Abstract

A doctor's communication is a way to gather information in order to facilitate accurate diagnosis, give therapeutic instructions, counsel appropriately and establish caring relationships with patients. SMART (sophisticated, modern, affordable, reproductive technology) in vitro fertilization(IVF)created a mobile application that allows registered users to access all information related to their IVF cycle. The software required to create SMART application was the Android 4.0.3 Ice Cream Sandwich. The hardware required to create the SMART apps were 4.0 inch screen, 512 mb random-access memory (RAM), and central processing unit (CPU) with dual core 1.2 Ghz. The application is built using the android software development kit (SDK), the android apps creator. This application has been tested on several android devices such as asus zenfone 5 and samsung galaxy grand prime. This application uses android studio software to transfer the application file into the testing devices. SMART IVF application works well. The SMART application consists of some example features, such as feature doctors profile and information on how to make appointment with doctors.SMART app designed to help parents get directly involved during all the phases of IVF process and reduce their stress during the treatment and pregnancy. Since now this application not yet ready to launch, but hopefully in few months ahead, we can launch soon in google playstore as the first Indonesian IVF application.

Abstract

More than 50% patients graves' disease (GD) can relapse after remission period. This can be influenced by treatment, gender, age of diagnosis and genetic. One of the genes that regulate immune response is CD40 gene, which is found on the surface of Lymphocyte B. The aim of this research was to determine genetic variation at 5'-UTR of CD40 gene and the role of age and gender that influence the risk for relapse. This study was conducted in Dr Cipto Mangunkusumo Hospital from August to December 2014. This study was a case-control study comparing 30 relapse patients and 30 non-relapse patients after treatment with anti-thyroid drug was terminated. Genetic variation was analyzed with PCR-RFLP and clinical characteristic by medical record. In this research, we found that both genotype and alleles 5'-UTR of CD40 gene variation have no association with risk for relapse (p>0.05), but age of diagnosis was considered significant with risk for relapse (p=0.001). In conclusion, genetic variation in 5'-UTR CD40 gene is not risk for relapse but age of diagnosis may be a risk for relapse in GD patients.

Abstract

Extracorporeal shockwave lithotripsy (ESWL) is the most common method of ureteral stone management. Since 2008, RSCM has ben using ESWL piezolith 3000 richard wolf and efficacy quotient (EQ) value have not yet studied. The study aims was to determine the efficacy quotient (EQ) of ESWL using piezolith richard wolf 3000 machine for ureteral stone by analyzing free-stone rate with location of stones, number of stones, stone burden, stone opacity, obstruction and kidney function. This cross sectional study was carried out in January 2008-December 2011, with multivariate analytical study. Ninety five percent (n=113) of 119 patients were declared stone free after the first ESWL. EQ value was 0.89. Stone size was the correlated with stone free rate (p<0.05). It is concluded that ESWL procedure using richard wolf piezolith 3000 machine patients had better EQ and better stone-free rate than previous reports using similar machines.

Abstract

Oryza sativa L. indica or black rice is a food containing high level of polyphenol and anthocyanin that has potency as anticancer and prevent resistance of doxorubicin therapy as a result of phosphatydil-inositol-3-kinase (PI3K) hyperactivation. This study aimed to analyze the potency of black rice to inhibit the PI3K pathway. The study was done on March-June 2016 at faculty of veterinary Universitas Airlangga using 30 female Sprague dawley rats aged 7-8 weeks. Breast cancer induced by 7,12- dimethylbenz(&alpha;)anthracene(DMBA) and estrogen. Rats were divided into three groups: K1 (placebo), K2 (doxorubicin), and P (doxorubicin and black rice ethanol extract). Black rice ethanol extract was given with dose of 75mg/kg weight/day for six weeks. Parameters used in this study were the activity of PKB and CA 15-3 serum level, The results showed that there was significant difference in activity of PKB and CA 15-3 serum level between the groups (p < 0.05). The group of rats that treated by combination doxorubicin and black rice ethanol extract had the lowest activity of PKB and CA 15-3 serum level among the other groups. The conclusion of the study is that black rice ethanol extract can decrease the activity of PKB and CA 15-3 serum level in white rat modelled breast cancer.

Abstract

Poliomielitis adalah penyakit menular yangditandai dengan kelumpuhan akibat kerusakanmotor neuron di kornu anterior sumsum tulangbelakang; disebabkan oleh tiga serotipe virus polioyaitu serotipe 1 (brunhilde), serotipe 2 (lansig) danserotipe 3 (leon).1 Poliomielitis ditularkan secarafekal-oral atau oral-oral.Sebelum vaksin polio ditemukan, semua anakyang terinfeksi virus polio dan sekitar 1 dari 200anak yang terinfeksi akan menderita kelumpuhan.Setelah ditemukan vaksin polio inaktivasi (IPV,salk) pada tahun 1955, vaksin polio oral monovalen(mOPV, sabin) tahun 1961 dan vaksin polio oraltrivalen (tOPV) pada tahun 1963, program imunisasipolio berlangsung di seluruh dunia. Vaksin IPVdiganti dengan tOPV karena pemberiannyamudah, lebih unggul dalam merangsang kekebalanmukosa usus, dan lebih murah. Vaksin tOPV masukdalam Program Pengembangan Imunisasi/PPI diIndonesia sejak tahun 1978.

