Abstract

Clopidogrel is an anti-platelet agent commonly used in patients with atherosclerotic cardiovascular (CV) disease. Although formerly considered safe, several studies reported that the use of clopidogrel may cause a significant increase in the rate of gastrointestinal (GI) bleeding. This adverse effect could be minimized by coadministration of proton pump inhibitor (PPI). However, since PPI and clopidogrel share the same metabolic pathway, it has been hypothesized that the administration of PPI following clopidogrel therapy may cause a reduction in its anti-platelet effect, thereby increasing the risk of CV events. Recent studies found no significant inhibition in the activation of clopidogrel by CYP2C19 with administration of PPI in vitro. Pharmacokinetic and pharmacodynamic studies, as well as clinical studies, reported conflicting results regarding the potential interaction between PPI and clopidogrel. Until now, the available study investigated the PPI-clopidogrel interaction are primarily observational. The COGENT study is the only prospective, placebo-controlled trial examined the PPI-clopidogrel interaction. This study revealed no significant increase in CV events in patients receiving PPI following clopidogrel therapy, compared to the control group. Though remains controversial, current expert consensus recommended the administration of PPI in patients receiving clopidogrel, particularly in high-risk patients. Abstract

Evaluation of CAD is more routinely done by treadmill test (TMT i.e. physical strain) than the more frequently occurring mental strain, so a study was planned to assess the provocability of ischaemia by various mental tasks in patient with positive TMT. Thirty educated subjects, positive on TMT were put on a 24 hour holter monitoring. During this time, subjects were assessed by Mental Stress Test (MST) by subjecting to various mental tasks - (a) Time stress test (b) Mental arithmetic test (c) Reading test (d) Zeigarnik effect test and observed for heart rate, blood pressure and ischaemic/arrhythmia responses. The results showed that the male : female ratio was 14:1 with a mean age of 57 ± 8.03 years. The mean change produced during MST in (a) heart rate was 9.16 (SD  ± 1.24)/min (b) SBP was 8.86 (SD  ± 1.32) mmHg (c) rate pressure product was 82x103; which were statistically low (p<0.001) when compared to haemodynamic changes with TMT. Ischaemia was inducible in only one subject by MST and no increased incidence of arrhythmias during MST was noticed. The low yield of inducible ischaemia by MST when compared to TMT could be due to poor haemodynamic responses achieved by MST when compared to responses of TMT (p<0.001). It is concluded that mental stress does produce ischaemic changes. More intense and sustained MSTs which could bring about significant haemodynamic changes are required for inducing ischaemia as by TMT. Abstract

Fourty three patients with chronic renal failure undergoing chronic hemodialysis in Division of Nephrology and Hypertension, Faculty of Medicine, University of Indonesia/Cipto-Mangunkusumo Hospital, Jakarta, since October 2003 until February 2004, were examined for echocardiography (2-D, M-mode, Doppler imaging).Diastolic dysfunction was found in 58.1 % of chronic renal failure patients on hemodialysis. There was no significant difference between left ventricular mass in the group with or without left ventricular diastolic dysfunction. Abstract

Aim Zinc (Zn) deficiency remains a problem in most developing countries, including Indonesia, especially in the East Nusa Tenggara (NTT) Islands. Zinc plays a major role in pain through the modulation process by the N-methyl-Daspartate (NMDA) receptors, which also includes neuronal nitric oxide synthase (nNOS) as a pain parameter. The purpose of this study is to reveal the effects of Zn towards pain response and modulation stage at the spinal cord level in rats.Methods Twenty Sprague Dawley (SD) rats were divided into two groups, a deficient group and a normal group. The deficient group was fed on an IRI-OB diet. Every group was further divided into two more groups, the acute pain group (transient noxious stimuli), and the chronic pain group (continuous noxious stimuli). The rats in chronic pain group were subjected to CCI Bennet operation. The pain thresholds in the deficient group and normal group were measured clinically using a modified Ugo Basille plantar test (thermal transient noxious stimuli). Measurement of chronic pain level was carried out by measuring the nNOS level by immunohistochemistry.Results Deficient group showed an insignificant decrease in pain threshold (P= 0.251). However, there is a significant increase in nNOS (P= 0.027) especially in the deficient group with continuous noxious stimulation.Conclusions These results suggest that Zn deficiency increases pain response, especially in chronic pain. Abstract

Obesity has struck with all its might and the results are the increased world-wide prevalence along with increased mortality rate and other obesity-related diseases. Genetic, hormonal and environmental factors interact to cause obesity. Very low calorie diet and physically active lifestyle are the primary means to treat obesity. Additional intervention is by pharmacological treatment, using two new drugs, namely cyclobutane methanamine HCl  and tetra hidrolipstatin that are potential for the management of obesity. Another way to solve this problem is by performing a surgery. In conclusion, patients should consult their physicians in order to decide which methods are the most effective to find a solution to combat obesity. Abstract

