Abstract

Background : Multiple trauma one of trauma caused by huge traumatic force involves many organs and tissues. AIS and ISS is scoring for multiple trauma diagnosis and classification. In this Case Report discuss about multiple trauma patient with Severe Brain Injury cause by Impression Fracture with open fracture in regio cruris sinistra and close fracture femur sinistra. Case presentation : The following is a case report of a 63-year-old female patient came to the emergency room of Soegiri Lamongan with decreased of consciousness due to a traffic accident. Mechanism of injury the patient's head hit the chair and the driver's side of the car pillar and the patient's legs were pinched by the car seat. Since 25-year-old patient has a history of uncontrolled diabetes mellitus but the patient regularly takes anti-diabetic drugs (OAD). Patient was diagnosed with multiple trauma with severe brain injury with suspected DAI with open fracture grade 3A middle 1/3 tibia and close fracture middle 1/3 distal femur.Conclusion : Multiple Trauma has a bad prognosis. The most important from Multiple trauma management is prehospotality management. Additional Severe Brain Injury make the diagnosis become worse.

Abstract

Background:  Paclitaxel-carboplatin chemotherapy can cause hema-tological disorders. This regiment's hematological side effects include anemia, neutropenia, and thrombocytopenia. Data showed this regiment side effect occurs with grade iii/iv toxicity anemia, grade III/IV anemia by 41%, neutropenia (78%), and thrombocytopenia (41%). This hema-tological disorder occurs in bone marrow-producing areas due to chemotherapy. Each hematological disorder requires separate interven-tion.Case Presentation: A 54-year-old woman was diagnosed with a progressive disease of left lung adenocarcinoma (Exon 21 mutation) with TKI and received paclitaxel-carboplatin chemotherapy. The patient experienced side effects of anemia, neutropenia, and thrombocytopenia. Anemia was treated with PRC transfusion, neutropenia corrected with Filgrastim, and thrombocytopenia with platelet transfusion. Chemo-therapy was discontinued after five cycles because the Thrombo-cytopenia condition did not improve.Conclusion: Hematological parameters are essential in preparing for lung cancer chemotherapy. Uncorrectable hematological disorders lead to discontinuation of chemotherapy

Abstract

Background:  Interprofessional collaboration is essential in providing higher-quality health services and ensuring patient safety.Objective: This study aims to assess the attitude of cooperation between doctors and nurses as first line health workers in an academic health facility and explore their perspectives on the collaboration attitude between doctors and nurses.Methods: This study used a mixed-method design. The quantitative sample was chosen randomly but with total sampling for the primary clinic. Data on the attitude of cooperation between doctors and nurses from the questionnaire results were analyzed descriptively. The sample for the qualitative study was selected purposively, each health service consisted of 1 doctor and one nurse, so the number of informants was six. The qualitative data were transcribed verbatim and analyzed qualitatively using the constant comparative method.Results: The results showed that of 84 respondents, 54 doctors, and 30 nurses, 95% showed a good teamwork attitude. Some respondents (5%) showed a bad attitude, especially on three items, namely item no 4 (doctors do not have the right to change the treatment plan that has been made), 13 (doctors do not have to be the final determinant) and 18 (possibility of other professions to become leaders). The results of the qualitative analysis show that there are five final themes, namely: 1. teamwork is good; 2. the role of the profession; 3. the importance of interprofessional communication; 4. openness to receive feedback; 5—patient safety and PCC.Conclusion: Both professions have distinct but equal and complementary roles in patient management, each with their own advantages or capabilities. Open communication, which includes getting criticism, can help overcome a poor cooperative attitude.
