Abstract

Case study was conduct to a 30-year-old woman with a diagnose of Illness Anxiety Disorder. Subject complained of experiencing anxiety since being diagnosed with Fibroadenoma Mammae (FAM). She was not enough to be able to accept of doctor’s explanation regarding her condition of benign breast tumor caused by excess hormones from her birth control pill consumed. She always thinks of having a malignant breast tumor which make her dying process faster, this thought more repetitive and settled when she alone. She had medical history of surgical to remove her FAM in 2008. In 2010 she tried to check again, although declared by doctor it was clean, she still felt anxious about her condition. Psychological assessment involves of interviews, observation and psychological tests. The result can be drawn such require of intervention related with her cognitive distortion. The intervention given in this case was cognitive behavioral therapy aimed to reduce her anxiety while changing her cognitive distortion too which was done in 4 sessions. She reported about her progress and decreased of anxiety after therapy.Keywords:  cognitive behavior therapy, anxiety, FAM.

Abstract

Online games phenomenon that occur in Indonesia arises after internet access became easily as well as globalization around the world. Ignorance of problems that may arise quite late to realize that causing addiction problems due to the game has worldwide. In 2000 the number of Internet users in Indonesia reached two million people, this number increased to 71.190 million in 2014. When the game created an addiction condition, the game changed the balance in the neurotransmitters of the brain that play a role in the reward system, which would eventually change the structure of the nervous system anatomically and have an effect on changes in the regulation of emotions, cognition, and psychomotor. Clinical manifestations observed in a person with addiction games in the form of preoccupation in games; lying or hiding reality about the usual game; loss of interest and interest in other activities; withdrawal from social life; defensive and irritable. Treatment modalities that can be done include limiting screen time, support from parents or family, resting eyes and muscles, changing to educational games, diverting to more useful activities, academic guidance, family therapy, and comprehensive referral system should be made if the condition was permitted.Keywords: game, addiction, internet.

Abstract

Chronic Kidney Disease (CKD) is a health problem and has an effect on nearly 10% of the global population. Patients with end-stage renal failure will face complications in all organ systems and the most common psychiatric complications are depression, anxiety, dementia, delirium, coping difficulties and marital or family problems. Even the prevalence of depression is higher in patients with end-stage kidney disease than the general population. Depression in CKD patients has been shown to be associated with poor outcomes in CKD patients. The complex interactions between CKD and depression are dynamic and multifactorial, including socio-economic, lifestyle and biological factors. Biological factors that are closely related to the incidence of depression in CKD include the immune system, inflammatory pathway disorders of the Hipothalamus-Pituitary Axis (HPA) and changes in the sympathetic and parasympathetic nervous system. Inflammatory factors as a cause of depression have been identified with high concentrations of pro-inflammatory cytokines found in patients who are clinically depressed as well as patients with depressive symptoms, especially IL-1β, IL-6 and TNF-α. Furthermore, there is evidence to suggest that these pro-inflammatory cytokines interact with many pathophysiological mechanisms of depression, including neurotransmitter metabolism, neuroendocrine function, synaptic flexibility and behavior. Abstract

Divorce cases were dominated by marital conflict by partner disputes, miscommunication, economic problems, etc. The mediation process as one of the stages of the divorce process allows couples to discuss about their decisions. This will be followed by one of the couples come looking for help due to their psychological problems to psychiatrist or psychologist. Tracing the problem lies in the pattern of relationship structure and family intimacy that triggers marital conflict. Structural Family Therapy (SFT) is a psychotherapy modality that is expected to improve the structure of sub-system relations so that it appears as an effort to mediate marital conflict. This research will try to find out the results of Structural Family Therapy as an effort to mediate marital conflict through evaluation of the structure relationship and family intimacy.  This research was qualitative (case study) which conduct to 3 married couples, registered as patient at Psychiatry Clinic Universitas Brawijaya Hospital, research held for 20 weeks (December 2018 - April 2019). Evaluation from structure of the family area (family structure, system flexibility, resonance, life context, level of development and transaction patterns) and family intimacy. Post SFT evaluation, there was not only increasing of structure area and family intimacy in all cases but also slightly resolve of marital conflicts. This case study need to follow up in long term evaluation after SFT.Keywords:  structural family therapy, marital conflict, mediation.

Abstract

Child and adolescents with Attention Deficit Hyperactivity Disorder (ADHD) who experience game addiction are 3 times higher risk compared with other populations. Parents often report their inability to limit, regulate and supervise the use of games. The relationship between intentions in ADHD children with game addiction is reciprocal reaction. Wherein the symptoms of inattention trigger the emergence of game addiction and game addiction causes disruption of existing attentions to last longer. individuals with ADHD also have a high impulsivity and inability to think about the impact on something done. Impulsivity which is owned can cause difficulties in controlling excessive game which produces negative consequences. Impulsivity is the endophenotype underlying the relationship between ADHD and addiction. As such, effective interventions for impulsivity should be given immediately to adolescents with ADHD to reduce the risk of comorbid addiction games. Adolescents with ADHD have a higher hostility as well as those who experience game addiction so that the higher the hostility of adolescents with ADHD the more vulnerable. Hostility and impulsivity are important factors that clarify the comorbidities of ADHD and addiction. Pharmacotherapy treatment in ADHD and game addiction based on the underlying psychopathology is impulsivity, compulsivity as in the obsessive compulsive spectrum and based on high comorbidity in this disorder. Non-pharmacotherapy modalities in the form of CBT psychotherapy, METCBT, motivational interviews. Combination therapy becomes the basic modality for adequate management.Keywords:.ADHD, game addiction, impulsive.

