Abstract

On 1st January 2014 primary-care BPJS began to be applied to support the implementation of BPJS services at Community Health Centers. This system is implemented by all community health centers in Indonesia, inclusive of the 32 community health centers in the Sumedang District. Based on the observation results at the community health centers in the city area in the implementation of the of the primary-care system, there were several problems among others, lack of facilities liked computers, printers and internet network, also frequent system errors. The purpose of this study is to find out primary-care system implementation overviews at Sumedang City District community health centers. Method used in the study was qualitative with phenomenology approach and used HIPO analysis model. There were 3 informant groups in this study, i.e. informant group of 6 primary-care users, key informant group of 5 community health center Heads and 1 Head of administration, and support informant group being the Primary Services Head Management of Sumedang District BPJS. Data collection process comprised of in-depth interviews and observations. Study results showed that input factors made up of knowledge was good, good users’ skill, very minimized communication, inadequate resources comprised of human, facilities, budget and minimal information, acceptable users’ attitude and inadequate bureaucratic structure due to unavailability of SOP. Process comprised of data collection still inaccurate, process undone, data presentation accommodated to Community Health Centers needs and data analysis accommodating number of visitations. Output in the system implementation in the form of community health centers visitation reports for BPJS still need to be expanded for the sake of supporting primary-care system implementation optimization at community health centers.

Abstract

Security, privacy, secrecy and safety is a device covering the health record data/information, all parties involved in health care should maintain the security of patient’s data/information. Preliminary studies in RSHS showed that there were some problems related those data security such as, the release of medical record information without the written permission or authorization letter from the patient, as much as 660 medical record documents were not returned to the medical records.The purpose of this study are knowing the release of medical record information to the 3rd parties according to the legal regulation in RSUP Dr. Hasan Sadikin Bandung. The research design is qualitative case study. The sampling technique is snowball sampling when the choice of informant based on information from previous informant. Data analysis focused to important findings, classified in decided themes and patterns. The results found that: there was patients's ignorance about the value of medical resumes, users was late to restore the file and brought the file out of the hospital, and insurance was not explaine about informant’s medical record, utilization procedures were not followed, and not used the patient's written consent according to education, research and insurance of BPJS. It was suggested that users should respect to the patient's privacy right, such as obey hospital procedures for patient’s. confidentiality and attach patient’s written consent, and sanctions against the obidient users.

Abstract

Performance is the result achieved by a person under the applicable workload for the job. Nurse performance is the work behavior displayed by someone based on the motivation and behavior of a nurse. This study aims to determine the relationship between attitudes, knowledge, and motivation with the Nurse performance in nursing care documentation at the Orchid and Lotus Room in Bekasi Regional Hospital, West Java Province in 2015. The design used in this study was cross sectional method. The population in this study was the nurses at the Orchid and Lotus Room in Bekasi Regional Hospital totaling 41 people, and all the population sampled in this study (total sampling). The analysis results shows significant relationship between attitudes (p value: 0.019) and motivation (p value: 0.005) with the Nurse performance in nursing care documentation in the Orchid Lounge and Lotus Room in Bekasi Regional Hospital, West Java Province Year 2015. It is expected to nurses in order to better focus and right in doing nursing care documentation, and continues to maintain the attitude and motivation to improve nursing care services to patients.

Abstract

Codefication Diagnosis should be done precisely, accurate and appropriate, considering the data diagnosis is authentic law evidence and data required in morbidity reports and insurance interests. External cause code used as secondary code to describe external causes of a disease. Coding of external cause in RSUD dr. Soekardjo not optimally implemented in accordance provision applicable based on Standard Operating Procedure (SOP). This research purpose to analyze the accuracy of external cause code four-digit and five-digit and the contributing factors. The type of this research is mix methodes. The method was used for 94 patients medical record documents, which were taken by total sampling and indepth interview with two informants. The data analysis is using univariate analysis using frequency distribution formula and also qualitative analysis. The results obtained the percentage of external cause four-digit code 24,5% accurate and 75,5 % not accurate. While the code external cause five-digit not found the accurate code. Accuracy and optimally coding in RSUD dr. Soekardjo influenced by diagnosis of external cause that is not written, procedures for coding, human resources and all aplication process. Suggestions for hospital should do socialization about the complete recording of the traffic accidents information according to the rules of coding and standard applications.

Abstract

The Indonesian society at present needs quality health services which can be seen based on five quality dimensions, i.e. quick response (responsiveness), reliability, assurance, empathy and direct evidence/proof (tangibility). This case study aims to determine customers’ perception (patient and health officers) on quality health services at Pasar Rebo Sub-District of East Jakarta Community Health Centre and this study was carriedout in the month of May 2015. This type of study is a qualitative case study design. Study focus covered tangible (cleanliness, comfort, comprehensive equipments and drug availability), responsiveness (speed and service flow procedures), reliability (training, costs, officers’ responsibilities, service schedule and displinary officers), empathy (equity in services), assurance (courteous/friendly officers, safety, physically-able, intellectual officers and reward). Study samples were key and supporting informants, type of purposive sampling became the study subject for 7 informants covering: 1 Section Head Of Service as key informant and 6 supporting informants. Data collection techniques through in-depth interviews and non-participant observations. Interview guidance using a tape recorder, observation guidance using a camera and field notation. In-depth interview with quality health services key informant at Pasar Rebo Sub-District Community Health Centre was good and results of indepth interviews as well as observation on tangibility were in the passably category, while based on responsiveness, reliability, empathy, assurance dimensions were in good category. By upholding good quality health services can lead to every patients’ satisfaction seeking treatment.

Abstract

Continuous Ambulatory Peritoneal Dialysis (CAPD) therapy is one of substitute for renal therapy. Children undergoing CAPD therapy show good survivability. The roles of nurses in the CAPD unit are integral to this success. This study used a qualitative descriptive phenomenological approach and included as participants seven nurses with experience in CAPD therapy in children. Data were collected through in-depth interviews and were analysed by the Collaizi method. The results identified six main tasks CAPD nurses perform: optimisation of therapy in children, coordination and collaboration with other health care providers, training families to apply CAPD therapy, facing various challenges while providing care, strategising care for each individual child, and dealing with their own attitudes and emotions. Procedures need to be optimised in a comprehensive manner to care for children receiving CAPD therapy.
