Abstract

A medical record is a file that contains notes and documents about patient identity, examination, treatment, actions, and other services that have been provided to patients. Medical records in carrying out maintenance and protection so that medical records are protected from damage and can facilitate the service process. This study discusses the process or activities of maintaining medical records at the Tarakan Hospital, Jakarta. The purpose of this study was to find out what factors could trigger the occurrence of damage or dangers to the medical record unit and to find out the preventive actions taken by the hospital in protecting medical records. This type of research uses a qualitative descriptive method, which describes the implementation of the maintenance of medical record files in the filing room in 2021. The implementation of medical record maintenance by medical record unit officers, it does not follow the SOP that has been set by Tarakan Hospital Jakarta. Constraints in maintaining medical records are seen from biological, chemical, environmental, and security factors. These obstacles are often the factors that cause damage to medical records such as dust, fire/coal on cigarettes, pests/insects, and chemical liquids and maintenance of medical records at the Tarakan Hospital.

Abstract

Many factors can cause iron deficiency anemia with iron loss during menstruation, especially in teenagers. This study was aimed to describe hemoglobin levels related to Fe supplementation in teenage girls in Batam. This study is descriptive and observational research. The research was conducted at SMAN 5 Batam from May to July 2021. The inclusion criteria for the participant in this study were girls aged 14 to 17 years old, willing to participate in this study, and signed the informed consent. A total of 47 girls participated in this study. Hemoglobin level was recorded before and after supplementation. Most of the participants were over 15 years old (91.5%) and had normal menstrual cycles. All participants had menstruated and had no history of infectious disease. The results of the Hb level examination showed that before the administration of Fe tablets, 95.7% of the participants had normal Hb levels. However, after Fe supplementation, the number of participants who had normal Hb levels decreased to 87.2% of the total participants who took part in this study. In conclusion, we found a decrease in the number of participants who had normal Hb levels after Fe supplementation.

Abstract

This study aims to determine the relationship between knowledge of postpartum mothers and the behavior of consuming vitamin A capsules in the working area of Tanjung Uncang Public Health Center, Batam city in 2021. This study used a cross sectional design. The research was conducted in September 2021 in the working area of the Tanjung Uncang Health Center UPT, Batam city. The sampling technique used is simple random sampling technique. Research respondents consisted of 85 mothers in the postpartum period. Most of the respondents have less knowledge (50.6%). Meanwhile, the behavior of consuming vitamin A during the puerperium was considered good (52.9%). Based on chi-square analysis, p value = 0.015 (p <0.05), so it can be concluded that there is a relationship between knowledge of postpartum mothers and behavior of consuming vitamin A capsules. In conclusion, most of the mothers during the postpartum period have less knowledge and consumption behavior. vitamin A during the good puerperium. There is a relationship between knowledge and behavior in consuming vitamin A capsules during the puerperium.

Abstract

This study aims to determine the description of initiation early breastfeeding and increased body temperature of newborns. This research is an observational study with a cross-sectional design. The study was conducted at the Suratmi midwife maternity clinic in Batam City. The study population was mothers who had just given birth from February to March 2022. The inclusion criteria in this study were mothers who had just given birth at the clinic, aged 20-40 years, and were willing to participate in the study. Sampling was done by the purposive sampling technique. The results showed that the early initiation of breastfeeding (EIB) was achieved by 36 respondents with an increase in temperature of 0.20C as many as 18 respondents (45%) and an increase in temperature of 0.30C by 4 respondents. In conclusion, most of the new mothers have achieved early initiation of breastfeeding and there is an increase in the baby's body temperature of 0.1-0.2OC.

Abstract

The risk of breast cancer increases with age, due to the progressive accumulation of genetic and epigenetic changes in breast epithelial cells as well as epigenetic changes in the microenvironment that lead to changes in gene expression. This study aims to determine the relationship between age risk factors and the incidence of breast cancer in Cut Meutia General Hospital, Northern Aceh. The research method used a cross sectional design which was carried out from June 2021 to January 2022. A total sample of 52 breast cancer patients were taken by total sampling that met the inclusion criteria. We found that patients with the most age characteristics are in the 41-50 age group with elementary education level, living in Aceh Utara and housewives. Based on the stage, it is known that the most dominant stage of the patient experiencing the incidence of breast cancer is stage III, which consist of 25 patients. Also, there is a significant relationship between age risk factors and the incidence of breast cancer in breast cancer patients in Cut Meutia General Hospital, Northern Aceh.

