Introduction: The purpose of this study was to analyze the relationship between family support and self-esteem with motivation for the post-stroke rehabilitation in Ploso, Nganjuk. Methods: The design of this study was correlation through a cross-sectional approach. The sample used a total population sampling consisting 34 respondents. The instrument used in this study was questionnaire and analyzed by spearman rank was used to determine the relationship. Results: The results showed that 34 respondents who got a strong relationship between family support and self-efficacy p value (0,002), with direction of positive relationship that proved that Ha accepted. Discussion: The conclusion of this study is family support and self-esteem are closely related to patient motivation in undergoing post-stroke medical rehabilitation in Ploso, Nganjuk.
