Abstract

Beta-thalassemia major (BTM) is difficult to treat chronic disease, causing physical and psychological burdens for the patient. Several studies have confirmed a decrease in physical activity and depression in thalassemia patients, but limited studies examine the relationship between these two conditions. This study aims to analyze the relationship between depression and physical activity in BTM patients in Bandung city. It was analytical observational research with a cross-sectional design. Data were collected during September–December 2018 by interviewing 65 patients selected by simple random sampling from 300 thalassemia patients registered at the Association of Parents with Thalassemia Indonesia/Perhimpunan Orangtua Penderita Thalassemia Indonesia (POPTI) Bandung city. The instruments used were the Global Physical Activity Questionnaire (GPAQ) to measure physical activity and the Beck Depression Inventory (BDI) to assess depressive symptoms. Data were analyzed by chi-square test using SPSS for Windows ver. 23.0. The results showed that most BTM patients in Bandung city were depressed (52%) and had low physical activity levels (65%). Furthermore, there was a statistically significant relationship between depression and physical activity in thalassemia patients in Bandung city (p=0.04, p<0.05). Therefore, it can be concluded that BTM patients in Bandung city with depression have lower physical activity. DEPRESI BERDAMPAK PADA AKTIVITAS FISIK YANG RENDAH PADA PASIEN TALASEMIA BETA MAYORTalasemia beta mayor merupakan penyakit kronis yang sulit disembuhkan sehingga menimbulkan beban fisik dan psikologis bagi pasien. Beberapa penelitian telah mengonfirmasi penurunan aktivitas fisik dan depresi pada pasien talasemia, namun studi yang mengkaji hubungan antara kedua kondisi ini masih terbatas jumlahnya. Penelitian ini bertujuan menganalisis hubungan antara kondisi depresi dan tingkat aktivitas fisik pada penderita talasemia beta mayor di Kota Bandung. Desain penelitian bersifat observasional analitik dengan rancangan potong lintang. Pengambilan data dilakukan selama September–Desember 2018 dengan mewawancarai 65 pasien yang dipilih secara simple random sampling dari 300 pasien talasemia yang terdaftar di Perhimpunan Orangtua Penderita Thalassemia Indonesia (POPTI) Kota Bandung. Instrumen yang digunakan adalah Global Physical Activity Questionnaire (GPAQ) untuk mengukur aktivitas fisik dan Beck Depression Inventory (BDI) untuk menilai gejala depresi. Data dianalisis dengan uji chi-square menggunakan SPSS for Windows ver. 23.0. Hasil penelitian menunjukkan mayoritas penderita talasemia beta mayor di Kota Bandung mengalami depresi (52%) dan memiliki tingkat aktivitas fisik rendah (65%). Selanjutnya, terdapat hubungan bermakna secara statistik antara depresi dan aktivitas fisik pada penderita talasemia di Kota Bandung (p=0,04; p<0,05). Oleh karena itu, dapat disimpulkan bahwa penderita talasemia beta mayor di Kota Bandung yang mengalami depresi memiliki aktivitas fisik yang lebih rendah.

Abstract

Osteoartritis (OA) lutut merupakan masalah kesehatan masyarakat utama yang mengakibatkan nyeri dan disabilitas. Sampai saat ini masih belum jelas diketahui dampak penyakit OA lutut dilihat dari derajat nyeri dan klasifikasi radiologik OA dengan kualitas hidup pasien OA lutut. Tujuan penelitian ini adalah mengetahui hubungan derajat nyeri dan klasifikasi radiologik OA dengan kualitas hidup pasien OA lutut. Penelitian ini adalah studi analitik dengan pendekatan cross sectional. Sebanyak 85 pasien OA telah dipilih secara consecutive dan dinilai kualitas hidupnya yang terdiri atas delapan komponen menggunakan kuesioner SF-36, derajat nyerinya menggunakan numeric rating scale (NRS), serta secara radiologik berdasar atas klasifikasi Kellgren-Lawrence (KL). Penelitian ini dilakukan di RS Al-Islam Bandung selama periode April–Juli 2018. Data dianalisis dengan SPSS melalui Uji Kruskal-Wallis. Dari hasil penelitian didapatkan mayoritas pasien OA lutut mengalami derajat nyeri sedang (50%) termasuk dalam klasifikasi radiologik OA moderat (43%) dan skor kualitas hidup 548 (265,63–728,5). Penelitian ini menunjukkan hubungan bermakna antara derajat nyeri dan kualitas hidup pada aspek nyeri dan vitalitas (p=0,000 dan 0,005) serta hubungan klasifikasi radiologik OA dengan kualitas hidup aspek fungsi sosial (p=0,027). Disimpulkan bahwa terdapat hubungan derajat nyeri dengan kualitas hidup pada aspek nyeri dan vitalitas serta hubungan klasifikasi radiologik OA dengan kualitas hidup pada aspek fungsi sosial.RADIOGRAPHIC CLASSIFICATION WITH QUALITY OF LIFE IN KNEE OSTEOARTHRITIS PATIENTKnee osteoarthritis (OA) is a major public health issue causing chronic pain and disability. Until now there is a little explanation on the impact of degree of pain and radiographic classification of OA on quality of life (QOL) in knee OA patient. The objective of this study was to clarify association between degree of pain and radiographic classification of OA with QOL in knee OA patient. A total of 85 knee OA patients were selected consecutively and assessed for their eight component of quality of life using the SF-36 questionnaire, their degree of pain using numeric rating scale (NRS), and radiologically based on the Kellgren-Lawrence (KL) classification. This research was conducted in RS Al-Islam Bandung during period of April–July 2018. Data were analyzed with SPSS through Kruskal-Wallis test. From the results of the study, the majority of OA patients experienced moderate pain degree (50%) and were included in moderate OA classification (43%) and QOL score of 548 (265.63–728.5). This study showed that there was a significant association between degree of pain and QOL on pain and vitality component (p=0.000 and 0.005) and association between OA radiographic classification and QOL in social functioning component (p=0.027). It can be concluded that there is association between degree of pain with QOL on pain and vitality component also association between radiologic classification of OA with QOL on social functioning component in knee OA patient.

