Jurnal Plastik Rekonstruksi is an open access, peer reviewed, international journal focusing on global plastic and reconstructive surgery. JPR publishes on all areas of plastic and reconstructive surgery, including basic science/experimental studies pertinent to the field and also clinical articles on such topics as: breast reconstruction, head and neck surgery, pediatric and craniofacial surgery, hand and microsurgery, wound healing, and cosmetic and aesthetic surgery. Abstract

Background : In Singapore, we experience a unique annual surge of burn cases involving sole of foot during the Hindu fire walking festival. Traditionally, superficial partial-thickness burns of the soles were managed expectantly with regular dressings. We conducted a study to evaluate the effectiveness of an alternative approach of using synthetic skin substitute (Biobrane®). Method : A case series of foot burns admitted October 2016 to the Singapore General Hospital Burns Centre. Burn cases with superficial partial-thickness burn of bilateral sole of foot were included in the study. Exclusion criteria included deep dermal burns and burns that required excision and closure with skin grafting. The patient demographics, time to presentation, medical co-morbidities were analyzed. Outcomes such as length of hospital stay, incidence of infection, need for further surgery, return to pre-injury ambulation status and occupation and cost were studied. Result : A total of 6 partial thickness burns of sole of foot in 3 patients with bilateral foot burns which were treated with Biobrane® were included in the study. Patients were able to return pre-injury functional status- return to work and pre-morbid ambulation status. Neither incidence of wound infections nor any needs for repeat surgery were recorded. However, this group required an increased length of stay and incurred high treatment cost. Conclusion: Biobrane® is a viable adjunct in the management of sole of foot burns. The authors believe there is potential for an improved rate of recovery with Biobrane®. However, the higher cost and length of stay are among its drawbacks. In view of the limitations of our study- case series and small sample size, a prospective and double arm assessment will be required to present definite evidence for synthetic skin substitute in these cases

Abstract

Background: Cleft lip and palate is one of the most common congenital anomalies with the prevalence of 1 case for every 1000 birth. Nulliparity has been associated with increased of many birth defects. In contrast, multiparity has also been associated with several birth defects. Methods: This case-control study compares data from 74 cleft lip patients who was enrolled in Gentur Cleft Foundation to those of 87 normal children. The dependent variable of this study was the incidence of CL/P (cleft lip with or without cleft palate) and the independent variables were maternal parity. Binary logistic regression was used to analyze crude and adjusted OR. Nulliparous mothers were treated as the reference category. Result: Compared to nulliparous mothers, primiparous mothers has OR 1.57 (95%CI=0.78-3.2). While multiparous mothers has OR 27.54 (95%CI=3.5-216.32). P-value was insignificant for the association of primiparous mothers with CL/P Conclusion: We observe higher incidence of multiparous mothers with the incidence of CL/P compared to nulliparous mothers.

Abstract

Background: In the last decade, there are various methods of bone grafting in plastic surgery. Cancellous bone is more often used as bone graft than cortical bone due to its high osteoconductivity and osteogenic capability. The synthetic materials for bone graft such as tricalcium phospate (TCP) and hydroxy-apatite can be use as new alternative for bone grafting surgery. Methods: The PubMed® databases were searched using the terms: ‘bone graft OR bone grafting’ AND ‘cancellous bone OR iliac crest OR tibia’ AND ‘tricalcium phospate OR tricalciumphospate OR tri calcium phospate’, restricted to English language, and to a publication period from January 2010 to January 2017. The inclusion criteria were any kind of research paper that compared the utility of TCP synthetic graft and autograft as a bone graft. Results: Out of 139 selected studies, only 8 met the inclusion criteria. In 6 publications TCP graft has similar outcome and in 1 publication have a better result in reducing post operative complication. Conclusion: TCP graft can be used as substitution for autograft in particular clinical conditions and surgical methods.

Abstract

Background : Algorithm for vulvar reconstructions in defects after oncologic resection of vulvar tumors has been established. It goals are to restores form and function for the purpose of coitus, micturition, and defecation. The purpose of this article is to present our experience in vulvar and anal reconstruction. Method : A 43-year-old female with extensive vulvar, perineal and anal squamous cell carcinoma in-situ and groin lymph nodes metastasis requiring defect coverage after tumor resection. The defects involving exposed soft tissue around vulvar, perineal and anal areas. Result : Vulvar and anal reconstruction was done in stages utilizing triple VY advancement flap. The reconstruction was successful with preservation of sexual and anal functions; and minimal scarring. Conclution: Extensive vulvar and anal defects can be reconstructed by recruiting local tissue. VY advancement flap is one of the best options in these reconstructions.

Abstract

Background : Burns result in severe injuries that cause damage or loss of tissue due to contact with sources of heat resulting in injuries to all body systems. Injuries of the skin, which functions as a barrier to protect internal organs, may cause patients to experience damage to one's physical appearance and body image causing negative feelings that may lead to other problems such as psychopathology and symptoms of mental illness. Method : A cross sectional study with consecutive sampling method of burn patients who were treated at the Plastic Surgery Outpatient Clinic and Burn Unit of RSCM was conducted between April-May 2017. Subjects were asked to fill in self-report questionnaires including patient identity form, SRQ-20 (cutoff point ?6) for presence of psychopathology, and WHOQoL-BREF to obtain mean scores of quality of life that include four domains of physical, psychological, social, and environment assessment. Data collected was analyzed using correlation analysis. Result : 56 burn patients were included in the study. 30.4% did not work and 48.2% had very low income per month. 67.9% patients experienced burns due to fire and 44.6% had burns 10-30% of the TBSA with a majority of patients (80.4%) experiencing a combination of second & third degree burns. Based on the analysis, 57.1% of patients had a form of psychopathology and low mean scores of quality life (physical domain 48.1, psychological domain 51.5). Significant negative correlations (p ? 0.05) were obtained between the psychological domain and symptoms of depression, anxiety, low energy; physical domain and low energy; and social domain with anxiety. Conclusion: This study obtained significant results to identify the correlation between psychopathology and various domains of quality of life affected.
