Abstract

Kapok leaf (Ceiba pentandra (L.) Geartn) is a plant that contains several secondary metabolites in the form of alkaloids, flavonoids, tannins and saponins which have antibacterial properties. One of the pharmaceutical dosage forms that can be used to maintain healthy skin is antibacterial liquid bath soap. This study aims to determine whether kapok leaf extract (Ceiba pentandra (L.) Geartn) can be formulasited into a liquid bath soap that is physically-chemically stable and has antibacterial activity against Staphylococcus aureus bacteria. This type of research is an experimental study with a pretest and posttest only control design for testing the physical and chemical stability of the liquid bath soap formulasition and a posttest only control group design for testing the antibacterial activity of Staphylococcus aureus. In this study, there were 5 treatment groups, namely, F0 as a base without kapok leaf extract, F1 as a positive control of liquid lifebuoy soap, F2 as a preparation using 8% kapok leaf extract, F3 as a preparation of 9% kapok leaf extract and F4 using kapok leaf extract preparation 10%. The results of the research on liquid bath soap preparations from leaf extract of Kapok leaf (Ceiba pentandra (L.) Geartn) can be formulasited into liquid bath soap that has good stability from organoleptic tests, pH, viscosity, homogeneity, foam stability and free alkali with concentration antibacterial activity tests. 8% inhibition zone 15.8 mm, 9% inhibition zone 16.5 mm and 10% inhibition zone 18.1 mm. The formulasi for liquid bath soap with kapok leaf extract (Ceiba pentandra (L.) Geartn) can be formulasited with concentrations of 8%, 9% and 10% which are physically and chemically stable with strong antibacterial activity.

Abstract

In the provision of medical record files in hospitals, it is one indicator of patient satisfaction whose time has been determined in accordance with the hospital's minimum service standards, the standard for the time of providing medical record documents for outpatient services is less than or equal to 10 minutes.  10 minutes), while the time for providing inpatient medical record documents is less than or equal to 15 minutes (  15 minutes). The purpose of the study was to determine the length of time for providing outpatient medical records at Anna Medika Hospital Bekasi. The research method uses a descriptive method with a quantitative approach, the population of outpatient medical record files visiting the hospital, the sample studied is 106 files, through observation, interviews and time measurement records in the timely provision of medical records so that researchers get research results. The results of the study found that 59 (55.66%) medical records were provided on time and 47 (44.34%) medical records were not provided on time. Factors causing the length of time of provision > 10 minutes are errors in storing medical records, full storage shelves, lack of competence of medical record officers. Suggestions can improve the quality of service and the speed of time for providing medical records and holding standard operating procedures (spo) regarding the provision of medical records.

Abstract

The medical record storage system is very important to do in health care institutions, because it can make it easier for medical records to be stored in storage racks, speed up finding or retrieval and return, and protect medical records from theft, physical, chemical, and biological damage. The purpose of this study was to find out how the implementation of medical record storage in the filing room at Karang Mulya Health Center, Bojongmangu District, Bekasi Regency. The research method uses descriptive qualitative analysis method with the main informant being the head of the Puskesmas, other informants in charge of the medical record room, and medical record staff. The results of the study show that there is no standard operating procedure for storing medical records specifically for internal health centers, the results of the analysis of the storage space capacity can accommodate 30,000 medical records, and currently there are 16,319 medical records. The storage rack is still sufficient if there is a new medical record. The problem factor is in the filing room, the preparation of medical records is still stacked not in a row, and the medical records used are not immediately returned to the shelves, there has been no retention since 2017. Suggestions for standard operating procedures for storage procedures are made for internal health centers. Evaluate and improve the placement of medical records according to the applicable standards in the filing room.

Abstract

The number of deaths due to Covid-19 is increasing from time to time. The industrial era 4.0 is one of the eras of public information disclosure, therefore there are many media that can be used to support communication patterns and activities. This information disclosure will make it easier for citizens to carry out remote control and communication to monitor activities carried out by the government in dealing with this pandemic. This is one form of easy access to public information disclosure supported by information technology media. The purpose of this study was to determine the role of health information media in controlling the COVID-19 outbreak and to find out the most common way the media used to provide health information on Covid-19. This study was designed using a Literature review that examines the Role of Information Media in Handling Covid-19 cases. The results of a study of 8 journals found that health information media had several roles in handling Covid 19. The largest role was the media that played a role in educating the public about the importance of implementing health protocols and social distancing. It can be concluded that the role of the information media in handling the Covid-19 outbreak is very important, because the media are asked to always update data quickly, validly and in real time. The media are also asked to be able to break the chain of spread of Covid-19 by informing the travel ban, recommendations to comply with health protocols and recommendations to comply with social distancing procedures. distancing is the most common way.

Abstract

Boosting your immune system is one of the keys to preventing the spread of the Covid-19 virus. Consuming a balanced nutritious diet is one way to keep the immune system primed. However, we also need to increase the consumption of foods that contain nutrients that play an active role in increasing endurance, including vitamins A, C, E, B and D3. This study aims to determine public knowledge about the importance of consuming vitamins during the Covid-19 pandemic Salulayang Hamlet. The type of research used is descriptive with a quantitative approach, using a questionnaire instrument conducted in October-November 2021 with a population of 613 people with a sampling formula using the Slovin formula and a sample of 86 people. Based on the results of the study, it was found that the level of good knowledge was 73.3% while those who had poor knowledge were 26.7%. So it can be concluded that the knowledge of the people of Salulayang Hamlet about the importance of consuming vitamins during the Covid-19 pandemic can be categorized as good. The types of vitamins consumed by the people of Dusun Salulayang are vitamin C, vitamin B complex, and vitamin D. The most consumed vitamin is vitamin C.