Abstract

Pain, joint stiffness, and difficulty performing activities like rising from sitting to standing are signs and symptoms of knee osteoarthritis (OA). These conditions are risk factors for limited mobility and lower quality of life. Knee OA is closely associated with age, women, obesity, and other characteristics. The study's objectives were to determine the correlation of knee OA patients' characteristics with functional mobility using the 30-second sit-to-stand test (30STS) and the correlation of functional mobility with quality of life using the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) Index. The research method was descriptive-analytic cross-sectional using medical records of 73 knee OA patients at the Medical Rehabilitation Clinic at Soreang Hospital, Muhammadiyah Hospital, Al Islam Hospital, Al-Ihsan Regional General Hospital West Java Province, Bandung, from March until August 2021. Patients' characteristics such as age (p=0.02), onset (p=0.01), OA grade (p=0,03), and knee deformity (p=0.04 ) have a negative correlation with functional mobility based on 30STS as well as functional mobility had a negative correlation with various aspects of quality of life, such as pain (p=0.03), stiffness (p=0.02), and functional limitation (p=0.00) subscales based on WOMAC index. Age, the onset of disease, OA grade, and knee deformity significantly correlate to functional immobility. Based on the WOMAC index, functional immobility correlates with the patient's quality of life.

Abstract

Abstract. Dengue Hemorrhagic Fever (DHF) is a public health problem in Indonesia with many endemic areas and an increasing number of sufferers and a wider spread, and in 2016 West Java became one of the areas with the highest average dengue cases each year. The purpose of this study was to determine the pattern of platelet and hematocrit levels of DHF sufferers in the Internal Medicine Ward at Al-Ihsan Hospital, Bandung in January-December 2021. The research method used was an observational descriptive study with the cross-sectional research design. The data collection technique uses total sampling derived from primary medical record data. The results showed that in 202 DHF patients, the highest pattern of platelet decrease occurred on the 2nd day and began to increase on the 3rd day. Meanwhile, the hematocrit experienced the highest pattern of increase on day 1 and decreased on day 3. The conclusion of this study is that the platelet count in DHF patients tends to increase again on the 3rd day during hospitalization as well as the hematocrit level which tends to decrease again on the 3rd day. The weakness of this study is that the researchers do not know and examine patterns of fever and illness in DHF patients before receiving treatment at the hospital. Abstrak. Demam Berdarah Dengue (DBD) menjadi satu persoalan kesehatan masyarakat di Indonesia dengan banyak daerah yang endemik dan jumlah penderita yang semakin banyak serta penyebarannya semakin luas dan pada 2016 Jawa Barat menjadi salah satu daerah dengan rata-rata kasus DBD tertinggi setiap tahunnya. Tujuan penelitian ini adalah untuk mengetahui pola kadar trombosit dan hematokrit penderita DBD di Bangsal Penyakit Dalam RSUD Al-Ihsan Bandung pada bulan Januari–Desember 2021. Metode penelitian yang digunakan adalah penelitian deskriptif observasional dengan desain penelitian cross sectional. Teknik pengambilan data menggunakan total sampling yang berasal dari data primer rekam medis. Hasil penelitian menunjukkan pada 202 pasien DBD terjadi pola penurunan trombosit terbanyak pada hari ke-2 dan mulai meningkat pada hari ke-3. Sedangkan untuk hematokrit mengalami pola peningkatan terbanyak pada hari ke-1 dan penurunan pada hari ke-3. Kesimpulan dari penelitian ini adalah gambaran trombosit pada penderita DBD cenderung meningkat kembali pada hari ke-3 saat rawat inap begitu juga dengan kadar hematokrit yang cenderung menurun kembali pada hari ke-3. Kelemahan dari penelitian ini yaitu tidak mengetahui dan meneliti pola demam dan sakit pada pasien DBD sebelum pasien mendapatkan perawatan di Rumah Sakit.
