Abstract

The purpose of this study is to describe the anxiety levels in primigravida in the face of deliveries at the health center of Bukittinggi. This type of research is experimental with pre-experimental design using a one-group pretest-posttest with consecutive sampling technique. Data collection techniques used questionnaires anxiety level of the Zung Self Rating Anxiety Scale. Results of the study showed a mean value of bivariate analysis on a pregnant woman before being given health education (pretest) = and the mean value after a given health (posttest) = which shows the p value = 0,000 which means there is the influence of health education on anxiety primigravida.

Abstract

Assessment during pre registration in midwifery school is one of the necessary process which need to be concerned as it would strengthen the quality of the graduation. It also could become a tool to evaluate ability and competency of the pre registration midwifery student. There is an evaluation which applied in Indonesian midwifery school as national standard i.e. OSCE in which is considered could fulfil the validity, reliability and feasibility within Indonesian context. This study aimed to evaluate the implementation of OSCE among midwifery school in Yogyakarta. This research was a mixed methods study used sequential explanatory design method i.e. quantitative phase and qualitative phase. Analytic descriptive study was applied in quantitative phase, whilst a case study was applied in  qualitative phase. Quantitative data collection was conducted by using a checklist sheet and analyzed by using descriptive analysis, whilst qualitative data by using in-depth interviews and analyzed by using content analysis. The quantitative result show that the implementation of OSCE among midwifery schools in Yogyakarta has score 68 out of 92. The qualitative analysis showed that OSCE was challenging to be implemented due to technical problems such as broken media of assessment, different phantom model in delivery stages, assessor has been late, assessment rooms were not conducive and there were limited model who skilled as OSCE model. Conclusion: Standardized OSCE implementation has not been carried out properly in midwifery school. Therefore there is a need of national evaluation of the OSCE itself in order to understand the real condition across Indonesian midwifery school.

Abstract

Maternal Mortality Rate (MMR) as an indicator of maternal health during pregnancy, childbirth, and childbirth. Anemia contributes 20% to the incidence of maternal deaths caused by bleeding during labor. The government prevents anemia by giving iron tablets at least 90 tablets for pregnant women. However, the prevalence of anemia remains high, because of the low adherence in consuming Fe tablets every day. Self-efficacy adherence is a predictor of health behavior and adherence to treatment.To improve adherence with self-efficacy in consuming Fe tablets in pregnant women with health promotion and WhatsApp reminder in Yogyakarta City Health Center.This study used a quasi-experiment method with a pre-test post-test non-equivalent control group design. The sample amounted to 59, the sampling technique used was consecutive sampling which was taken by random sampling technique in which samples were taken alternately to determine the intervention group and the control group.There were significant differences in the self efficacy score between the intervention groups (Mean = 6.30, SD = 8.11) and the control group (Mean = 0.89, SD = 7.83) with t-test = 2.60 and ρ = 0.012.There is a significant difference in self-efficacy adherence with consuming Fe tablets in pregnant women before and after being given health promotion and WhatsApp reminder intervention and control groups. Health service agencies can consider the combined service between health promotion and WhatsApp reminder to improve self-efficacy in adhering to consuming Fe tablets in increasing Hb levels in anemic pregnant women.

