Abstract

Postpartum birth control is one of the services or conditions for jampersal membership. In Lumajang, only 57.8% of jampersal participants participated in post-delivery family planning. During this time midwives tend not to do family planning counseling in depth, so it is assumed that midwife motivation in family planning services after delivery is low. The purpose of this study is to determine the factors that influence the motivation of midwives in post-delivery family planning services in the jampersal program. This research is an observational study with cross-sectional design. Data was collected in a questionnaire using a structured questionnaire. Population are all midwives who are obliged to carry out jampersal in Lumajang Regency. The number of respondents was 164 Midwives who were selected by clustered random sampling. Bivariate analysis was performed with Chi square test and multivariate with logistic regression. The results showed that the majority (78.7%) of respondents had a Diploma III education, had a tenure of> 10 years (57.9%) and (54.9%) aged> 35 years. Most respondents (56.1%) have low motivation. Respondents' perceptions of jobs, incentives, supervision techniques, and administrative policies are mostly good. Midwives' responsibilities and competencies are mostly good. There is a positive relationship between responsibilities, perceptions of supervision techniques, perceptions of administrative policies and midwives' motivation in post-delivery family planning services. There is no relationship between perceptions of work, perceptions of incentives, competencies and motivation in family planning services after delivery. The most dominant variable influencing midwife motivation in family planning services after delivery to Jampersal is responsibility. It is recommended that the Health Office provide a format for post-delivery family planning reporting, the supervision of the jampersal program and simplify the claim process.

Abstract

Study about Diabetes Mellitus has showen the increases of its prevalention. Approximately in year 2002 there are 150 million patients in the world, year 2003 are 194 million, and year 2005 are 330 million. Diabetes Mellitus is one disease that can causes complications in many bodies organ, one of them due to circulation system and manifestating as orthostatic hypotension. Diabetes Mellitus is an endocrine metabolic disease with the chronic hyperglicemic, its sign decrease or none of insulin production. The disorder manifestation to circulation system is orthostatic hypotension, was define to acute blood pressure decrease (systole > 20 mmHg and diastole > 10 mmHg). To prevent this condition, nursing intervention that can doing is active mobilization. The aim to this study to know the influence of active mobilization to prevent orthostatic hypotension to Diabetes Mellitus with hyperglicemic. Its design was Quasy-Experimental with pre and post test design. It was held on November 2005 and located in IRNA I Saiful Anwar General Hospital. Sample was taken by used purposive sampling with 20 persons to experiment group and 20 persons to control group. The data analyze using Chi-Square test. In significant level 95% X2 = 6,667, degree of freedom (df) = 1, got result p-value = 0,010. it means there is a significant influence active mobilization to prevent orthostatic hypotension to Diabetes Mellitus with hyperglicemic. The next study is needed to do the instrument test first to get the exactly active mobilization technique to prevent orthostatic hypotension to Diabetes Mellitus with hyperglicemic, because this condition is still happen in many patient.

Abstract

Aging process causes decreasing musculoskeletal function such as degeneration, erosion, and calcification of cartilage and joint capsule that manifest in decreasing joint movement width. Aging process also causes decreasing cellular immune function such as increasing inflammation activity in joint. The causes are contributed to joint pain among elderly. Nurse is expected to give a safe and healthy solution to reduce joint pain in elderly. This study was aimed to identify the effect of rheumatic exercise on decreasing joint pain among elderly. Design used in this study was quasy experimental. The population was elderly whose age were 65-70 years old in Desa Sudimoro Kecamatan Tulangan Kabupaten Sidoarjo in June-July 2010. Total sample were 18 respondents taken according to inclusion criteria. The dependent variable was joint pain which was measured by Burbonais pain scale. Data were then analyzed using Wilcoxon Sign Rank Test and Mann Whitney Test with level of significance is ≤0,05. Result showed that there was effect of rheumatic exercise on decreasing joint pain among elderly after intervention in intervention group (p=0,014 Wilcoxon Sign Rank Test). But, there was no difference between intervention group and control group after Rheumatic Exercise (p= 0,297 Mann Whitney Test). It could be concluded that there was no effect of rheumatic exercise on decreasing joint pain among elderly. Many factors contributed to this result such as respondent dietary pattern and activity pattern during the research which were beyond control of researcher. Further studies should more control respondent’s dietary pattern, activity pattern, quantity and quality of the rheumatic exercise by respondents to obtain more accurate results.

Abstract

Approach to risk evaluation is an attempt to determine any possible risks in pregnancy can lead to maternal mortality and morbidity and premature babies. Hadijono in getting the results of his research on maternal mortality high risk pregnancy is 20 times higher than the low-risk pregnancies, perinatal mortality also obtained six times higher. The death of one of the problems caused due to late referral. The purpose of this study to determine the relationship approach to risk pregnancy Rochjati Poedji Score Card (KSPR) with referral patterns. Analytic design of this study is the correlation with the population of pregnant women who were referred in RSD dr. Soebandi Jember during month of April to June of 2013, as many as 320 samples taken randomly. Measuring instrument is secondary data medical records. The results of risk pregnancies and referral patterns indicate a low risk of pregnancy complications 12%, 62% of high risk pregnancies and pregnancies are very high risk 27%, 96% planned referral patterns and 4% referral fee. Chi-Square statistical test with significance level of 5% and df = 2 price obtained χ ² = 7.93 is greater than the table χ ² (7.93> 5.591) and the contingency coefficient obtained KK = 0.16 which means that there is a low relationship Among approach KSPR risk pregnancies with referral patterns. There are other factors that affect referral patterns, but with intensive approach to risk and maternal health services through a family-based problem-based pregnancy and safe delivery of the package, it will prevent the occurrence of late referral.

Abstract

Postpartum hemorrhage is bleeding that occurs within 24 hours after the birth to go Parity, age and duration of the second stage are the factors that influence the occurrence of cases of postpartum hemorrhage. Obstetric hemorrhage is technically defined as blood loss of 500 ml or more immediately after delivery. The purpose of this study was to determine the relationship of parity, age and duration of the second stage with the incidence of postpartum hemorrhage in Jember Ambulu health center in 2012. This research design using correlation studies, the retrospective approach. Populations that are experiencing maternal postpartum hemorrhage in health center Ambulu in 2012 a number of 64, the sample used by 55 samples. The sampling technique used was simple random sampling. From the test results data using Chi Square analysis obtained count 0.47 x2 < x2 table 5.991, meaning that there is no relationship with the incidence of maternal parity postpartum hemorrhage. Count x2 obtained at age 0.02 < 3.841 x2 table, meaning there is an association with maternal age incidence of postpartum hemorrhage. Whereas the second stage of a long relationship count obtained 4.09 x2 < x2 table 3.841, which means that the second stage of a long relationship with the incidence of maternal postpartum hemorrhage in Jember Ambulu Health Center in 2012. This fits the theory that the second stage of abnormal causes muscle failure myometrium to contract after delivery. To handle the midwife needs to improve midwifery care to pregnant women so that complications can be detected early.
