Abstract

Asthma is a heterogeneous disease with chronic airway inflammation that still be a concern in the world. GINA 2021 explain that if asthma patients are not continuously controlled, it is necessary to review common problems that frequently found, one of which is the wrong inhaler technique. The incorrect of inhaler techniques will have an effect on drug delivery to the lungs which affects the success of therapy and improvement of lung function. To determine the relationship between the accuracy of inhalers technique with the results of the spirometry test in asthmatic patients.This study used a quantitative research with an analytical survey method and a cross-sectional study approach. The study was conducted by observation to see the correlation between the accuracy of the inhaler technique and the results of the spirometry test with parameters FEV1. Sampling was carried out at the Harum Melati Clinic and Wisma Rini Hospital, Pringsewu. There were 173 asthma patients that using inhaler, but only 103 samples met the inclusion criteria, namely patients aged >5 years who used inhalers and taken spirometry examinations. In this study, the inhaler accuracy test was carried out using a checklist 2 times where the first was carried out before education of inhalers technique and the second was carried out after education. Spearmen correlation test showed that there was no relationship between inhaler technique and FEV1 before education (p-Value 0.941; p>0.05), but there is a significant relationship between the technique of using the inhaler with the results of FEV1 after education (p-Value 0.049; p < 0.05).

Abstract

Acne vulgaris is a disorder limited to the visible pilosebaceous unit. Acne presents with a pleomorphic arrangement. lesions consisting of comedones, papules, pustules, and nodules of varying extent and severity. While the course of acne may be self-limiting, the sequelae can be lifelong, with scar formation that is difficult to go away. Benzoyl peroxide is an effective topical agent for the treatment of acne vulgaris. The way it works can penetrate the stratum corneum or follicular openings unchanged and is converted metabolically to benzoic acid in the epidermis and dermis. Approximately 5% of the applied dose is absorbed from the skin in an 8 hour period. It has been postulated that the mechanism of action of benzoyl peroxide in acne is related to its antimicrobial activity against P. acnes and to its exfoliating and comedolytic effects.

Abstract

Diabetes mellitus occurs due to disturbances in the body's metabolism that causes hyperglycemia and is associated with abnormalities of work and/or insulin secretion. One of the complications of type 2 diabetes mellitus is diabetic nephropathy which can be diagnosed early by measuring creatinine levels. Correlative analytic research with cross sectional method uses purposive sampling technique. The study sample consisted of 30 patients with type 2 diabetes with HbA1c levels <7% percentage (26.7%) and HbA1c levels 7% percentage (73.3%). For serum creatinine levels, serum creatinine values ​​were <1.1mg/dl percentage (23.3%) and serum creatinine values ​​1.1mg/dl percentage (76.7%). The results of the chi square test showed p value = 0.037 and odd ratio value (CI 95%) 6.3333 because the p value = 0.037 < 0.05 there was a relationship between HbA1c levels and serum creatinine levels in patients with type 2 diabetes. Odd ratio 6.3333 which means high HbA1c levels height 6.3 times higher is a factor in the increase in serum creatinine levels in patients with type 2 diabetes at Pertamina Bintang Amin Husada Hospital Bandar Lampung in 2022.

Abstract

Sepsis is a life-threatening condition of organ dysfunction due to dysregulation of the host response to infection. Antibiotic therapy is the main treatment for sepsis patients. The high rate of antibiotic resistance in the community is a serious problem in sepsis therapy that affects patient outcomes. The objective was to determine the relationship between antibiotic sensitivity and the outcome of sepsis patients in intensive care unit (ICU) RSUD Dr. H. Abdul Moeloek Lampung Province 2018-2021. The design of this research was observational analytic with cross sectional approach. Total sampling technique was used. Analysis of the data used was chi square test. The characteristics of sepsis patients were male patients (51.1%), elderly age group (43.2%), respiratory tract infection (28.4%), patients with comorbidities (51.1%), and the patient recovered (51.1%). The antibiotic profile showed that the most widely used antibiotic was ceftriaxone (36.4%), frequency of administration ≤2 times (65.9%), duration of administration ≤7 days (76.1%), and sensitive antibiotics (59.1%). Bivariate analysis showed that there was a significant correlation between antibiotic sensitivity and outcome of sepsis patients in the intensive care unit (ICU) RSUD Dr. H. Abdul Moeloek Lampung Province in 2018-2021 with a p value of 0.01 (<0.05).

Abstract

Asthma is a chronic inflammation of the airways that causes narrowing of the airways. The prevalence of asthma in Indonesia ranks highest for the category of non-communicable diseases. Asthma is a disease that needs to be controlled to prevent exacerbations and improve the patient's quality of life. Both patients and families are expected to have the knowledge and motivation to make lifestyle modifications so that treatment goals can be achieved, so treatment is needed through a family approach. Primary data were obtained from home visits to complement family data, psychosocial data, and the environment. Secondary data obtained from medical records. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. Patient Mrs. PF is 18 years old, has the main complaint of shortness of breath since 1 day ago. Shortness of breath accompanied by wheezing and coughing, worsening especially at night and early in the morning, cold weather, and after cleaning the house. The patient was diagnosed as bronchial asthma. In this case, diagnosis and treatment have been carried out according to the latest theory and journals. After the intervention, there was a decrease in clinical symptoms and an increase in the knowledge of patients and their families. The diagnosis of bronchial asthma in this patient was in accordance with the theory from several guidelines and journals, it was seen that there was a change in knowledge of the patient and his family after an intervention based on Evidence-Based Medicine that was patient-centred and a family approach.

