Journal of Nursing Science Update (JNSU) [formerly known as Jurnal Ilmu Keperawatan: Journal of Nursing Science] is a media to share scientific studies about nursing published by the Department of Nursing, Faculty of Health Science, Universitas Brawijaya. This journal covered basic nursing sciences, pediatric nursing, medical surgical nursing, maternity nursing, emergency nursing, mental health nursing, community nursing, family nursing, gerontic nursing, and nursing, and health policy. This journal is delimited into topics related to original research, reviews, case studies, and letter to the editor, which contribute to the development of the nursing scientific field. We aim to promote the development of nursing research into clinical evidence in practice. Our audience is the national and international nursing community, educators, health professionals, policymakers, and interested public members worldwide.

Abstract

Antiretroviral (ARV) therapy does not only provide some benefits but also has side effects that cause symptoms experienced by people with HIV (PWH). Strategies for effective symptom management were needed, especially during the COVID-19 pandemic. The purpose of this study was to explore the symptom experience and management strategies in PWH during the COVID-19 pandemic. A qualitative phenomenology design was used. A cross-sectional with purposive and snowball sampling techniques was used from July to October 2021. Eight participants joined an in-depth interview based on the criteria of the participants, such as people who were infected with HIV, 18 years or older, on ARV therapy, and willing to sign the informed consent. Thematic analysis was used after data saturation was reached. Three themes were found in this study; 1) The symptoms of ARV side effects, which included physical and psychological symptoms that appeared in PWH; 2) The symptom management of ARV side effects, which described the history of hospitalization, pharmacological and non-pharmacological management; 3) The symptom condition during the COVID-19 pandemic. The symptom experience of ARV side effects required strategies to manage the appearance of the symptoms, especially those amid the COVID-19 pandemic. This study could become fundamental evidence to consider in providing nursing care to PWH and supporting the symptom management model among PWH further.

Abstract

Covid-19 is becoming a global pandemic that presents new challenges in critical care. Visitation restrictions and isolation are major challenges in end-of-life care communication. The use of communication as an indicator of the quality of end-of-life care in the critical care management chain is required to adapt creatively and effectively. The aim of this research is to know the description of communication in end-of-life care in intensive care units during the covid-19 pandemic based on literature sources related to scientific research journals. The research was conducted by searching for research results published in 2019-2021 from databases such as PubMed, google scholar, and science direct using the keywords "communication", "end-of-life", "intensive care unit", and "pandemic". The search results selected twelve articles that met the PICO analysis for review. Twelve articles explain that adaptive communication can be carried out between patients, families, and the medical team through continuous video and telephone calls. Long-distance communication is carried out by following special rules, namely on time, coordinated by the entire team, and environmental and patient readiness, both in outward appearance and hemodynamic status. Communication adaptations in other forms are described in more detail in the communication guide checklist and written information sheets, as well as efforts to increase the role and number of nursing staff in large-scale communications. The use of adaptive communication positively can be beneficial in improving the quality of end-of-life care in intensive care units during a pandemic.

Abstract

A stroke is a neurological disease caused by the blockage or rupture of blood vessels in the brain. Functional dysfunction due to stroke causes stroke patients to be very susceptible to falls. This research aimed to identify the association between the severity of stroke with the level of patient dependence on the risk of falls. This research was a quantitative observational design with a cross-sectional approach. The sample of this study was 66 hospitalized stroke patients who were recruited by quota sampling method. The National Institute of Health Stroke Scale (NIHSS) and Barthel Index are used to measure stroke severity and level of dependence. We assessed the risk of falls using the operational procedure from the hospital. Based on the Spearman rho test, it is found that there is a significant relationship between stroke severity and the level of risk of falls (p=0.000394). Similarly, a significant correlation was found between the level of patient dependence and the level of risk of falls (0.000016). Nurses need to set the nursing care plan for stroke patients who are at risk of falls to prevent further injuries and complications. Continuity of care and family support will help the patient's dependency, reduce the risk of falls, and promote quality of life.

