Abstract

Composite resin is a caries filling material. Generally, dentists choose this type of resin as a restoration material because this material has a high compressive strength. There are several factors that influence the compressive strength of composite resins, including beverages consumed such as coffee. This study aims to determine the difference in the compressive strength of the composite resin with Sidikalang coffee immersion in 9 hours, 27 hours and 54 hours immersion time. This type of research is experimental laboratories with posttest only control group design. The research sample was composite resin measuring 8 mm in diameter and 4 mm in height. The sample size is determined by the Federer formula and a total sample of 24 was obtained for the four treatment groups. Measurement of compressive strength using a three-point bending tool on a universal testing machine. Data were analyzed by using the oneway Anova test. Based on the results of the study, the average compressive strength of composite resin in the group without immersion, 9 hours, 27 hours and 54 hours of immersion was 20.69 ± 0.84, 18.94 ± 0.41, 14.36 ± 0.66 and 6.70 ± 0.48. From the results of the study it can be concluded that the ads are a significant difference in the average compressive strength of composite resin after immersion in Sidikalang coffee for 9 hours, 27 hours and 54 hours.

Abstract

Aesthetics is a philosophy of the concept of beauty which is the reason people takeorthodontic treatment. One of the factors that influence aesthetics is a smile. Apartfrom personal experiences and social environment, the patient's perception of theaesthetics of the smile after orthodontic treatment can be influenced bycharacteristics. This study aims to determine the perception of smile aesthetics inpost-orthodontic patients based on characteristics. This type of research is descriptiveanalytic with cross sectional design. The study population were patients who wererecorded in medical records at the practice of drg. Josep Ginting, Sp. Ort in Pontianak,West Kalimantan Province with a sample size of 30 people. Collecting data bydistributing questionnaires containing nine aesthetic indicators of smiles through theWhatsApp application. Data were analyzed using the Mann Whitney test. Based onthe results of the study, it was seen that there were differences in the aestheticperceptions of respondents' smiles based on age (p = 0.017), gender (p = 0.004) andeducation (p = 0.015). The conclusion of this study is that there are differences inpatient perceptions of aesthetic smile after orthodontic treatment based on thecharacteristics in the practice of drg. Abstract

Introduction: Cholelithiasis or gallstones are hardened deposits of digestive fluid that can form in gallbladder. The treatment of gallstones depends upon the stage of disease. Once gallstones become symptomatic, definitive surgical intervention with cholecystectomy is usually indicated (typically, laparoscopic cholecystectomy is the first-line therapy). Laparoscopic cholecystectomy removes the gallbladder and gallstones through several small incisions in the abdomen. This case report aims to discuss the managent of segmental thoracic spinal anesthesia in laparoscopic cholecystectomy. Case: We report on the cases of 2 patients who undego elective laparoscopic cholecystectomy. Female, 53 years old, will undergo laparoscopic cholecystectomy, performed anesthesia with segmental thoracic spinal anesthesia technique using the anesthetic agent 3 ml of Levobupivacaine 0,5% + 25 mcg of Fentanyl, requiring a sensory block to at least T10-T11 dermatom. Patients received a 1,2 mcg target control infusion (TCI) propofol intravenous is administered as intermittent boluses to achieve deep sedation. The operation lasts 1 hour, with a bleeding 15 cc and urine output 100 cc, hemodynamically stable. Female, 42 years old, will undergo laparoscopic cholecystectomy, performed anesthesia with segmental thoracic spinal anesthesia technique using the anesthetic agent 3 ml of Levobupivacaine 0,5% + 25 mcg of Fentanyl, requiring a sensory block to at least T10-T11 dermatom. Patients received a 3 mg of midazolam intravenous is administered as intermittent boluses to achieve deep sedation. The operation lasts 1 hour, with a bleeding 15 cc and urine output 150 cc, hemodynamically stable. The level and duration of sensory block, intensity and duration of motor block were recorded. A 20 % or more decrease in MAP compared to baseline was considered as hypotension, iv ephedrine 5 mgr bolus administreted. Conclusion: Laparoscopy is a surgical procedure that uses minimally invasive surgical techniques (minimally invasive surgery) where the doctor uses a small telescope / camera that is inserted into the stomach and surgical instruments in mini form. This procedure has many advantages because it is not invasive, the amount of bleeding can be reduced, postoperative pain can be minimized. Regional anesthesia procedure in laparoscopic cholecystectomy based on several studies and case reports can be performed and is a safe procedure.

Abstract

Eosinophilic esophagitis is an immune-allergic pathology of multifactorial etiology (genetic and environmental) characterized by major symptoms of esophageal dysphagia and eosinophil-predominant inflammation of the esophageal mucosa that affects both pediatric and adult patients. EoE is an immune-mediated disease by which environmental and food antigens stimulate the Th2 inflammatory cascade. It is correlated with food allergy and atopy condition such as asthma, atopy dermatitis, rhinitis allergic and often in conjunction with Gastroesophageal Reflux Disease (GERD). Eosinophilic esophagitis (EoE) was first described in the 1990s, showing an increasing incidence and prevalence since then, in the United States is estimated to be approximately 57 per 100,000 persons being the leading cause of food impaction and the major cause of dysphagia. Its symptoms, which include heartburn, regurgitation, and esophageal stenosis. This symptomps similar to those of gastroesophageal reflux disease, causing delays in diagnosis and treatment. The endoscopic findings such as furrows, esophageal mucosa trachealization, and whitish exudates, this diagnosis should be confirmed histologically confirmed by biopsy on the presence of more than 15 eosinophils per high-power field and the exclusion of other causes of eosinophilia. Management includes medications, diet, and surgical dilatation.

