Abstract

One of the stages of bottled drinking water production at PT X is bottle making using preform material. Preform material is made from PET (Polyethylen Terphtalate). During the production process, there are preform material rejects such as broken bottles, unstable bottle thickness, white preforms, pinched bottles, and white bottles. The total preform material rejects during January to October 2022 amounted to 0.98% or there were 234,504 pcs of preform rejects from the total production of 23,905,322 bottles. The percentage of preform material rejects is above the company standard of 0.5%, so it requires Six Sigma and FMEA methods to reduce these rejects. The proposed improvement recommendations are blowing machine operation checklist form, preform specification form for suppliers, and training schedule form. However, the recommendation that was implemented was the blowing machine operation form checklist because machine malfuction was the main factor causing preform material rejects based on FMEA analysis. The recommendation was implemented for one month in November and succeeded in reducing preform material rejects by an average of 0.86% or there were preform rejects of 298,641 pcs of preforms from a total production output of 34,599.74 bottles during January to November 2022. The final calculation after the implementation of the proposed improvements shows that there is a decrease in the DPMO value from 1961.93 to 1726.26 and an increase in the sigma level value of 4.42 sigma which was previously 4.38 sigma, from this value it shows that the sigma level has increased by 0.04 sigma. Abstract

PT Torabika Eka Semesta is a company producing healthy drinks and food. The existence of factories and various products certainly requires effectiveness and efficiency at work. The productivity level in the company is quite good, as seen from the achievement of the output target from January to December 2023 above 80%. The achievement of the output target can be caused by several things, such as off-production machines, delays in starting production, and output fluctuations. The purpose of doing productivity analysis is to discover the problems that affect productivity. The productivity analysis uses three methods: the Objective Matrix (OMAX), the Fault Tree Analysis (FTA), and the Analytical Hierarchy Process (AHP). The OMAX method is designed to calculate productivity values. The FTA method is used to identify how a problem can occur at the highest level in a system. The AHP method is used to calculate the weighting of each indicator. The results of the AHP method obtained values of 0.1888, 0.4121, 0.1252, 0.1728, 0.1011 for indicator 1 to indicator 5 respectively. Then, using the OMAX method, the productivity values of 1.640, 1.415, 6.665, 5.548, 2.371, 1.716, 2.227, 2.470, 2.089, 1.387, 1.591, 1.396 were obtained from January 2022 to January 2023. Based on the calculation results, it is known that defect/rework output, machine stop time and late start of production are indicators that have deficient performance. Implementation was performed by attaching a "Cello Replacement Instructions" sticker to the machine. The implementation results showed an increase in productivity value in January by 1.613.

Abstract

Sutindo Raya Mulia is a stockist company that purchases, stores, and resells it to consumers. PT Sutindo Raya Mulia is headquartered in the city of Surabaya and has branch offices in Jakarta, Semarang, Balikpapan, and Samarinda. One of the products offered by SRM is a stainless steel plate. Plate products have 3 series (S200, S300, and S400) and have different suppliers consisting of both local and international. Some of the weaknesses of the purchasing process can be caused by the selection of suppliers that are not fixed when purchasing products from suppliers. This is because the company does not have priority criteria in selecting suppliers. Priority criteria are determined using the Analytical Hierarchy Process method. Supplier assessment is determined using the Technique for Order Preference by Similarity to Ideal Solution method. The results of the AHP method obtained the weight and order of these criteria, price (0.261), availability (0.206), quality (0.200), position (0.106), quantity (0.102), MOQ (0.060), delivery (0.038), payment method (0.027). The results of the TOPSIS method obtained the value and sequence of suppliers, AB (0,587 - 21,0%); AC (0,764 - 27,3%); AD (0,565 - 20,2%); AE (0,490 - 17,5%); AF (0,397 - 14,1%).

Abstract

PT X is a company that engages in the production of large vehicles, particularly buses. The company currently has a sub-assembly department responsible for providing support materials used in the production process. Within the sub-assembly department, there are several constraints that affect the ongoing production process. These hindrances can lead to various losses for the company. Therefore, a study is being conducted to identify and understand the obstacles present in the sub-assembly department. The problem identification process employs the empathize stage of design thinking, aiming to determine the importance of each problem. To achieve this, a ranking system known as Failure Mode Effects and Analysis is being utilized. The results indicate the existence of six problems within the sub-assembly department. Following the prioritization process, it was determined that the primary issue revolves around inadequate administrative procedures. Based on these findings, an evaluation is planned for the company, specifically focusing on the sub-assembly department. The objective is to promptly address these problems and prevent any significant impacts on the company. Abstract

The Interior Department is one of the departments of PT X that produces bus and minibus interior parts. During the interior production process there are still quite a lot of defective products. There are six types of defects, namely air bubbles, not 100% printed, double tip insulation does not stick, torn, too dense/less dense, and color striped. The main focus of this research is to analyze the causes of failure of a process and make recommendations for improvements that can be made using the Fault Tree Analysis (FTA) method. From the FTA, improvements are needed to reduce defects in the Interior Department. Recommendations for improvement that can be given to the company are to create a checklist form to make it easier for operators to clean the mattress regularly, to check that the non-stick liquid has been sprayed twice, to ensure that the mattress has been heated every morning, to ensure that the paint has been shaken before use so that there are no striped colors, and to ensure that the paint has been sprayed twice. Then the next improvement recommendations are to provide and attach a standard description to each injection number on each product, make a machine maintenance schedule, replace the non-stick liquid with an old model that is not too strong, provide a timer on the mattress that is used as a reminder when waiting for the paint to dry, procure a mattress equipped with a sound sensor, and procure a generator.