Abstract

ABSTRACTCoronavirus Disease 2019 or COVID-19 first appeared at the end of 2019 in Wuhan, China, and spread across the world, and declared as pandemic at Mach 11th, 2020. Rapid increasing of cases and mortality rate accompanied by less understanding of the disease causing anxiety and fear among people. Conspiracy theory is a baseless belief that there are enormous and evil power planning for massive events for hidden and evil intention. Conspiracy theory associated with anxiety and lack of control faced by individual. Conspiracy theory is a mechanism to giving a simple explanation of a condition to rationalize an event, with the aim to make people feel safe and secure inside their environment, and giving feel of control their environment. Conspiracy theory also associated with paranoia, paranoid idea, schizotypal personality, and belief of paranormal phenomenon. Abstract

Sindroma Gilles de la Tourette adalah kelainan neurodevelopmental yang ditandai dengan tic motorik kronis dan vokal yang dimulai pada masa kanak-kanak. Etiologi gangguan ini bersifat kompleks dan multifaktorial dengan hasil akhir berupa anomali otak fungsional dan struktural serta keterlibatan sirkuit saraf cortico-striato-thalamo-cortical dan neurotransmitter. GTS dianggap bersifat poligenik dan dapat pula non-genetik, seperti faktor perinatal dan imunologis, yang berkontribusi pada manifestasinya yang heterogen. GTS umumnya memiliki onset sebelum usia 10 tahun, memberikan perjalanan klinis naik-turun, dengan puncak saat remaja dan umumnya remisi pada usia dewasa. Diagnosis berdasarkan DSM-5 ditegakkan melalui anamnesis, pemeriksaan fisik dan psikiatrik menggunakan instrumen penilaian tervalidasi. Tatalaksana berdasarkan klinis dan severitas, meliputi psikoedukasi, terapi perilaku, farmakologis hingga deep brain stimulation. Kata kunci: Sindroma Tourette, patofisiologi, manifestasi klinis, tatalaksana Gilles de la Tourette syndrome is a neurodevelopmental disorder characterized by chronic motor and vocal tics that develop in childhood. The etiology of this disorder is complex and multifactorial with the final result being functional and structural brain anomalies and the involvement of cortico-striato-thamo- cortical neural circuits and neurotransmitters. GTS is considered polygenic and also be non-genetic, such as perinatal events and immunological factors, which contribute to heterogeneous manifestations. GTS generally has onset before 10 years of age, exhibit a waxing and waning clinical course, with peaks in adolescence and generally remits in adulthood. Diagnosis based on DSM-5 is made through history taking, physical and psychiatric examination using validated assessment instruments. Management is based on clinical and severity, including psychoeducation, behavioral therapy, and pharmacology to deep brain stimulation. Abstract

Background           Among the pharmacotherapy recommendations for Tourette syndrome (TS), Aripiprazole is a potential treatment option for TS and a relatively safe alternative in the pediatric population. Case presentationWe report the case of a pediatric patient with TS who was admitted to the hospital after demonstrating severe involuntary movements. The symptoms emerged early in childhood and remained subclinical. It became more noticeable at 3 months before admission when the frequency and the severity started to increase progressively reaching its peak at one month before hospitalization. From further assessment, we found speech delay and a notably high score of Strength and Difficulties Questionnaire (SDQ)At the time of hospitalization, the symptom relieved following the admission of Aripiprazole 1 x 2 mg. It significantly diminished to infrequent minor facial tics. There was no adverse effect reported upon hospital discharge. We also performed family psychoeducation and speech therapy as adjuvant therapy.ConclusionsWe selected Aripiprazole because of its unique mechanism of action resulted in a dopamine stabilizer feature with less side effect. The most common ones are nausea, sedation, and increased appetite where, generally, are lessen within 2 weeks. Moreover, in contrast with other antipsychotics, Aripiprazole would not provoke hyperprolactinemia. The more severe side effect such as extrapyramidal symptoms and weight gain are usually dose-dependent at around 10 mg/day. Aripiprazole is also favourable, especially for refractory cases.

Abstract

Telemedicine is an emerging industry with the ability to revolutionize healthcare delivery for the benefit of consumers, providers and payors. In general, it is a common concept which is used to define various aspects of healthcare at a distance. Telemedicine has been revealed as one potential solutions to some of the medical dilemmas that are faced by many developing countries. In recent years, the burden of caregiving for mental illness service users has been alarming due to the lack of resources in mental health services. The problem of treatment for mental illness in Malaysia and Indonesia is now becoming more complicated with people who have different attitudes and views. Abstract

Depressive disorder is the main cause associated with recurrent morbidity which is often associated with suicides. Treatment of depression with antidepressant and electroconvulsive therapy is commonly used and proven effective. Based on previous study, administration of lithium as an augmentation therapy showed efficacy in suicidal behavior. The role of lithium as first line for bipolar disorder in manic episodes, also be an option in unipolar depression with some conditions such as antidepressant resistence, recurrent depression, maintenance therapy which is respond to electroconvulsive therapy and suicide behavioral. Methode : The authors conducted an extensive search of the published literature using several terms, including major depressive, lithium, antidepressant and antisuicidal effects. Relevan article were retrieved from PubMed, Cochrane, Springer, ScinceDirect. Conclusion: Lithium given along antidepressants has proven effective in treatment of unipolar depression, preventing relaps and also has anti-suicide effect.
