Abstract

National Health Insurance is a health program organized by the Social Security Organizing Agency with a prospective payment system or package system. Payment for health services with this package system is based on a diagnosis and procedure grouping that has similar resource needs. This study is an analytical study by examining differences in hospital tariff with final claims on the INA-CBG system. Data collection was carried out by observing 100 INA-CBGs claim files in five hospitals in Central Java Province, Indonesia. Data were analyzed by Paired Sample T-test. The results of the study showed that there was a significant difference with p value < 0.001. The total hospital tariff is IDR 582,373,996.00 compared to the total claim rate in the INA-CBG system of IDR 526,431,595.00, resulting in a negative difference of IDR 55,942,371.00. The difference in tariffs reached 9.6% of the total hospital tariff. The percentage of negative difference between hospital tariff and the tariff in the INA-CBGs system is 53%, slightly more than the 47% positive difference. Most hospitals already have efficient health service management by referring to the established clinical pathway. Financial management with cross subsidies between cases with positive and negative tariff differences can help hospital operations to continue to run well. Compliance of health workers with clinical pathways and effective and efficient management of health services can help hospitals overcome losses in the National Health Insurance system.

Old age has a higher risk for suffering from dementia. Although there is currently no cure for dementia, there are several modality therapies that can be applied to help deal with complaints that arise in dementia. This study aims to identify the effect of modality therapy on cognitive, physiological and psychological changes in dementia patients. A systematic review was conducted, we performed a systematic review on Science direct, PubMed, Ebsco, Web of Science, Cinahl, Research Gate database for relevant articles. Studies were included if the interventions with modality therapy were carried out in Dementia patients, both those treated at home and nursing homes. A total of 17 articles were included and 3 main categories emerged: cognitive, physiological, and psychological. Some therapeutic studio modalities can improve cognitive and physiological. Of these three categories, the category of psychological change is most often found. Psychological changes in depression, delusions and hallucinations (psychosis), agitation, apathy, aggression, sleep disturbance and disinhibition (inappropriate social relations and sections). The provision of modality therapy can help improve psychological problems of patients treated both at home and in nursing homes, and this modality therapy can be recommended in providing implementation and activities tailored to the patient's characteristics and controlling external factors to achieve therapeutic effectiveness.

Abstract

Less nutritious foods that are and poor food variations can cause a lack of nutrients causing health problems, one of which is anemia. Anemia causes lack of oxygen intake to body tissues, especially brain tissue. Lack of oxygen to the brain tissue among under-five children can result in decreased cognitive function and it can inhibit growth and psychomotor development. Anemia in children can also interfere with the immune system so the children are susceptible to infectious diseases. To determine the relationship between nutritional status and the incidence of anemia among children aged 6 months - 3 years. This was an observational analytic study with cross sectional approach. The study population was children aged 6 months - 3 years amounting to 30 children. The sampling used total sampling technique. The variabel of age of nutritional status was divided into 2 groups, namely good nutrition and undernutrition. It was said to be good nutrition if the nutritional status was in -2SD up to 2 SD and undernutrition if the nutritional status was from -3 SD to <-2SD. Furthermore, the variable of the incidence of anemia was divided into 2 groups, namely had anemia and did not have anemia. The child was said to had anemia if the hemoglobin level was <11 gr% and the child was said to did not have anemia if the hemoglobin level was ≥11 gr%. The instrument used to determine the nutritional status was a a Z-score table, while hemoglobin levels were measured using a haemometer using peripheral blood. Data analysis was performed using the Chi Square test. The results of analysis on the nutritional status showed that most of respondents namely 25 people (83.3%) had good nutritional status and 5 people had undernutrition status (16.7%), while the incidence of anemia showed that the majority of respondents were not anemic of 93.3% and 2 people were anemic (6.7%) Bivariate analysis showed that of all respondents with good nutritional status as many as 25 people, all were not anemic (83.3%); while of 5 people with undernutrition status, 3 people were not anemic (10%) and 2 people were anemic (6.7%), p=0.001<α=0.05 which indicated that there was a significant relationship between nutritional status and the incidence of anemia. There was a relationship between nutritional status and the incidence of anemia.
