Abstract

The left ventricular aneurysm is one of the worst complications of acute myocardial infarction episodes. Fibrous tissue replacing dead myocardial cells in an infarcted area can’t perform contraction and herniate outward during systole. In turn, a left ventricle aneurysm will present devastating clinical outcomes such as heart failure, ventricular arrhythmias, even sudden cardiac death. A 65 year-old man came to our hospital complaining of worsened chest pain 5 days before admission, located on the left side of his chest. It was described to have a squeezing character. The symptom was inducted when he doing some mild activities and relieved with a short time rest. He had this symptom for one year. In the past, this symptom was inducted by heavier activities. He also complains of having shortness of breath for the past 2 months, the symptom was also relieved by resting. He had been admitted to the hospital for a heart attack twice before.. He has diabetes and hypertension, thus he consumed metformin 500 mg 3 times a day, and amlodipine 5 mg daily. He also had a history of kidney stone and prostate hypertrophy operation on March 2021. No remarkable abnormality was found in physical examination. On laboratory workup, there was a slight elevation of serum creatinine concentration. The troponin level was in the normal range. ECG and Echocardiography finding supported the ventricular aneurysm as one of the complications of acute myocardial infarction.

Abstract

As many as 59% of nurses work on the front lines during the covid-19 pandemic which is the largest proportion of health workers (WHO, 2020) Starting from the beginning of the pandemic until March 2021, 147 nurses in RSMH have been exposed to covid-19 (31.21%) and until now incidence is still high. The purpose of this study was to identify risk factors that influence the transmission of covid-19 in nurses at RSMH Palembang. The dependent variable was covid-19 incident on nurses and the independent variable were age, gender, work experience, work area, distance-keeping behavior and compliance with wearing PPE. This research used an analytic observation design with a retrospective cohort approach on 1196 nurses who participated in the RSMH covid-19 screening during the period March 2020-March 2021. The sample selection was carried out using a consecutive sampling technique and 1131 samples were obtained that met the inclusion criteria. Covid-19 occurred in 87 nurses (7.7%). The results of the bivariate test showed that there was an effect of gender (p=0.023), work area (p=0.026) and adherence to wearing PPE (p=0.001) on the incidence of Covid-19 in nurses. The results of the logistic regression test show that compliance with wearing PPE is the most influential factor on covid-19 with a probability of 87.92%. Hospitals must pay more attention to nurses who are at risk of being exposed to COVID-19, namely nurses who are male and work in red zone areas. Guaranteeing the availability of PPE and monitoring and evaluation of PPE compliance must be routinely carried out by hospital management.

Abstract

Thyroglossal duct cysts (TGDC) are usually located in the midline of the neck. A remnant of the thyroglossal duct, usually a cyst, is the most common congenital abnormality of thyroid gland development. This study aimed to describe a patient with TGDC and the microscopic appearance of the supporting examination. A woman, 22 years old, was admitted to the hospital with complaints of a lump in the submandibular area. Excision surgery was performed, with the result microscopic showing the cyst was composed of cuboidal epithelial cells and thyroid follicles present in the cyst wall, and some histological findings, such as; formation of papillary structures, nuclear morphologic variations such as ground glass nuclei, pseudoinclusions, intranuclear grooves. Based on the clinical symptoms and histopathology, the mass in the left submandibular is consistent with papillary carcinoma arising in thyroglossal duct cysts.

Abstract

COVID-19 infection may predispose thrombotic complications mediated by inflammation, endothelial dysfunction, and platelet activation. Several studies have reported that infection with COVID-19 is associated with both ischemic and hemorrhagic stroke. Stroke with COVID-19 Pneumonia is associated with increased morbidity and mortality. This study aimed to describe factors related to clinical outcomes in stroke patients comorbid with COVID-19 pneumonia. This study was an observational study with a cross-section approach. The population of this study is the medical records of stroke patients with pneumonia due to COVID-19 who were treated at Dr. Mohammad Hoesin General Hospital Palembang. Sample inclusion criteria in the form of medical record data for stroke patients with COVID-19 pneumonia and treated from January to June 2021. It was found that the most demographic characteristics were age (30-60 years), female, and comorbid hypertension. The most clinical characteristics were hemorrhagic stroke with severe NIHSS category and hypercoagulable conditions, and respiratory failure. From the bivariate analysis, it was found that the use of a ventilator was significantly associated with the incidence of death from stroke with COVID-19 pneumonia.

