Abstract

The Cardiovascular disease (CVD) prevalence continues to increase. This study aims to identify the CVD spending and utilization in the Indonesia National Health Insurance (JKN) scheme 2017-2021 using the JKN claims classified by Disease Accounts analyzed descriptively. The CVD spending increased by $125,4 million in 2019 compared to 2017 and mostly for the hospital (91% in 2019) with the highest spending occurring in the age above 60 years. Moreover, the hospital inpatient spending was dominated during these five years. The highest portion of spending by severity level was at mild level, followed by moderate and severe levels. Health facilities’ capacities across the regions remain the issue in providing health services, especially for the CVD that require further treatment using advanced technology. The CVD cost continues to increase annually; fortunately, the JKN plays a key role as financial protection for participants. But, there was a decrease in the number of visits and spending on CVD during the COVID-19 pandemic because of lockdown policies and health systems shifting to provide capacity to overcome COVID-19. The decrease in the number of visits to health facilities raises the potential of being diagnosed later with more severe. As policy recommendations, strengthening primary care screening and hospital capacity across regions through implementing the Health System Transformation are expected to reduce the impact of these problems.

Abstract

This study aims to identify marketing or socialization strategies for hospitals that provide Cardiac Surgery Services in Indonesia. The findings of this study will contribute to optimizing the services available in the regions as well as contributing to the transformation of health services, particularly in cardiac surgery services. This study is observational descriptive research. Data were collected from Cardiothoracic and Vascular surgeons and hospital websites with cardiac surgery services. Through the marketing analysis approach of 7P, this study identifies problems and policy recommendations that can be alternative solutions. The price of the service is high, but most of the patients used BPJS, and it is found that the hospital rate is higher than BPJS claims. The hospital should provide the costing and financial analysis that generate the hospital rate to BPJS revise the BPJS's claims. It can only be done if hospitals from many regions produce similar rates or slight variations. Jabodetabek still dominates as the region (place) with the most cardiovascular surgery services in Indonesia, with 29 of 35 hospitals. The recommendations to respond to this issue include ensuring data on demand for cardiac surgery services from various regions and encouraging the government through specialist expert associations and legislation to provide cardiovascular surgery services in various regions. Regarding the specialist (people), the government must provide scholarships for doctors to take BTKV specialists, especially those who can perform pediatric ones.

Abstract

The Lapor Vaksin COVID-19 Application was developed to support a vaccination coverage surveillance database sourced from community self-report. This application provides data on vaccination coverage and allows users to view updates to educational content about vaccines and visualization of COVID-19 vaccine data in real-time. This study aims to analyze  the perception among health worker about Lapor Vaksin COVID-19 application in the community. This study uses descriptive research with a case study design. This study was conducted in September 2021 through a virtual zoom meeting, in Ogan Ilir Regency, South Sumatra. The research sample was 42 health workers consisting of vaccine officers at the Health Office, RSUD and Puskesmas in Ogan Ilir Regency who were selected by purposive sampling. Data is presented descriptively. Based on the evaluation results of the application's use, more than half of the respondents were quite satisfied (52.4%) with the Lapor Vaksin COVID-19 application. This application can be used and developed as an alternative to the COVID-19 surveillance database.

Abstract

Ambulance is one of the hospital infrastructures that must meet service standards, security, and occupational safety and health in hospital operations. Therefore, it is the obligation of every hospital to ensure the standard of ambulance service. In the DKI Jakarta area, every ambulance, including hospital ambulances, must meet the standards in accordance with regulations and have an ambulance operating license. Based on data until the end of 2021, there were 82 (34.9%) hospital ambulances in Jakarta that carried out the recommendation process for an ambulance operation license from a total of 235 hospital ambulances, meaning that most hospital ambulances in Jakarta did not meet service standards. DKI Jakarta Governor Regulation number 120 of 2016 concerning Ambulance Services and Hearse is a reference for hospitals in Jakarta in order to meet ambulance service standards. In conclusion, there are 3 (three) standards that must be met in standardizing hospital ambulance services in Jakarta, namely standardization of vehicle units, standardization of equipment and standardization of human resources.

Abstract

To regulate the existence of Drinking Water Depots (DAM) in the city of Pariaman, the Pariaman City government issued Regional Regulation (Perda) Kota Pariaman No. 9 of 2015 concerning Business Permits for Drinking Water Depots. However, many depot owners do not implement these regulations. The purpose of this study was to analyze the implementation of Pariaman City Regional Regulation No. 9 of 2015 concerning Drinking Water Depot Business Permits. This research is a type of qualitative research. Data collection was carried out through in-depth interviews with informants involved in the implementation of Pariaman City Regional Regulation No. 9 of 2015 and focus group discussions (FGD) on drinking water depot owners. Selection of informants based on the principles of suitability and adequacy. The variables based on the framework of Edward III include communication, resources, disposition, and bureaucratic structure. The results of the research show that the implementation of the policy has not been implemented because the communication has not been optimal. The available resources and facilities are quite adequate, for sub-budget variables it has not gone well. Coordination has not been carried out properly, the sanctions set forth in the Regional Regulations have also not been realized optimally. Disposition related to the government's commitment to implementing policies has not been implemented. It is necessary to monitor the implementation of regional regulations and make derivative regulations related to supervision of drinking water depots, so that depots can carry out routine inspections according to what is stated in the regional regulations and make it easier for the Health Service to carry out supervision