Abstract

Educating children as early as possible must start from the time the child is in the womb. Biologically human life begins at the time of conception or fertilization as well as human biological development. This study aims to determine the knowledge, attitudes, and behavior of pregnant women in providing stimulation to the fetus. This research is an observational study with a cross-sectional design. The population is pregnant women more than 20 weeks who check their pregnancy at the Baloi Permai Health Center in September-November 2021. The sampling technique uses purposive sampling with a sample of 48 people. Collecting data using a questionnaire and analyzing it using the chi-square test. The results showed that there was no relationship between knowledge and behavior of giving stimulation to the fetus (p=0.563); Likewise, there was no relationship between attitude and behavior in giving stimulation to the fetus (p=1.00). This study concludes that there is no relationship between knowledge and attitudes regarding the behavior of pregnant women in providing stimulation to the fetus.

Abstract

Coding activities include the coding of diagnosis and the coding of medical treatment. The important thing that must be considered by medical recorders is the accuracy in giving the diagnosis code. This study aims to determine the accuracy of the main diagnostic code for fractures in inpatients at Gatot Soebroto Army Hospital using ICD-10. This research is an observational study with a descriptive method. The study was conducted at the medical record unit of the Gatot Soebroto Army Central Hospital from October to December 2020. Sampling in this study was carried out using a systematic random sampling technique. The sample in this study was the medical record of patients with a fracture diagnosis at Gatot Soebroto Army Hospital. This study found that standard operating procedures (SOPs) for giving diagnosis codes in general according to ICD-10 at Gatot Soebroto Army Hospital already exist. However, the SOP code for fracture diagnosis is not yet available. Most of the documents (72.88%) had a diagnosis code that did not match the ICD-10. In conclusion, Gatot Soebroto Army Hospital does not yet have a special SOP regarding the diagnosis code for fracture cases. Most fracture diagnosis codes in medical record documents are incorrect. This is probably due to the lack of accuracy of the coder in coding the diagnosis, the absence of an evaluation or audit of the diagnosis coding for fracture cases, and the writing of the diagnosis by doctors being unclear and illegible.

Abstract

Breast cancer is the most common type of cancer in women and is one of the most common causes of death. Breast cancer can develop into malignant when the cell invades surrounding tissue or metastasizes (spreads) to other organs of the body. Several examinations can be done to monitor the growth to malignancy of breast cancer, such as histopathology examinations. The purpose of this study is to find out the location of metastasis based on the type and grading of histopathology in breast cancer patients at Cut Meutia Lhokseumawe Hospital in 2020-2021. This study was a descriptive observational study using a cross-sectional design. The study sample was breast cancer patients who had metastases and met the inclusion and exclusion criteria of as many as 36 people. The incidence of breast cancer metastasis mostly occurs at the age of 36-45 (38.9%), the most histopathological type was invasive ductal carcinoma (83.3%), and the most histopathological grading was grade III (52.8%). The most common site of metastases in this study was bone (50%). Invasive ductal carcinoma and invasive lobular carcinoma tend to metastasize to the bone. Grade II tends to metastasize to the bone. While grade III tends to metastasize to the lungs.

Abstract

Stunting is an overview of stunted growth as a result of a lack of nutrient intake in the long term. According to the WHO Child Growth Standard, stunting is based on the index of body length for age (PB/U) or height for age (TB/U) with a limit (z-score) of less than -2 SD. This study will describe the pattern of parenting and the incidence of stunting in toddlers with UPTD. Banjarangkan II Community Health Center. This research is an observational study using a qualitative approach with a cross-sectional study design. This research was conducted on 8 mothers of stunting toddlers in the UPT working area. Banjarangkan II Community Health Center The instruments used in this study was an in-depth interview guide and observations on feeding habits, parenting habits, hygiene habits, and habits of receiving health services, as well as measuring children's height. In this study, it was found that the parenting pattern of mothers with stunting toddlers was still not good, seen in the habits of feeding, caregiving, hygiene, and getting health services. It is necessary to increase a mother's knowledge about parenting, both from feeding habits, caregiving, hygiene, and getting health services.

Abstract

In this era of digital technology, development makes it easier for humans to do things such as seek treatment easily and practically. Registration application online still has drawbacks, namely bugs in the application, the queue paper printing machine does not work, and error system barcode. Online and offline registration. Descriptive method using quantitative analysis. This research was conducted at Gatot Soebroto Army Central Hospital with a sample of 45 offline and 45 online, and 7 registration officers. SOP only covers offline, for SOP online still does not exist. Most of the patients in offline and online registration stated that the quality of service, knowledge, experience, and satisfaction was good. Officers feel that there has been a change since the online registration because it has become a new thing in the surgical polyclinic because the officer at registration at the surgical polyclinic assumes that it is necessary to socialize with patients on the online application because the majority of patients do not register online because they do not know how to register online, especially patients Elderly officers are of the opinion that online registration makes it easier at the registration section.