Abstract

Honey is widely used to cure many disorders including digesting system disorder, but the scientifical evidence remains lacking. One of the most recurring digesting system disorders is gastric ulcer. This research is aimed therefore at figuring out the effect of honey on aspirin induced gastric ulcer on rat. The subject consist of 30 rats Wistar strain, classified into 5 groups, 2 control such as positive control group exposed to cimetidine and negative control group exposed to amylum and 3 treatment groups. Treatment group is treated with honey in 3 levels of dosages, which are 30 ml/day, 45 ml/day and 60 ml/day given orally for 2 days then left for 48 hours without meal. Aspirin 150 mg/kgBWis given as ulcer induction. All the subjects are then decapitated. The gasters are later examined and the ulcers appearing on the gaster are calculated using modified Ralph method. The observation result is then analyzed and statistically tested using Kruskal Wallis and Man Whitney Test, with reliability level of 95%. The result shows that the score of group exposed to amylum (9,08 ± 4,43) is significantly different from group exposed to cimetidin (1,42 ±1,11) but is not significantly different from group exposed to honey treatment, sequenced from the smallest dosage to the highest (3,75 ± 4,78; 6,50 ± 4,43; 4,58 ± 2,94). Meanwhile, the group exposed to honey with 30 ml/day dosage shows ulcer score of 3,75± 4,78 which is slightly different from that group exposed to cimetidin 1,42 ±1,11.Madu banyak digunakan untuk mengatasi berbagai gangguan atau penyakit termasuk gangguan pada saluran pencernaan. Salah satu gangguan pencernaan yang bersifat kronis dan kumat-kumatan antara lain adalah ulkus lambung. Penelitian ini dilakukan untuk mengetahui efek profilaksis madu pada ulkus lambung tikus yang ditimbulkan olah aspirin. Penelitian dilakukan pada 30 ekor tikus putih jantan galur Wistar yang dibagi menjadi 5 kelompok terdiri dari 2 kelompok kontrol dan 3 kelompok perlakuan. Kelompok kontrol terdiri dari kelompok kontrol positif diberikan cimetidin dan kontrol negatif diberikan amilum. Kelompok perlakuan diberikan madu dalam 3 peringkat dosis yaitu 30 ml/hari, 45 ml/hari dan 60 ml/hari. Bahan uji diberikan selama 2 hari secara oral, selanjutnya dipuasakan selama 48 jam. Ulkus lambung diinduksi dengan pemberian aspirin 150 mg/kgBB. Hewan uji didekapitasi dan lambung diperiksa, dihitung jumlah ulkus dan perdarahan menurut cara Ralph yang dimodifikasi. Hasil pengamatan dianalisis dengan uji statistik nonparametrik Kruskal Wallis dengan tarafkepercayaan 95 % dilanjutkan dengan uji Man Whitney Test. Hasil penelitian menunjukkan perbedaan skor ulkus yang bermakna antar kelompok. Didapatkan skor ulkus kelompok amilum (9,08 ± 4,43) berbeda bermakna dengan kelompok cimetidin (1,42 ±1,11) tetapi tidak berbeda bermakna dengan kelompok madu sesuai urutan dosis terkecil (3,75 ± 4,78 ; 6,50 ± 4,43 ; 4,58 ± 2,94) namun demikian kelompok madu dosis 30 ml/hari menunjukkan skor ulkus 3,75± 4,78 tidak berbeda signifikan dengan cimetidin 1,42 ±1,11.

Abstract

Problem Based Learning atau PBL merupakan metode pembelajaran inovatif dalam kurikulum pendidikan dokter yang dinilai sesuai dengan tuntunan zaman dan kemajuan ilmu pengetahuan dan teknologi khususnya dibidang kedokteran. Berbeda dengan metode pembelajaran konvensional yang teacher centered dan menempatkan mahasiswa pada posisi pasif, maka metode PBL ini dengan strategi pendekatan SPICES (student centered, problem based, integrated teaching, community ori­ented, early clinical exposures dan self directed learning) menuntut keaktifan dari mahasiswa untuk belajar mandiri berdasarkan masalah secara terintegrasi dan berorientasi pada kebutuhan komunitas, serta sedini mungkin terpapar dengan kasus- kasus klinis. PBL dilaksanakan dalam bentuk diskusi kelompok atau small group discussion sekitar 10 mahasiswa tiap kelompok yang akan dibimbing oleh satu orang tutor. Dosen atau tutor dalam sistem PBL ini akan berfungsi sebagai fasilitator. Untuk membantu memecahkan massalah dalam diskusi kelompok dikenal istilah seven jump atau 7 langkah yang harus ditempuh meliputi: clarify term and con­cept, define the problem, analyze the problem, makae a systematic inventory, formula learning objectives, collect additional information, synthesize and check.

