Abstract

The key to the success of HIV / AIDS treatment is ARV therapy. Continuous therapy can inhibit the spread ofHIV infection in the body and improve the quality of life of patients. Lost to follow-up is the absence of thepatient to return to the VCT clinic according to the date of the drug agreement and consultation. The KencongCommunity Health Center found a problem regarding lost to follow up. The cause of lost to follow upsuspected due to factors that can be related to Lawrence Green's theory such as presdiposing factors(knowledge, attitudes), enabling factors (patient books and help cards), and reinforcing factors (motivation,SOP). The purpose of this research was to analyze the factors that can cause lost to follow up antiretroviral(ARV) treatment in HIV patients in Kencong Health Center. The type of this research was qualitative and thisresearch used 6 patients, 1 counselor and 1 doctor as respondent. The results of this research shows that theknowledge of patients is still lacking, especially way of virus transmission and the side effects caused and thelocation of the HIV virus in the body, meanwhile scheduling knowledge of patient is good. Respondents have apositive attitude towards ARV treatment. The book which use to record the patient’s visit data has beendeemed capable to control the arrival of patient. There are still many patients who don't carry a patient cardwhen they want to take medicine. Counselors and doctors have provided the best motivation during thecounseling process. Kencong Puskesmas still does not have SOP about lost to follow up

Abstract

Registration of loans and refunds of medical records at RSD Mangusda Badung still there is an obstacle that is the loan and refunds data of medical records in the period of 1x24 hours hours is not complete and, there is no description of loan and return time  of medical record documents in the expedition. This research aims to design and create information system for loans and refunds the  medical records by using waterfall method at RSD Mangusada Badung. This research was conducted in RSD Mangusada Badung. The method of this research used is a qualitative method with the waterfall system development method. The data collection methods used are interviews, observations and documentation. The result of this research is the design and creation of a loan information system for the return of medical record documents using waterfall method is expected to facilitate the officer filing at Puskesmas Banjarsengon to do their duties.

Abstract

Provision of medical record documents in the medical record research center Dr. Hospital CiptoMangunkusumo is still not in accordance with the number of requests needed, in October there were 4,836requests, 3,953 documents were available, in November there were 3,823 requests and 2,977 documentswere available, in December there were 4,151 requests and 3,143 documents were available. The purpose ofthis study was to analyze the absence of medical record documents in the research section. This type ofresearch is a qualitative study using interview, observation and documentation data collection techniquesinvolving 3 respondents. The results of the study showed that the cause of the absence of medical recorddocuments was that the officers' knowledge was still low, the attitude of the officers who were still lacking indiscipline, supporting facilities and infrastructure were lacking and there was no motivation. Improvementefforts to overcome these problems are providing training, disciplinary officers need to be improved, do systemdesign, provide awards and conduct monitoring and evaluation.

Abstract

National Center General Hospital Dr. Cipto Mangunkusumo is one of the hospitals whose services haveused Hospital Management Information System (SIMRS). The SIMRS used in RSCM is called the ElectronicHealth Record (EHR). In its application, there were still obstacles that made the staff less satisfied with theEHR, one of which was the loading of the EHR. The purpose of this study is to evaluate the EHR in termsof user satisfaction using the PIECES method (performance, information, economic, control, efficiency,service). This type of research is qualitative research, by describing the results of interviews andobservations that have been conducted by researchers and assessed by the PIECES method. Respondentsin this study were 1 filling officer, 1 reporting officer, 1 warehouse officer, and 1 expedition officer. The resultsobtained in this study are that the EHR performance has produced data according to user needs and thereis still a fairly long loading. EHR is able to provide quality information that is easily understood and has beenintegrated with BPJS. that the EHR can produce data according to user needs and the EHR still needs timeif the officer inputs or loads. The EHR has a use value because it is integrated with BPJS Health and alsohas an EHR officer or improvement team. EHR users feel that having an EHR makes it easy for EHR userswhen performing health services such as searching for patient files and making reports. The conclusion isthat the EHR makes it easy for users to do their work. Suggestions given by researchers are to the UMSI toupdate the EHR and perform maintenance on the system and hardware.

Abstract

The online registration system is important component to improve hospital service quality. Regional GeneralHospital K.R.M.T. Wongsonegoro City of Semarang has implemented online registration system since 2017and never been evaluated. An online registration system that is often used by patients is via the MYRSWNapplication, can be downloaded at the play store and app store. Online registration system users in 2019totaling 15,437 users from 199,188 patients. Based on data, it can be seen that users of the online registrationsystem are only 8% of the total number of patients. This shows the user's interest in using the onlineregistration system is very lacking. This research was aimed to analyze the acceptance of an onlineregistration system. The research showed that overview of the online registration system based on aspects ofperceived ease of use can be assessed as easy to learn, quite flexible and easy to use, aspects of perceivedusefulness is considered to have considerable benefits in increasing productivity, making work effective andfast, last aspects of behavioural intention to use tend to be satisfied using the online registration system.Based on aspects of perceived ease of use, perceived usefulness,and behavioural intention to use applicationof an online registration system has been running according to user needs, but it is not free from variousproblems, so there needs a efforts to improvement and development of online registration system through therecommendation of researchers so that online registration system can maintain quality and improve servicequality to patients.

