Abstract

This research was conducted in an international company engaged in iron and steel products manufacturing industries. One of the equipment that is often damaged is a hot roller table machine in the furnace section mill unit. The availability results obtained in hot roller table equipment is 96.571% and is still below the company standard which is set at 98%. Therefore, we need an analysis of the root causes of the problem and search for the best solution to fix the existing problem by applying the method of Failure Mode and Effect Analysis (FMEA). FMEA is a method that can systematically and structurally analyze and identify the consequences of a system or process failure, and also reduce or analyze the probability of failure. The purpose of this study is to identify and analyze the level of damage and its causes with the application of the FMEA method. Based on the pareto diagram the damage to the hot roller table machine, it was found that the highest frequency of damage was in the rotary coupling with a down time percentage of 26.9%. From the FMEA Analysis, two components that have very high RPN values are categorized as potential severit i.e. bearing as the first with an RPN value of 392 and the second is a seal ring with an RPN value of 294. The two components are the main priority for repair of the furnace section. mill, especially for machine and human aspects.

Abstract

This study aims to maximize the decline in the level of product defects to lead to zero defects. Researchers use the six sigma method through 5 stages of completion, namely, define, measure, analyze, improve, and control. Based on the results of the study it can be concluded that the sigma level and the value of DPMO (Defect per Million Opportunities) of the company amounted to 3.47 with possible damage of 24,239 sheets for one million productions. After further analysis using the Pareto diagram, it found that the cutting fault transfer defect was the most common with a percentage of damage of 43%. And based on the results of the analysis of fishbone diagrams, it can be seen that the main factors causing contamination in the production process come from the worker factor (human), followed by machine factors, material, and work methods. The proposed improvements that can be made by the company include updating existing methods and SOPs (standard operating procedures) and monitoring the operator's performance and developing knowledge and planning proper maintenance scheduling.

Abstract

Deaths and injuries from traffic accidents have become health problems for people throughout the world including Indonesia. The saddest data from the victims who died due to traffic accidents found that 10,428 people were killed in 2017 because the drivers did not use seat belts. This research aims to see the relationship between the perception of safety risk i.e. the ability, knowledge and environmental factors with the behavior of the use of safety belts in truck drivers in mining companies. This research is a comparative causal research i.e. research that states the relationship of one variable causes other variables. What is affected is the dependent variable, namely the use of safety belt behavior and the influencing variable is the independent variable, namely the perception of the risk of driving safety. Participants are 25 mining company truck drivers. The data obtained is then processed and analyzed using the SPSS version 16. Based on the results of data processing and analysis, it is found that the ability, knowledge and work environment factors have an influence on the safety belt usage behavior.

Abstract

This study aims to determine the value of the risk of work hazard potential and the level of risk of potential occupational hazards in the blast furnace division of hot metal treatment plant which is currently in the commissioning stage. This study uses an approach with the HIRA (Hazard Identification and Risk Assessment) method to determine the value of the risk of potential occupational hazards and the level of risk of potential occupational hazards. The results obtained by using HIRA (Hazard Identification and Risk Assessment) obtained 26 risks classified by potential hazards, namely nine risks included in the high category (H), seven risks moderate category (M) and the remaining ten risks involved in the low category ( L). The recommendation that can give is to carry out detailed mitigation of the risk of high (H) and moderate (M) categories until the risk level falls to the risk of the low (L) category before the blast furnace plant is fully operating.

Abstract

Sinter ore merupakan produk setengah jadi dalam pembuatan baja. Produk ini dihasilkan dari perpaduan (mixing) raw material dibagian sinter plant untuk mendapatkan karakteristik produk baja yang berkualitas. Berdasarkan hasil identifikasi masalah di bagian Sinter Plant, ditemukan bahwa proses produksi produk sinter ore masih belum efisien. Hal ini dibuktikan dari komposisi material hasil mixing design produk cenderung melebihi target properties proses mixing design yang diinginkan oleh perusahaan. Penelitian ini bertujuan untuk mencari nilai efisiensi biaya produksi produk sinter ore dan dapat mencapai target properties produk sinter ore yang ditetapkan oleh perusahaan. Penelitian dilakukan dengan menggunakan data karakteristik produk yang menjadi target properties dalam mixing design produk sinter ore tahun 2019. Perhitungan mixing design dengan menggunakan metode simpleks. Pengolahan data dengan menggunakan metode simpleks dilakukan dengan cara eliminasi secara iteratif sebanyak 5 kali perhitungan sampai diperoleh solusi optimal. Penerapan metode simpleks dalam pengolahan mixing design produk sinter ore menghasilkan nilai efisiensi yang cukup signifikan bagi perusahaan. Nilai efisiensi yang dapat dicapai sebesar 1.050.210 USD berdasarkan penurunan biaya mixing design dari semula sebesar 20.445.600 USD menjadi sebesar 19.395.390 USD. Pencapaian target properties komposisi material hampir mendekati target yaitu fines ore A sebesar 0,8997, fines ore B sebesar 0, fines ore C sebesar 0,0027, concentrate sebesar 0,0201, dan sub material sebesar 0,1627. Hasil penelitian menunjukan bahwa penerapan metode simpleks dapat menghasilkan solusi optimal bagi perusahaan, baik menyangkut pencapaian keuntungan maupun efisiensi biaya.