Abstract

The 7 “P” factors influencing the child-birth process are “Power”, ”Passage”, “Passenger”, “Psyche”, ”Pain”, “Patience”, and  “Position”. Psyche includes anxiety, which for laboring mothers creates tension, vasoconstriction in uterine blood vessels, pain, reduced uterine contraction, and delayed cervical opening so that childbirth takes longer. Long childbirth or partus can increase mothers' and babies' morbidity and mortality rates. This research aimed to identify the correlation between Knowledge Level and Anxiety Facing Childbirth in Primiparous Mothers at Panembahan Senopati Hospital Bantul. This study was quantitative research with a descriptive-analytic design using the cross-sectional approach. The population in this research consisted of 145 primipara mothers, with purposive sampling of 59 respondents. The data were collected with a questionnaire and analyzed with the Spearman Rank Correlation Test. The respondents’ knowledge level belonged to the ‘good’ category, 96.6%, while the anxiety level among the primipara mothers belonged to the ‘moderate’ category, 79.7%. The analysis of the correlation between the knowledge level and the anxiety level among Primipara mothers in Panembahan Senopati Hospital Bantul resulted in a p-value = 0.001 with a Rho-Spearman correlation value of -0,415. There was a moderate correlation between knowledge and anxiety levels among primipara mothers in Panembahan Senopati Hospital Bantul.  Healthcare providers can promote the seven essential power education models of childbirth ("Power," "Passage," "Passenger," "Psyche," "Position," "Pain," and "Patience") in maternity nursing practice so that nurses can pay attention to the complexities of care holistically for women who give birth

Abstract

Nursing is an important factor of all professional services, including Bio, Psycho, Socio, and Spiritual, which must be possessed because it is a crucial aspect of service, especially for the people we serve. This is because moral sensitivity and a strong sense of spirituality can assist someone in interpreting changes in their daily lives in light of God, the environment, and themselves. This study aims to determine the relationship between spirituality and moral sensitivity of undergraduate nursing students at STIKes Santa Elisabeth Medan T.A 2021/2022. We used a cross-sectional design. The sampling technique in this study used purposive sampling. The numbers of respondents are 358 respondents. The instrument used was a spirituality and moral sensitivity questionnaire for nursing students (MSQ-ST). The findings revealed great moral sensitivity (87.4%) and spirituality (82.4%). According to the findings of the Spearman Rank statistical test (rho), r = (.271) and p (value) = 0.000 (p 0.05) were obtained. These results indicate a weak relationship between the two variables with a positive direction, meaning that the higher the spirituality level of the students, the higher the moral sensitivity of nursing students at STIKes Santa Elisabeth Medan.

Abstract

The COVID-19 pandemic presents complex problems, especially in the health sector, especially for people with HIV-AIDS (PLWHA), who are one of the groups vulnerable to COVID-19 transmission. Psychological responses shown by PLWHA are anxiety and stress. Various factors allegedly affect stress in PLWHA. This study aimed to determine the factors influencing stress levels in PLWHA during the covid-19 pandemic. The research design is descriptive-analytic with a cross-sectional approach using primary data. The sample of 34 people living with HIV in the Kediri area was obtained by purposive sampling. Inclusion criteria were PLWHA aged 20-55 years and cooperative. Exclusion criteria included people living with HIV with advanced complications or severe opportunistic infections. The instrument measuring stress uses the Depression Anxiety Stress Scale 42 (DASS 42). Analysis of the Spearman rank correlation test data showed that there was no relationship between gender and age with stress levels (ρ-value = 0.428 > =0.05, -value = 0.131>α=0.05), and there was a relationship between education, knowledge , social support with stress levels in PLWHA in the Kediri area (ρ-value = 0.000 > =0.05, (r)=-600), -value = 0.000 < =0.05, (r)=-0.658 , p-value < d =0.05, (r)=-799). Respondents can actively increase knowledge related to COVID-19 through accountable information media and be open with families, communities, and health workers to get high support from families, communities and health workers as a priority for vulnerable groups in efforts to prevent COVID-19.