Abstract

The population growth rate in the city of Bandar Lampung for the 2002-2010 period was 1.61%, in the 2010-2015 period it increased to 2.04. One way to overcome this problem is the family planning program. The results of the 2017 Indonesian Demographic and Health Survey (IDHS) show that people still use a lot of short-term contraceptives, even though short-term contraceptives have a high risk of failure. This is evidenced by the data on the percentage of MKJP and non-MKJP users at Kedaton Health Center, which is 0.08% while non-MKJP users are 0.92%. Husband's support is one of the external factors in the choice of contraceptives which become an amplifier to influence someone's behavior. Much information states that the decision was obtained from the wife because of the husband's interference. This study aims to determine the effect of husband's support on the choice of contraceptive methods at the Kedaton Inpatient Public Health Center in Bandar Lampung. This research is an observational analytic study with cross sectional design with a measuring instrument in the form of a questionnaire and with the research subjects as many as 109 respondents, taken by consecutive sampling method. The results of this study were processed using computer software with the Chi-square test method. The results obtained showed that there was a statistically significant relationship between the influence of husband's support on the choice of contraceptive methods at the Kedaton Inpatient Public Health Center in Bandar Lampung (p = 0.02).

Abstract

Osteoarthritis (OA) is a degenerative disease of the joints that involves the cartilage, joint lining, ligaments and bones causing pain and stiffness in the joints. The causes of osteoarthritis are multifactorial, including gender, obesity, genetic factors, race, family history, joint injury, strenuous physical activity, work, and repetitive movements of the same joints. Asthma is a chronic respiratory disease based on chronic inflammation which results in obstruction and hyperreaction of the respiratory tract as a result of the interaction between genetic, host, and environmental factors. Both osteoarthritis and hypertension are diseases that must be managed comprehensively because they are chronic.To apply the principles of family doctor medicine according to the problems found in patients in various aspects and carry out a holistic and comprehensive management that includes patient-centered, family approached, and community-oriented based on evidence-based medicine. Patient Mrs. I, 62  years, with complaints of knee pain since about 5 day ago. Knee pain is felt especially when the patient does strenuous activities or when in a squatting position and then stands up, and when the patient goes up and down stairs, but the complaint improves when the patient rests.  The patient was diagnosed with osteoarthritis.  The patient has a history of asthma since childhood and asked for asthma medication because he was worried that in the rainy season with cold weather his asthma could recur. Several factors can affect the patient's condition, namely internal and external risk factors. In this case, diagnosis and treatment have been carried out according to the latest theory and journals. After the intervention, there was a decrease in clinical symptoms and an increase in the knowledge of patients and their families.

Abstract

Obesity has a tendency to develop throughout the world, both in developed and developing countries. This is due to the prevalence which continues to increase preogressively from year to year. This central obesity is closely related to degenerative diseases such as hypertension that occur through the mechanism of Renost Angiotensin Aldostseron and sympathetic nerve activity. Poor sleep quality can increase the increase in blood pressure so that it worsens the development of hypertension in patients with central obesity. This type of research is a non-paired observational analytic type with cross sectional research design. This research was conducted in November 2012 in the University of Lampung. The sample used in this study was based on consecutive sampling and obtained as many as 80 men with central obesity. This research was conducted by interviewing the Pittsburgh Sleep Quality Index questionnaire and measuring blood pressure to respondents from all the academic community of men with central obesity selected in the University of Lampung. The results of this study were processed using computer software. The results of the analysis with Chi Square test of sleep quality on systolic blood pressure showed p value <0.05, namely p value 0.021 and PR 0.056 and 95% CI 0.320-0.932 and sleep quality on diastolic blood pressure obtained p values ​​0.013 and PR 0.531 and 95% CI 0.312-0.903. The conclusion of this study, there is a relationship between sleep quality and blood pressure in men with central obesity at Lampung University.

Abstract

The incidence of Diabetes Mellitus (DM) in the world and in Indonesia continues to increase. One factor that can affect high blood glucose levels and end up being DM is psychological stress. Patients with central obesity are more vulnerable and have a higher risk of experiencing stress caused by various psychological and emotional factors. The occurrence of stress will cause the activation of the Corticotropic-Releasing Hormone (CRH)-Adrenocorticotropic Hormone (ACTH)-cortisol hormone system which has a direct effect on increasing glucose levels in the blood (hyperglycemia) and if the condition continues, it will increase the risk of DM disease. Observational analytic method with a cross-sectional approach was used in this study. The study sample consisted of 82 patients with central obesity in the University of Lampung environment, with a consecutive sampling method. This research was conducted by filling in and direct interviewing the Perceived Stress Scale (PSS)-10 questionnaire and measuring fasting blood glucose levels in all the academic communities on men with central obesity at the University of Lampung. The results of the univariate analysis of the research sample were dominated by the age group of 18-25 years; the stress level was in the moderate category; with uncontrolled fasting blood glucose levels. The results of bivariate analysis with Chi-Square test showed p-value = 0.001 (p-value <0.05). There is a relationship between stress level and fasting blood glucose in men with central obesity at the University of Lampung.

Abstract

Diabetes mellitus (DM) has reached epidemic proportions worldwide, Indonesia is the sixth ranked country in the world. Diabetes with complications is the third highest cause of death in Indonesia. This study applies a holistic and comprehensive family doctor service by identifying risk factors, clinical problems and patient management based on Evidence Based Medicine with a patient centered approach and a family approach in cases of diabetes with complications of diabetic foot ulcers. Primary data were obtained through history taking (autoanamnesis from the patient and alloanamnesis from the patient's family), physical examination and home visits. Patient Mrs. K, 56 years old, was diagnosed with Diabetic Foot Ulcers and Type 2 Diabetes Mellitus. The patient has limited support and knowledge of the family about the patient's disease and its complications as well as the pattern of curative treatment. Holistic and comprehensive management of Mrs. K has been carried out to increase the knowledge of patients and families about DM with complications.