Abstract

The assessment of the quality of medical record document information can help to find out the scores thatdetermine the level of quality of information produced by inpatient medical record documents in cases ofcerebral infarction. Based on preliminary studies, it has been carried out at Sleman District GeneralHospital. Yogyakarta on February 26, 2020 found a problem related to the contents of incomplete medicalrecord documents. This study aims to assess the quality of inpatient medical record document informationin the case of Cerebral Infarction in Sleman D.I.Yogyakarta Hospital. Descriptive method and crosssectional research design by observing 23 medical record documents taken from the sampling resultsusing the Slovin method to obtain a sample of 204 populations related to cases of cerebral infarction in theinpatient unit. Research results The assessment of the quality of information on medical record documentsuses four dimensions of product and service performance for information quality. Dimension assessmentwas considered good because of the results of 13 scores in filling out the inpatient medical recorddocuments that became the study sample. The contents of medical record documents which wereconsidered to be still poor were the contents of the clarity of the doctor's writing and the completeness ofthe name and signature of the care provider, one of which was at the DPJP.

Abstract

One of the obligations of the hospital is carrying out medical records. The implementation of medicalrecords at the Primasatya Husada Citra Surabaya Hospital has used an electronic system. The applicationis an Electronic Medical Record (ERM) with a website-based that runs since September 1, 2018. SinceSeptember 1, 2018, until now there has never been an evaluation of the application. This study aims toevaluate ERM using the Human Organization Technology (HOT) -Fit Model evaluation method. This typeof research is qualitative research with data collection methods using interviews, observation, anddocumentation. The results of the study indicated that the implementation of ERM has been going well,socialization and training have been provided by organizations, the presentation of data are not right, andcomputer facilities are still lacking. The researcher's suggestion for ERM at PHC Surabaya Hospital is totest the reliability and security of the ERM system, add one computing device, and improve thepresentation of data by the system to improve the quality of the ERM system.

Abstract

The number of outpatient medical records at RSUP dr. Soeradji Tirtonegoro has the highest late provisionthat occurred in June 2019 by 44.66%. The purpose of this report was to analyze the factors causing lateprovision of outpatient medical record files at RSUP dr. Soeradji Tirtonegoro Klaten. This type of researchwas qualitative. Data collection techniques used in this report were interviews and observation. The resultsshowed that the factors causing late provision of outpatient medical record files was man factors, whichare the indiscipline of filing staff in retrieving medical record files and the poly / ward officers' indiscipline inreturning medical records in a timely manner. Material factors were damage to the medical record file onthe map, missfile, emergency patient form without a folder, and the form has not been assembled. Themachine factor were the sudden power failure and the lack of borrowing and returning medical records onSINERGIS. The method factor was the unavailability of regular outpatients in the registration room. Themoney factor is that there is no financial support to give awards to officers. Improvement efforts that can bedone are filing officers not waiting for some tracers to pile up when retrieving files, evaluating the processof returning outpatient or inpatient medical record files to the filing room, always sweeping againstdamaged or non-bound documents. IT added features or menus for borrowing and returning medicalrecord files to SINERGIS. The registration officer installs a regular outpatient registration flow in theregistration room.

Abstract

Integrated Admision Information System (SISMIDU) is a system that serves to conduct bed managementdata in RSUD Dr. Saiful Anwar Malang. This system is a supporting facility to know the availability of bedinformation so that it can provide fast service in patients. The implementation of SISMIDU still has someobstacles that the system often encountered error in conducting patient data search, it takes a long time tosave data and system that is less update related to information availability of room in the inpatient area soas to cause inpatient buildup in the hospitalization of admission patient. The Purpose of this research is toevaluate the SISIMIDU based on user satisfaction. This type of research is qualitative research with datacollection through questionnaires. Method used is the End User Computing Satisfaction the methodconsisting aspects of content,aspects of accuracy, aspects of the format, aspects of timeliness and aspectof ease of use. Result of the research of the five aspects, among others, the aspect of content 67%respondents expressed satisfaction, aspect of accuracy 100% respondents expressed dissatisfied, aspectsof format 67% of respondents expressed dissatisfied, the aspect of timeliness 100% of respondentsexpressed dissatisfied and aspect of ease of use 100% expressed dissatisfied. It is hoped that hospitals canmake improvements and development against SISMIDU in order for SISMIDU to provide information thatsuits the needs of the user.

Abstract

The development information technology has a major influence on health services in meeting community'sneed for speed and accuracy of health services provided by hospitals. General registration counters areone place \ service for visitors who do not use state health insurance or from government as initial sourceservices provided by hospitals. Problems arising from general registration performance aspect is the timerequired while the service is in operation in operating the information system causes loading problems thatresult in the continuation of service.The aspect of general registration information system can not bridge orrun with other application systems such as applications provided by the Sidoarjo District Health Office. Inaspect of system control, differences in access for each user, but officers sometimes still lend users toother officers because each user has different access rights security still not guaranteed. Efficiency andService aspects, where features in general registration information system have made it easier for officersto operate easily but provide new features that must be learned by self-taught officers. As Service, thereare no modules or guidelines for operating a general patient registration information system. Datacollection methods with interview techniques and arragements. Data analysis by drawing conclusions thenanalyzing and giving advice. The purpose of this study to study general registration information system.Type of qualitative research. Respondents used 1 general registration officer. Results of this study notethat Service, Efficiency, Control, Economic, Information and Performance factors are related to use of ageneral registration information system.