Abstract

The high risk of death for patients infected with COVID-19 has resulted in many stakeholders making policies and regulations related to COVID-19 in an effort to prevent the spread of this virus in Indonesia, starting from activating lockdowns, physical distancing to vaccinations in order to reduce morbidity and mortality rates. This study aimed to analyze various factors that could increase the risk of death of COVID-19 inpatients at Dr. Mohammad Hoesin General Hospital Palembang. The study uses the design of case-control by analyzing univariate, bivariate, and multivariates. The place of this research was carried out in Dr. Mohammad Hoesin General Hospital Palembang in the range of April until June 2020. It uses subjects based on study criteria. The result of bivariate analysis of the cause of death with independent variables based on data of COVID-19 claims in 2020. Statistically, there were some relationships among the gender (p-value=0,010), age (p-value=<0,0001), comorbid diseases (p-value=<0,0001), history of close contact with COVID-19 sufferers (p-value=0,006), length of stay (p-value=<0,0001). COVID-19 patients who have aged ≥50 years are at risk 2,74 to a higher risk of causing death than those aged <50 years after controlling for comorbid disease variables and length of stay (95% CI: 1,84-4.07). In conclusion, age is the dominant factor causing death. It can be used as a policy reference and additional information for readers. To apply the health protocols correctly and always consume nutritious food, especially for risky groups.

Abstract

The extracellular matrix is a complex and highly dynamic molecular network and serves to provide biochemical signals in the process of development, invasion, and metastasis of breast cancer. There are several proteins that contribute to the structure and function of the extracellular matrix, including collagen, laminin, glycosaminoglycans, and proteoglycans. Alterations of the extracellular matrix in breast cancer are not only limited to the composition of the extracellular matrix but also remodeling enzymes such as matrix metalloproteinase (MMP), cathepsin, urokinase plasminogen activator (uPA), lysyl oxidase (LOX) and heparanase. The extracellular matrix can increase breast cancer progressivity and resistance to treatment, so it is important to know its role because it can be a targeted therapy in the future.

Abstract

The COVID-19 pandemic has created a significant crisis for various groups, including nurses. Direct contact with COVID-19 patients increases the risk of infection, thereby can increase nurses’ anxiety due to the risk of transmission that may happen to their family members. This study aims to analyze the associated factors of nurses' anxiety levels in caring for COVID-19 patients. The design is an analytic study with a cross-sectional survey. The research sample was 201 nurses with purposive sampling in Covid and non-Covid wards at Dr. Mohammad Hoesin General Hospital from April to May 2021. The instrument was the Indonesian version of the Hamilton Anxiety Rating Scale (HAM-A). Data analysis was done using univariate, bivariate, and multivariate. Of the 201 nurses, 163 came from the non-Covid and 38 from the Covid wards. The levels of nurses' anxiety were mild (94%), mild-moderate (2.5%), moderate-severe (3%), and severe (0.5%). Associated factors were related to gender (p=0.039), living with the elderly (p=0.041), and being pregnant or breastfeeding (0.013) (p<0.05). Male respondents and being pregnant or breastfeeding showed the most significant relationship in influencing nurses' anxiety (p=0.048, p=0.017). There was no relationship between work units and nurses' anxiety in caring for COVID-19 patients (p=0.086; p>0.05). Gender factors and being pregnant or breastfeeding are the dominant factors in nurses' anxiety levels. The COVID-19 pandemic has been shown to affect nurses' emotional responses. Counseling is needed to reduce anxiety and prevent psychological problems during the pandemic.