Abstract

Background: vitamin D has an important influence on the host’s immune system by modulating both innate and adaptive immunity and regulating the inflammatory cascade. Vitamin D deficiency may be related with reduced immunomodulation, specifically in preventing the occurrence of chronic tonsillitis. Objective: to identify the association between vitamin D deficiency with chronic tonsillitis in pediatric patients. Methods: the search was conducted with advanced searching on PubMed® and Cochrane® according to the clinical question. The screening of title and abstract using inclusion and exclusion criteria, double filter, and reading full text led to six useful articles. The selected studies were critically appraised for validity, importance, and applicability. Results: three original articles were eligible for this evidence-based case report. All studies were considered to have good validity. All three studies reported an increased risk of chronic tonsillitis with the vitamin D deficiency and/or insufficiency. Conclusion: Vitamin D deficiency may be associated with the increased risk of chronic or recurrent tonsillitis in children.

Abstract

Diabetes Mellitus (DM) is a group of metabolic diseases characterized by hyperglycemia. Various epidemiological studies have shown an increasing trend in the incidence and prevalence of diabetes mellitus in various parts of the world. Lifestyle changes lead to an increasing prevalence of the metabolic syndrome diabetes mellitus. This study aims to determine the description of blood sugar levels during class 2017 students of the Faculty of Medicine, Prima Indonesia University. This type of observational analytic research with sampling techniques using non-methods This research is a descriptive study with a cross sectional study design. This research will be carried out at the UNPRI campus building 1. The sample of this study is all medical students of the 2017 Prima Indonesia University class totaling 99 people. Data were collected using a questionnaire and analyzed by univariate. The results showed that the majority of the respondents were female, namely 66 people (66.7%), while only 33 male respondents (33.3%) were male respondents. According to blood sugar levels, all respondents had normal blood sugar levels (100%). Respondents are advised to maintain a healthy diet and lifestyle so that their blood sugar levels remain stable and ultimately avoid the risk of diabetes mellitus.

Abstract

This study aims to determine the clinical and cytological features of various neck lumps. This study used a descriptive observational method with a cross-sectional approach to determine the clinical and cytological features of various neck lumps. This study used secondary data of patients with indications of a neck lump who were undergoing treatment or examination at the Medan Madani Hospital in 2018 - 2019. The population in this study were all patients with symptoms and positives experiencing a lump in the neck, at Madani Hospital Medan. Data processing is carried out after the observation and recording of medical record data is in accordance with what is needed, the data obtained is tabulated and coded for later analysis. The results of this study indicated that respondents who experienced thyroid nodules were more dominant in female respondents with a total of 11 respondents, with the most common type of thyroid nodule adenoma follicular thyroid, namely 9 respondents with female gender. Respondents who experienced inflammation were dominated by female respondents with a non-specific chronic inflammation process on the right side of the neck with a total of 10 respondents with a percentage of 23.8%.

Abstract

Chronic Kidney Failure describes as the gradual loss of kidney function and it is irreversible, to replace the loss of kidney function a therapy to kidney function is needed, which is hemodialysis. The objective of this research is to know the levels of ureu and creatinine serum for pre hemodialysis and post hemodialysis at RSU Royal Prima Medan. The methods that are being used for this research is descriptive with 54 patients as samples. The results of this research is loss of urea and creatinine serum level, with the average of urea level for pre hemodialysis is 128.11 mg/dL, and the average of urea level for post hemodialysis is 43.26 mg/dL with 1,8% patient with low urea level, 50% with normal urea level, and 48.2% with high urea level. The average of creatine level for pre hemodialysis is 11.56 mg/dL, and the average of creatine levels is 4.3 mg/dL, with all of the patient still have a high creatinine levels. The conclusion is there is drop for both urea and creatinen serum level, but the urea level for half of the patient did not go down to normal, and for the creatinine level there is none of the patient creatinine level that go down to normal.

Abstract

Renal involvement is the most common complication of systemic lupus erythematosus (SLE) and is also an important predictor of patient mortality. The incidence of flares is estimated at 65% each year in patients with lupus nephritis. Therapy in lupus nephritis with flare also uses high doses of steroid agents and strong immunosuppression agent. Mycophenolate mofetil (MMF) as a immunosuppression agent tends to favor for flare in lupus nephritis. We describe a patient who had flare in lupus nephritis that resolved with high-dose steroid and MMF. The combination of immunosuppression agent and high-dose corticosteroid is an effective for control of active diseases. Cyclophosphamide as the steroid sparing agent was discontinued because of adverse effect as well as hematuria. Partial remission was later achieved and maintained with MMF and corticosteroid after five month with protocol treatment. Thus, MMF while maintaining the steroid dose may induce remission for this case.

Abstract

This study aims to determine the effect of paprika ethanol (capsicum annuum) on blood sugar levels of alloxan-induced Wistar rats (rattus norvegicus). This type of research is experimental. The method used was pre test and post test with control group design. The results of this study indicate that for Post Test 1 or after being given treatment for 7 days, the average result shows that there is an effect of giving red paprika ethanol extract on blood sugar levels in Wistar rats where the significant value of each treatment is all less than 0.05. For Post Test 2 or after being given treatment for 14 days, only at a dose of 250 that has no effect on blood sugar levels of the rats, because the significant value is 0.813 greater than 0.05, for other treatments it has a significant effect because the significant value is more smaller than 0.05.
