathology or as a supplementary treatment for chronic illness. AbstractBackground: Suicide is the second most common cause of death and is a major public health problem in many countries globally. Chemical poisoning, such as hydrochloric acid (HCl), accounted for 18.8% of the total suicide cases in Indonesia. Through ingestion, this acid chemical contributes as the most common cause of swallowing injuries that can adversely affect the gastrointestinal mucosal through various pathological processes, primarily through an excessive inflammatory process. On the other hand, hyperbaric oxygen therapy (HBOT) has been widely used as a non-pharmacological therapy in many diseases, although its mechanism for reducing inflammation in HCl poisoning has remained unclear. Objective: This study aimed to provide a better understanding on hyperbaric oxygen's biomolecular mechanism as a potential adjuvant therapy in HCl poisoning. Discussion: HCl poisoning causes an excessive inflammatory process, leading to tissue hypoxia indicated by increased expression of Hypoxia Inducible Factor 1 (HIF-1). Hyperbaric oxygen therapy decreases the expression of HIF-1 through activation of the HIF-1 hydroxylation pathway via prolyl hydroxylase (PDH) in the proline pathway and HIF inhibiting factor (FIH) in the asparagine pathway. Reactivating both pathways will decrease HIF-1 activity, eventually reducing the ongoing inflammatory process. In addition, HBOT also plays a role in wound healing by stimulating angiogenesis growth factors. Conclusion: Hyperbaric oxygen therapy has the potential to be used as adjuvant therapy in HCl poisoning due to its beneficial effects on reducing inflammatory mediators and wound healing.

AbstractBackground: Type-1 diabetes mellitus (DM) is a chronic auto-immune disease characterized by insulin insufficiency and resultant hyperglycemia caused by the destruction of insulin-producing beta cells in the pancreas. Metabolic control in type-1 DM affects the metabolism of patients, one of which is growth status, which is influenced by growth hormone. Improved growth rates can indicate stable HbA1c levels. However, whether growth is affected by the degree of diabetic control is still debatable. Objective: This study was conducted to determine whether the metabolic control correlates with growth status of children with type-1 diabetes in the Endocrine Polyclinic at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. Material and Method: Retrospective observational analysis with a cross-sectional method was conducted on 96 type-1 DM patients at Dr. Soetomo General Academic Hospital Surabaya, Indonesia. The analysis was carried out using Chi-Square test to determine whether there was a correlation. Result: From a total of 96 pediatric patients, the majority was male (54.1%), patients with high-risk metabolic control were 79.2%, and those with z-score normal category were 62.5%. The results indicated no significant correlation between growth status and metabolic control of pediatric patients, with Chi-Square test results showed p=0.421 (p>0.05). Conclusion: There was no significant correlation between growth status and metabolic control of type-1 diabetes pediatric patients at the Endocrine Polyclinic Dr. Soetomo General Academic Hospital, Surabaya, Indonesia.

Abstract

Highlights The thoracic CT scan can be an alternative evaluation before administering EGFR-targeting medicine to patients. EGFR is now a therapeutic target for lung cancer.   Abstract Background: EGFR mutation is one of the molecular markers associated with prognosis and therapy in lung adenocarcinoma patients, including with lung metastases. The predominant imaging modality for assessing lung cancer is a chest CT scan, which correlates with prognosis. Objective: This study aimed to explore the association between characteristics of lung adenocarcinoma based on chest CT-Scan and EGFR mutation in lung adenocarcinoma with lung metastases. Material and Method: This study involved 65 lung adenocarcinoma patients in Dr. Moewardi Hospital, Surakarta, Indonesia, from January 2018-December 2020. Result: Chest CT-Scan and EGFR mutation were assessed in all subjects. EGFR mutation was found in 32 (49.8%) subjects. The risk of EGFR mutation was found to be higher in patients with lung adenocarcinoma who had lung metastases in a peripheral location (OR=11.56; 95%CI 3.57-37.33; p<0.001), tumor size <4 cm (OR=8.00; 95%CI 2.64-24.22; p<0.001), and semi-solid density (OR=5.21; 95%CI 1.79-15.19; p=0.002). Conclusion: EGFR mutation is associated with tumor location, size, and thickness based on chest CT-Scan in lung adenocarcinoma with lung metastases patients.