Abstract

Health care provider provide first response to patients in disaster. Medical and health institutions in Indonesia which produce health professional are still in early process to develop IPE for their students. Health professional unaccustomed with team work to serve patients, therefore they are not properly prepared to response natural disasters. The aim of this article is to give understanding for health institutions about interprofessional education (IPE) concept related to natural disaster. Review article conducted by analyzing scientific article from journals and text books. IPE is a necessary step in preparing a collaborative practice ready health work force to respond in disaster and getting better patient outcomes. Patients are the primary focus of any emergency response such as disaster. In disaster team work is the crucial competency that must be think first. Team performance however may not be optimal and it is often the patient who suffers from this dysfunction. Improved inter professional practice in emergency response will lead to better patient outcomes when teams have adequate knowledge and skills. They will manage the patients each other appropriately based on their each role. Health institutions should develop IPE including module focus on disaster/emergency preparedness to optimize role of health provider. Real experience into community exercises also necessary to ensure that student involvement during actual disasters also needed as a part of local disaster management plans.

Abstract

Nonsteroidal anti-inflammatory drugs (NSAIDs) exert its action through inhibition of prostaglandin (PG) synthesis. These drugs act by inhibition of cyclooxygenase (COX) which involved in PG synthesis. COX-lwhich is involv in PG synthesis may maintains normal gastric mucosa and influences kidney function. COX-2 a COX-1 homolog which is induced by inflammatory stimulan is inhibited by NSAIDs. The anti-inflammatory actions of NSAIDs are due to the inhibition of COX-2, whereas the unwanted side effects such as irritation of the stomach lining and toxic effect on the kidney are due to inhibition of the COX-1. In the treatment of inflammatory diseases such as osteoarthritis, using NSAIDs with selective COX-2 inhibition show effective benefit with minimal gastrointestinal side effect.Obat analgesik anti-inflamasi nonsteroid (AINS) bekerja menghambat enzim cyclooxygenase (COX) sehingga sintesis prostaglandin (PG) juga terhambat. COX- 1 yang terlibat dalam sintesis PG dapat mempengaruhi fungsi ginjal dan pertahanan mukosa lambung, sehingga hambatan pada COX-1 oleh obat AINS dapa: menyebabkan timbulnya efek samping pada lambung dan ginjal. COX-2 suatu ho¬molog COX-1 terinduksi pada keadaan inflamasi dapat dihambat oleh obat AINS. Oleh karena itu obat AINS yang selektif menghambat COX-2 akan lebih menguntungkan terapi penyakit inflamasi seperti osteoartritis karena efek samping pada saluran cerna dan ginjal dapat dicegah.

Abstract

Waktu terus beijalan demikian pula proses pendidikan terus berkembang sesuai kemajuan zaman, sehingga metode pembelajaran yang menempatkan mahasiswa ialam posisi pasif atau teacher centered sudah tidak sesuai lagi untuk menghadapi 3ntangan ke depan yang lebih berat bagi calon-calon dokter. Metode pembelajaran tradisionil yang selama ini banyak digunakan dalam proses pendidikan dokter di Indonesia mungkin perlu diinovasi dalam beberapa hal agar dapat memenuhi kebutuhan dan tuntutan perkembangan zaman. Inovasi dalam pendidikan tidak hanya dalam hal metode saja tapi juga dalam banyak hal. Inovasi tidak harus dengan merubah secara total kurikulum pendidikannya ataupun metode pembelajarannya tetapi bisa dilakukan secara bertahap atau parsial disesuaikan dengan kemampuan institusi baik dalam hal sarana, biaya, SDM, dll.