Abstract

Self-compassion is a mindset of accepting, understanding, and loving oneself in order to be capable of manipulating feelings positively, overcoming problems faced, decreasing anxiety, stress, and depression; and also being superb in knowing oneself when experiencing difficulties so as no longer to criticize oneself excessively and can enhance the exceptional of life, accordingly self-compassion is needed in nursing services in enhancing the excellent of nursing services. This study aims to identify self-compassion in nurses at Santa Elisabeth Hospital Medan in 2022. We used a descriptive method. The sampling technique is total sampling, and we have 196 respondents. This study uses a compassion scale questionnaire with 26 statements, consisting of 6 components, namely self-kindness, self-judgment, common humanity, isolation, mindfulness, and overidentification. The results obtained from the description of self-compassion of nurses at Santa Elisabeth Hospital Medan 2022. We found that in high category as many as 62.8%, and in low category as many as 37.2%. It can be said that the self-compassion of nurses at Santa Elisabeth Hospital Medan is high in treating themselves and others well and being able to understand one's shortcomings. We hope that nurses can maintain their self-compassion in service tasks.

Abstract

The rapid transmission of the Covid-19 virus, coupled with the circulation of social media information that cannot be verified, will impact stigma in the form of discriminatory behavior in society. This study aimed to determine the relationship between stigma and community discriminatory behavior in patients with COVID-19 at the Permata Bonang RT Perbon Tuban. This study uses a design with a Cross-sectional Study approach. The population used is part of the community in the Permata Bonang RT, with a total of 45 respondents and a sample of 40 using simple random sampling. The dependent variable is stigma, and the independent variable is discriminatory behavior. Data collection using a questionnaire sheet was analyzed using the chi-square test. The results showed that the positive stigma of society was 60% and the negative stigma of society was 40%;  as for the results of discriminatory community behavior that occurred, a small number of respondents had discriminatory behavior, namely 32.5%, and some respondents who did not discriminate, namely 67, 5%. So the results obtained in this study show a relationship between stigma and community discriminatory behavior in patients with COVID-19. The results of the chi-square test evidence this; the p-value is <0.05 (p-value = 0.000), so there is a relationship between stigma and discriminatory behavior in society. Based on the results of this study, respondents are advised to keep thinking positively by providing support and motivation for positive patients for Covid-19, increasing proper knowledge about education and information based on facts so that they can be wiser in responding to current conditions.

Abstract

Diabetes mellitus (DM) and hypertension are diseases that often appear together. DM patients with hypertension are at risk for microvascular and macrovascular complications. Self-care activities that are carried out regularly will improve the health condition of DM patients, thereby reducing the risk of complications. This study aims to determine the relationship between self-care activities and blood pressure in type II DM patients at the BPJS Kesehatan Palangka Raya Prolanis Club. The research design uses correlation analysis with a cross-sectional approach; the research population is all members of the Prolanis Club BPJS Kesehatan Palangka Raya. The number of samples was as many as 35 people with the purposive sampling technique. The research instrument used a questionnaire, The Summary of Diabetes Self-Care Activities Measure (SDSCA); the collected data was analyzed using the chi-square test. The results showed that as many as (51.4%) of diabetic patients had good self-care activity status, totaling 18 respondents. As many as 51.4% of respondents have normal blood pressure status. The statistical results of this study obtained p value = 0.028 (p <0.05), and r = 6.240, meaning that there is a relationship between self-care activities and blood pressure in type II DM patients. It means that the higher the self-care activity in diabetic patients, the blood pressure tends to be normal. Recommendations for further research are identifying the inhibiting factors in self-care activities for type II DM patients.

Abstract

Older adults are the group that has the lowest COVID-19 vaccination coverage of all age groups. Information about the safety and side effects of vaccines is a factor that causes older adults not to be vaccinated. Family as the closest support system for older adults is vital to achieving COVID-19 Vaccination. This study aimed to explore the various types of support families provide to older adults in the COVID-19 vaccination process based on published research articles. In this scoping review, the authors used four databases, Embase, Cinahl, Cochrane, Pubmed, and Google Scholar, like other reference sources, to obtain eleven articles discussing family support for the COVID-19 vaccination process. Results Based on the study using the PRISMA diagram, seven articles were eligible with the inclusion and exclusion criteria for this scoping review; from this article, it was found that the support provided by the family during the COVID-19 vaccination process in the older adults consisted of emotional, accessibility, and informational support. Providing information accompanied by solid motivation from the family is the key to the success of the COVID-19 vaccination process in older adults. Ease of accessibility is the main supporting factor beyond the motivation and information provided by the family.