Abstract

To maintain the security and confidentiality of patient medical records, a medical record storage room is required that meets the provisions in maintaining the security and confidentiality of medical records. This study aims to determine the security aspects in terms of physical, chemical and biological as well as the confidentiality aspects of medical records in the filing room. This study uses a literature review method with the PICO framework. Search articles using the Google Scholar database with keywords and Boolean operators in this study are "security" and "medical record confidentiality" and "filing". Research journal published in 2015–2020, in Indonesian, research design: using all research designs. The results of the study found security from the physical aspects that were most often found, namely the maintenance of the cleanliness of the filing room which was not maintained and the humidity temperature was less than optimal, the chemical aspect was that there were still officers eating and drinking in the filing room, the biological aspect of finding insects and pests. Meanwhile, from the aspect of confidentiality, there are officers other than medical records who enter the filing room.

Abstract

Informed consent is the consent that a patient gives to a doctor to perform a medical action after the patient receives a complete and understood explanation of the medical action. The purpose of this study is to find out the completeness of filling informed consent of inpatients at Hospital X South Jakarta, the type of research used is quantitative descriptive with a population of 353 and a sample of 78 inpatient medical records in the period October-December 2020. The method of data collection is Observation and Interview, following the results of research that has been conducted at Hospital X South Jakarta on 78 medical records known percentage average value based on 4 components, namely completeness 82% and incomplete 18%. Filling of patient identification components averages the percentage of completeness (89%), completeness of filling important report components 90%, completeness of filling the author authentication component (73%), completeness of filling of good note components (63%). Factors causing the incompleteness of informed consent are, the absence of standards of special operational procedures governing the filling of informed consent, doctors and nurses are busy due to the number of patients so that it is not fully filled, and the number of patients who do not have or bring witnesses / families when going to approve medical actions, this makes incompleteness in informed consent. With this, it is expected that the creation of SPO and socialization about filling the completeness of informed consent to be improved again.

Abstract

The medical record is an administration system that records all diagnoses and actions followed by the storage of medical records. Medical record retrieval is an important part to support the effectiveness of services in providing medical records for patients who return to the hospital. This research method uses quantitative descriptive and data collection techniques by observation, interviews and literature study. Based on the results of the research, the filing officer of the Islamic Hospital of Jakarta Sukapura often faced problems during retrieval, the results of the study found that 17 (3.4%) medical records were not found and 26 (5.2%) medical records were misplaced. Factors inhibiting the implementation of medical record retrieval include man factors such as the educational background of officers and the habitual factor of officers who do not use tracers when carrying out medical record retrieval that is not in accordance with SPO at the Islamic Hospital of Jakarta Sukapura. The money factor does not affect the implementation of medical record retrieval. The machine factor is the SMART system for medical record data entry that comes off the shelf. The method factor is that the standard operating procedure for retrieval of medical records is not fully appropriate. The material factor is the absence of loan receipts.

Abstract

Medical discharge is a summary of every action and treatment provided to the patient during hospitalization and must be signed by the DPJP doctor. The purpose of this study was to identify SOPs, determine the percentage of completeness of medical discharge and identify the factors causing the incompleteness. In this study, the method used is quantitative analysis method and data collection techniques are carried out through observation, quantitative analysis and interview guidelines, using the formula for estimating the proportion of the population of hospitalized patients in June and samples taken as many as 62 medical records. Based on the results of research and discussion, the completeness of the medical discharge at the Islamic Hospital of Jakarta Sukapura, the average percentage of completeness of the four components is 85.57% and the average incompleteness is 14.43%. Judging from the 4 components of quantitative analysis, the highest component of completeness analysis was found in the patient identification component 91.94% and the lowest was in the important note component 73.45%. Factors causing the incomplete filling of medical discharges are the absence of policies that regulate filling out medical discharges, the tight schedule of doctor's practice, and the lack of socialization of discipline in filling out medical discharges.

Abstract

The contents of the Medical Record are not only data on the treatment of sick patients, but also overall health data so that it is more accurately called Health Records. In general, the Health Record is an overview of the patient's health provided by the service provider/doctor to the patient to become the patient's health record. The purpose of this study was to determine the quality of good documentation in inpatients at Dharmais Cancer Hospital. The research design is cross sectional, namely research conducted at a certain time. Data sources: article searches conducted on Google Scholar to use articles that are in accordance with the research.research method Descriptiveis to describe directly the object under study using a quantitative approach. The results of the study obtained the number of completeness of medical records reached 89.13%. The sample obtained 92 medical record files, with the results of the Initial Medical Assessment Form getting a completeness score of 88.77%, CPPT Form 87.68%, Shift Handover 90.58%, and Consultation Sheets 89.49%. Medical Record Documentation still needs to be improved. Dharmais Cancer Hospital, the number of completeness of medical records needs to be increased so that the documentation of medical records is of higher quality.