Abstract

Pembelajaran klinik merupakan hal yang sangat penting dalam sebuah pendidikan keperawatan karena pembelajaran klinik merupakan proses tranformasi mahasiswa untuk menjadi perawat yang professional. Pemikiran yang kritis, tindakan dan sikap profesionalisme diperankan oleh pembimbing klinik, namun pada kenyataanya pembimbing klinik dilapangan belum memahami kompetensi yang harus dimiliki. Tujuan penelitian ini untuk mengetahui persepsi pembimbing klinik tentang kompetensi pembimbing klinik. Penelitian ini deskriptif dengan pendekatan kualitatif dengan melibatkan 6 pastisipan dalam FGD dan 6 informan dalam wawancara tidak terstruktur. Responden diteliti dengan menggunakan panduan FGD dan wawancara. analisa data menggunakan constant comparative method. Penelitian ini menghasilkan 4 makna final kompetensi pembimbing klinik yaitu kompetensi sebagai perawat professional, kompetensi dalam membina hubungan interpersonal, kompetensi dalam mengajar (pedagogic) dan kemampuan dalam manajerial. Kesimpulan didapatkan Kompetensi pembimbing klinik sesuai presepsi pembimbing klinik

Abstract

Dosen pembimbing klinik pendidikan dokter tahap profesi di rumah sakit sebagai role model yang baik diperlukan untuk mengajarkan sikap perilaku, skills dan knowledge. Studi ini dilakukan untuk merumuskan konsep role model yang diharapkan oleh mahasiswa pendidikan dokter. Disain penelitian ini adalah deskriptif, cross sectional dengan pendekatan kuantitatif dan kualitatif. Metode pengumpulan data dengan kuesioner, wawancara mendalam dengan the general interview guide approach dan observasi dengan insider observer. Lokasi penelitian pada 8 rumah sakit pendidikan FKIK UMY, penentuan sampel secara purposive dengan criterion reference, yaitu mahasiswa yang menjalani stase paling lama di bagian atau mahasiswa di bagian 4 besar pada 8 RS pendidikan. Hasil penelitian menunjukkan, 70% dosen pembimbing klinik pada bagian 4 besar 8 rumah sakit pendidikan FKIK UMY merupakan role model yang baik. Berdasarkan kriteria role model dari Passi, 98% dosen pembimbing klinik menunjukkan clinical skills yang baik, 93% menunjukkan teaching skills dan personal quality yang baik. Terdapat kesesuaian terhadap role model yang diharapkan oleh mahasiswa dari hasil kuesioner, wawancara dan observasi. Rumusan role model yang baik menurut mahasiswa, yaitu mempunyai kemampuan mengajar atau teaching skills yang baik antara lain cukup waktu untuk membimbing, mampu berkomunikasi, me­motivasi mahasiswa belajar, memberikan feedback, dan memiliki pengetahuan yang luas serta up date.In clinical rotation at teaching hospital, the role of clinical teacher as a good role model was needed to teach attitude, skills and knowledge. The aim of this study is to develop concept of role model based on student’s opinions at school of medicine. This study is descriptive, cross sectional using quantitative and qualitative approach. Collecting data method conducted by distributing of questionaires, indepth interview by the general interview guide approach and direct observation by insider observer. This study conducted at 8 teaching hospitals of FKIK UMY. Sampling method by purposived and criterion refer­enced. The respondents was the students who conduct in longest clinical rotation i. e. 4 major departements at 8 teaching hospitals. This study revealed, 70% of clinical teacher in 4 major departements at 8 teach­ing hospitals is good role model. Regarding Passi’s criterias of role model, 98% of clinical teacher show good clinical skills, 93% show good teaching skills and personal quality respectively. There is similar result of concept of role model from questionaires, indepth interview and direct observation. Based on student’s opinions, a good role model should has good teaching skills such as sufficient of time to assist students, good communication skills, can motivate students, give feedback and has both wide and up date of knowledge.