Abstract

Osteogenesis imperfecta is a disorder of the formation of collagen tissue that functions as connective tissue and is caused by a gene mutation that causes disturbances in the formation of type 1 collagen. This study aimed to describe the diagnosis of osteogenesis imperfecta. A 23-year-old man came to the rheumatology polyclinic of Dr. Mohammad Hoesin General Hospital Palembang with a complaint of recurrent fractures since ± 17 years ago. The patient also complained that the hearing in the left ear was slowly decreasing. The patient's right thigh was still pinned 2 years ago, but after being controlled by orthopedics, it was said that the bones were still not fused. Then the patient was referred to the rheumatology polyclinic for further examination and management. On examination, the patient was 145 cm tall and weighed 40 kg. He had blue-gray sclera, triangular facial appearance, right leg length 86 cm, left leg length 78 cm, blue sclera, and scoliosis, while secondary sex growth was within normal limits. From the examinations of bone survey and bone age, it was found osteoporotic bone structure, scoliosis, plate-screw in the middle 1/3 of the right femur, fracture union of the femur, bowing of the left femur, BMD Z-Score -3.0 means very low compared to the same age and gender. Management is more focused on supportive therapy with the aim of minimizing the occurrence of fractures, minimizing disability, and helping people with osteogenesis imperfecta to be independent and maintain overall health. The goal of orthopedic management is to treat osteogenesis imperfecta with fractures and prevent or correct bone deformity. In conclusion, osteogenesis imperfecta is a complex hereditary disease characterized by striking clinical variability necessitating a logical classification system. Disease management requires multidisciplinary experts and further research on therapeutic approaches such as bisphosphonates.

Abstract

Background: COVID-19 is a pandemic that occurs in all parts of the world and can cause worsening health conditions and even death. Several factors are suspected to aggravate risk factors that can worse clinical conditions and cause death in COVID-19 patients. Risk factors such as age, gender, and the presence or absence of chronic disease are often associated with increased mortality in COVID-19 patients. This study aimed to determine the relationship between factors that affect the survival of COVID-19 patients, such as age, gender, diabetes mellitus, hypertension, heart disease, kidney disease, lung disease, smoking history, alcohol history, and obesity. Methods: An analytical description study of 66 participants was conducted for bivariate and multivariate analysis. Results: The results showed that aged more than 45 years(p=0,001; OR 6,111; 95% CI: 2.102-17.769), male (p=0,003; OR 5.417; 95% CI: 1.862-15.756), diabetes mellitus (p=0,000; OR 36.000; 95% CI: 7.194-180.151), heart disease (p=0,003; OR 9.286; 95% CI: 1.894-45.522), kidney disease (p=0,001; OR 8.953; 95% CI: 2.192-33.686) dan lung disease (p=0,002; OR 7.632; 95% CI: 1.943-29.971), smoker (p=0,003; OR 4.846; 95% CI: 1.683-13.955), dan obesity (p=0,005; OR 4.875; 95% CI: 1.600-14.855) influenced the patient's survival. Conclusion: Risk factors from age, gender, diabetes mellitus, heart disease, kidney disease, lung disease, smoking, alcoholism, and obesity influence the survival of confirmed COVID-19 patients. Among these factors, the most influential is diabetes mellitus.

Abstract

Meningitis is an infection of the meninges that has high mortality and morbidity. Magnesium is an enzyme co-factor that plays a role in cellular physiology. Administration of magnesium has a neuroprotective effect that potential to improve the clinical outcome of meningitis patients. An experimental knowing the effectiveness of oral magnesium on clinical outcomes in meningitis patients at RSUP Dr. Mohammad Hoesin General Hospital Palembang and this is a randomized control trial (RCT) using the add-on and double-blind method in meningitis patients by measuring the Bartel Index (BI) and Modified Rankin Scale (MRS) scales at RSMH for the period 1 September 2022 – 30 November 2022. There were 30 meningitis patients, 13 patients (43.3%) were dominated by tuberculous meningitis, and another types of meningitis were bacterial meningitis, cryptococcal/fungal meningitis and viral meningitis for 11 patients (36.7%), 4 patients (13, 3%) and 2 patients (6.7%) with female sex 16 patients (53.3%) and male 14 patients (46.7%). The difference of value between BI and MRS, the magnesium group is better than the placebo group with a comparison value of 49,333 and 45.33 for the Bartel Index and comparison value of 2,867 and 2,533 for the modified rankin scale. However, this difference was not statistically different with p value > 0.05. The conclusion is there was an increase in BI and MRS values in both the placebo and magnesium groups with the increase in BI and MRS values being higher in the magnesium group.
