Abstract

Background: The Wide Awake Local Anesthesia No Tourniquet (WALANT) has been reported to offer simpler pre-surgical instruction and assessment, faster operating time, better surgical visualization, shorter hospital stay, fewer complications and side effects, lower cost, faster post-operative recovery time, better patient’s satisfaction, and less pain and discomfort. This study aims to review the pain score in hand surgery using the WALANT technique compared to local anesthesia or general anesthesia with a tourniquet. Methods: A comprehensive literature search was conducted in PubMed, Cochrane Library, and Google Scholar in March 2021. Eligible randomized-controlled trials (RCTs) and cohort studies compared injection time intra-operative or post-operative pain using Visual Analog Score (VAS) in WALANT and local anesthesia or general anesthesia with a tourniquet. Results: Five studies (3 RCTs and two cohorts) were included in this article, including 645 hand surgeries (376 carpal tunnel syndrome cases, 181 trigger finger cases, 42 cubital tunnel syndrome cases, 44 de Quervain’s disease cases, and two ganglion cyst cases). Three studies reported a significantly lower VAS in the WALANT group, while the other two reported a lower VAS in the WALANT group, but not statistically significant. Conclusion: WALANT is still a technique worth considering in hand surgery because of its superiority in minimizing intra-operative and post-operative pain.

Abstract

Introduction: A rare congenital deformity known as "mirror image polydactyly" of the feet is typified by symmetrical duplication of the foot's digits. According to one study, only 28 cases (seven of which have treatment records) have been described in the English-language literature. Mirror foot can arise as a single congenital defect or a component of a hereditary syndrome involving several congenital defects. In this study, we reported a mirror foot case and emphasized its rarity, unusual presentation, and successful surgical treatment. Case presentation: A 3-month-old female child was born with unilateral mirror image polydactyly of the foot with congenital talipes equinovarus (CTEV) and congenital genu recurvatum. She was referred to the outpatient clinic of the surgery department of dr. Kariadi General Hospital. In a radiological examination, ten digits and nine metatarsals were found on the left foot. Preaxial duplicates of the first to fourth metatarsals were disarticulated, followed by ablation of five digits. The aim of surgical management for this case was to reconstruct a functional and cosmetically pleasing foot, which will benefit orthopedic surgeons in managing CTEV for serial cast correction. Six months after surgical intervention, the foot showed a satisfactory appearance. However, management of the CTEV was uneventful at the local orthopedic department, and the patient was referred to the pediatric orthopedic center in Jakarta. Conclusion: Incidence of mirror foot is extremely rare, and treatment must be tailored to each individual. Coordination among different departments should be pursued to ensure optimum care for patients.

Abstract

Introduction: Identifying the CKD progression of the patient should be presented with a range of choices for renal replacement treatment. Therefore, establishing vascular access for hemodialysis is significantly correlated with the survival rate of patients as well as knowledge related to access. Thus, in this study, we aim to report the characteristics and knowledge of the family of patients with CKD about AV-shunt at Dr. Zainoel Abidin General Hospital. Method: This research is descriptive research with a retrospective approach. This study evaluated the "Social Demographic and Medical Character" and "Knowledge towards AV Shunt" family of patients with CKD at RSUD Dr. Zainoel Abidin in Banda Aceh.  The samples in this study were all patients undergoing hemodialysis who had carried out AV-shunt at the hemodialysis installation at RSUD Dr. Zainoel Abidin from July 1 to July 31, 2021.  The data were analyzed descriptively by using SPSS. Results: From 80 respondents, an average age of 50.39 ± 12.594. Most AV Shunt care trainers are from doctors, and the rest are from doctors and nurses (6,3%). Regarding our study, 88,8% of the patients' families received education related to the AV-shunt, and only 11,3% did not. Conclusion: Regarding this study, we can conclude that the patient’s family knows about AV Shunt but must still be evaluated to maintain good AV Shunt function.

Abstract

Background: Diabetic foot ulcers are one of the complications of DM which can cause a decrease in productivity in patients. Diabetic ulcers make it easier for bacteria to grow and infect the body. Some pathogenic bacteria that are often found are Staphylococcus aureus, And Coli, Pseudomonas aeruginosa, Klebsiella pneumonia, and Proteus is wonderful. Antibiotics play an important role in the management of diabetic ulcers infected with bacteria, where the most sensitive antibiotics are Amikacin, Erythromycin, Meropenem, Ciprofloxacin, and Ampicillin. Therefore, this study aims to determine the bacterial patterns and bacterial sensitivity to antibiotics in diabetic ulcer patients in the surgical inpatient room of RSUDZA Banda Aceh. Method: This research is descriptive observational with a retrospective approach using medical record data from diabetic ulcer patients from August 2021 – August 2022. Sample collection was carried out randomly. non-probability sampling namely with total sampling. Data will be analyzed using univariate analysis Results: There were 60 samples of diabetic ulcer sufferers who underwent pus culture examination and antibiotic sensitivity testing in this study. Based on gender, men are the most likely to suffer from dibasic ulcers, namely 41 samples. The average age of diabetic ulcer sufferers was above 65 years in 19 samples. From the pus culture examination, it was found that the 4 most commonly found bacteria were: Staphylococcus aureus (13), E. Coli (13), Klebsiella pneumonia (10), and Pseudomonas aeruginosa (8). Based on the antibiotic sensitivity pattern, Meropenem has the strongest sensitivity to bacterial pneumonia and E. coli. Meanwhile, Levofloxacin has the strongest sensitivity to bacteria Staphylococcus aureus and Pseudomonas aeruginosa.  Conclusion: It can be concluded that the cause of most diabetic ulcer infections are Staphylococcus aureus and E. Coli, while the antibiotics of choice are Meropenem and Levofloxacin because they have the highest sensitivity.

Abstract

Background: Extensive literature has established a strong correlation between breastfeeding and a decreased susceptibility to breast cancer. The objective of this study was to examine the breastfeeding status of moms diagnosed with breast cancer. Methods: This study employs a descriptive-analytical approach with a cross-sectional design. The study comprised adult female patients (aged 20-60) who were diagnosed with Breast Cancer, irrespective of the specific kind of cancer confirmed by anatomical pathological investigation. Only non-smokers were considered for analysis. The study excluded patients who had a history of malignancy and those with a family history of cancer. A bivariate analysis was performed to evaluate the study hypothesis. The analysis was conducted utilizing the Chi-Square and unpaired T-test statistical methods using SPSS software. A p-value less than 0.05 is deemed statistically significant. Result: This research encompassed a total of 45 individuals diagnosed with breast cancer. The majority of individuals diagnosed with breast cancer (55.6% of the total) did not engage in exclusive breastfeeding. Similarly, individuals without a breast cancer diagnosis also tended to refrain from exclusive nursing (24.4%). The association between the two variables is statistically significant, as indicated by a p-value of 0.000. The average length of nursing in individuals diagnosed with breast cancer was 2.04 ± 1.81, but in those without breast cancer, it was 10.10 ± 9.01. The statistical tests indicate a substantial association between the two variables, as evidenced by a p-value of 0.000. Conclusion: A notable correlation exists between breastfeeding and a diminished likelihood of developing breast cancer.