Abstract

Loss of E-cadherin and increased vimentin expression are associated with epithelial-mesenchymal transition andcancer stemness which are responsible for treatment resistance. The study aims to evaluate the role of E-cadherin andvimentin as predictors of resistance to preoperative systemic therapy in patients with advanced breast cancer. This wasa cross-sectional analytical study in patients with stage III-IV breast cancer in Dharmais Cancer Hospital and dr. CiptoMangunkusumo National Hospital from July 2015 to April 2016. Patients had biopsy specimens embedded in paraffinblocks. Expressions of E-cadherin and vimentin proteins were done immunohistochemically. Treatment response wasevaluated histopathologically using Miller-Payne criteria on mastectomy specimens. A total of 65 patients were enrolled.Five patients with invasive lobular carcinoma were excluded. Thirty one had chemotherapy and 29 had hormonaltherapy. After treatment, 46 patients were eligible for mastectomy. E-cadherin and vimentin were positive in 28 (60.9%)and 11 (20.3%) of specimens. Twenty-three (50%) patients showed no response. Treatment resistance were associatedwith type of therapy (OR=4.4; 95% CI=1.27-15.41; p=0.017) and vimentin expression (OR=6.75; 95% CI=1.27-30.02;p=0.016). Hormonal therapy (ORadj=6.26; 95%CI=1.59-24.6; p=0.009) and positive vimentin (ORadj=8.75; 95%CI=1.43-57.4; p=0.019) were independent predictors of treatment resistance. In conclusion, E-cadherin and vimentin can beused as predictors of resistance to preoperative systemic therapy in patients with advanced breast cancer.

Abstract

Rabies is widely known as disease with high mortality rate. We conducted this study to assess level ofknowledge, attitude and practice regarding rabies in Manggarai regency, Nusa Tenggara Timur. This study wasconducted using cross sectional design. The knowledge, attitude and practice of subjects were determinedusing validated questionnaire. We gathered the data from primary health care located in 9 Manggarai districton July 2014. Out of 278 respondents, 65.8% have poor level of knowledge while 34.2% have good level ofknowledge. Its only 18.3% have poor level of attitude while 81.7% have moderate level of attitude. The 53.2%resident have poor practice related to rabies and 46.8% have moderate level of practice. Level of educationand place of residence are shown to have significant relationship with level of knowledge, attitude and practicetoward rabies. Moreover, subjects with adequate knowledge of rabies tend to have better practices againstthe disease itself (PR=2,391;CI95% 1,879-3,043;p<0,001). Raising level of knowledge of the communityregarding rabies is crucial to improve the practices against the disease.

Abstract

Heart failure (HF) with atrial fibrillation (AF) is correlated with worse prognosis requiring special approach.Rate control has been the first line of treatment in cases of HF and HF. On the other hand, rhythm controlhas been proven to be effective in returning sinus rhythm resulting in better prognosis for patients with HFbut not HF. Its role in cocurring cases of HF and AF is not fully understood. Thus, this study aims to analysewhether pharmacologic rhythm control can be applied to cases of HF and AF to reduce mortality. A searchwas conducted via PubMed, Medline, ProQuest, and Cochrane Database on January 2016. One study wasselected after filtering process by inclusion and exclusion criteria and critical appraisal was performed. It wasfound that there was rhythm control and rate control do no have favouring effect towards mortality shown byRR 1.03 (95% CI 0.90-1.17, p=0.69). Rate control has protective effect towards hospitalizations by RR of 0.92(95% CI 0.86 &ndash; 0.98, p=0.008, NNT=19). To conclude, rhythm control is not superior to rate control in reducingmortality and rate control should be still be considered as first line treatment of HF and AF.

Abstract

The advancement of molecular biology provides benefit in the field of medicine to develop genetherapy. The aim of gene therapy is to repair the genetic damage or to replace damaged gene with thenormal gene. Delivery of gene is carried out by transfection technique, a technique to transfer nucleic acidinto eukaryote cells either using viral vectors (known as transduction), and also using non viral methodsuch as chemical substance, lipid and physical method. Some of the viral vectors used in the transductionare retrovirus, adenovirus, Adeno-associated virus (AAV) and Herpes Simplex Virus (HSV). The success oftransfection is determined by various factors which can be assessed using several reporters such as GreenFluorescence Protein (GFP).

Abstract

Nasopharyngeal carcinoma (NPC) is the most common head and neck malignancy in Indonesia. NPCpresents numerous challenges from non-specific signs and symptoms until lack of awareness from generalpractitioners (GP), which lead to late or missed diagnosis. Early diagnosis and prevention are proposedas the best solutions for this problem. In order to do that, we need a complete and well-managed patients&rsquo;data. This study aims to reveal the demographic, clinical and histopathologic characteristics of NPC inIndonesia. A cross-sectional study was conducted by collecting medical records of all NPC patients in 2010from Otorhinolaryngology Department of Dr. Cipto Mangunkusumo National Hospital. The extracted datawere then analyzed to describe the problem of NPC in Indonesia. Out of 167 patients, 68.3% of the patientsare male and most of them are Sundanese and Javanese. Palpable lump in the neck is the most commoncomplaint in presentation (58.1%), followed by nasal congestion (49.1%). Salted fish consumption was themost prevalent risk factor (29.9%). Based on the histopathologic findings, 75.4% of the cases were classifiedas WHO-3 and around half of the patients (51%) were in stage IV upon diagnosis. Studying demographic andclinical characteristics of NPC patients is the first step to overcome problems caused by NPC in Indonesia.