Patients with severe inherited β-globin chain disorders may have milder illness if they produce high levels of fetal hemoglobin (HbF). Hydroxyurea (HU) has been shown to enhance HbF levels in patients with sickle cell disease and may be useful in β-thalassemias. We administered HU to 13 patients with β-thalassemia intermedia or major, including 6 splenectomized patients. The patients received escalating doses (10 to 25 mg/kg/d) of HU for around 2 years (median: 21 months, range: 8 - 55 months). Eleven patients responded with an increase in the pre-transfusion HbF levels, from a base line median of 8.0% (2.5 - 61.3%) to 28.0% (6.6 - 49.2%) and 40.7% (4.8 - 72.3%) at 3 months and 18 months post-HU, respectively. A concomitant increment in median hemoglobin levels was noted at 1, 3 and 18 months of HU therapy. Six of 7 transfusion-dependent patients who had an increment of HbF (one with β-thalassemia major) also had reduced transfusion requirement over the 2-year period of HU therapy. Response to HU was also shown by a reduction in spleen size. Apart from oral ulcers that resolved upon dose reduction of HU, no significant toxicity was noted. We conclude that increased HbF production in β-thalassemia patients, with an improvement in erythropoiesis, can be achieved using HU with minimal toxicity. Abstract

Background: Bacterial contamination is a common phenomenon in foods served in campus canteens and my cause physical illness which will affect academic activity. The aim of this study was to rank the level of Escherichia coli contamination in food and drink in campus canteens.Methods: Forty nine (49) foods and 24 types of drink were examined using conventional agar broth method for calculation of most probable number (MPN). The steps of the mothod were presumptive test for coliforms, fecal coliforms and E. coli, confirmes test for coliforms, fecal coli and E. coli and then completed test for E. coli. An analysis for contamination by E. coli in meals, utensils, and on the hands of the server was also undertaken. The data analyzed in percentage and rank all type of foods and drinks, also rank based on the location.Results: Almost all type of meals was contaminated. Meals with chili sauce were the most risky from the contamination of E. coli (90.15 %), then followed by dry meals (38.89%), while the wet meals were the the most unrisky meals. In  drinks, the highest was lacy juice, followed by jambu (guava) juice, then Sirsak and Orange juices on the third rank, while the mango juice was the lowest contamination. Melon juice, cappucino and fruit-coctail did not have E. coli  contamination.Conclusion: The contamination in the top three rank of contamination could be from the utensils used and foodhandler. Abstract

This study was designed to investigate the biochemical alteration in human caused by free radical accumulation due to air pollution. The policemen recruited were 60 country policemen consisting of 30 non smokers (group I) and 30 smokers (group II); 30 smoking policemen with administrative work in Jakarta (group III) and 30 smoking traffic policemen from Jakarta (group IV). Our results show that the plasma malondialdehyde (MDA) contents of groups I, II, III, IV, respectively were : 3.34 ± 0.81 umol/l; 4.28 ± 0.77 umol/l; 5.20 ± 0.66 umol/l and 5.12 ± 0.78 umol/l which were statistically different (p < 0.01) among each others, except between groups III and IV. The activity of superoxide dismutase (SOD) of the groups respectively, were: 16.75 ± 9.80 U/mg protein; 22.83 ± 6.82 U/mg protein; 26.10 ± 8.50 U/mg protein and 26.90 ± 9.34 U/mg protein, which were statistically different (p < 0.05) between group I vs the other groups. The catalase activity of the groups respectively were : 106.25 ± 47.47 U/mg protein; 118.84 ± 42.73 U/mg protein; 119.83 ± 35.35 U/mg protein and 145.57 ± 61.85 U/mg protein. The statistical difference (p < 0.05) was found between group I & IV. Administration of vitamins C (500 mg) and E (300 mg) daily for 30 days to the policemen with high oxidant stress caused a decreased in the activities of SOD from 27.34 ± 10.05 U/mg protein to 16.91 ± 6.51 U/mg protein (p < 0.05) and catalase from 134.29 ± 53.28 U/mg protein to 67.07 ± 25.26 U/mg protein (p < 0.05). This study shows us that the oxidant status of city policemen was higher than that of rural policemen and that a combination of vitamins C & E could reduce the oxidant stress to a certain extent. Abstract

Background: Buceng is a combination of plant of purwoceng (Pimpinella alpina Molk) and pasak bumi (Eurycomalongifolia Jack) that has been traditionally used to enhance adult male vitality. However, the mechanism of action of buceng has not been understood. This study was aimed to elucidate whether buceng could increasing Testosterone (Te), Luteinizing Hormone (LH), and Follicle Stimulating Hormone (FSH) level in rats.Methods: 20 male Sprague Dawley rats were randomly assigned into two groups. After one week of acclimatization, control group was given aquadest 2 mL, while treated group B received 2 mL (containing 25 mg) buceng extract for seven successive days. The plasma concentration of Te, LH, and FSH were assesed by RIA and IRMA method respectively. Independent t-test was used to analyze the different concentration of Te, LH, FSH between the two groups.Results: Testosterone level of treated group was significantly higher (3.55 pg/mL) compared to control group (1.00 pg/mL), p = 0.003. LH concentrations of treated group (0.12 pg/mL) was slightly but not significantly higher compared to control group (0.11 pg/mL), p = 0.810. Likewise, the FSH level was not significantly different between the two groups (0.15 vs 0.14 pg/mL, p = 0.088).Conclusion: Administration of buceng extract increases testosteron level which might play a role in enhancing male vitality.
