Abstract

Fracture of the penis is a rupture of the corpus cavernosum due to blunt trauma. In the West the penile fracture is commonly resulted from trauma in sexual intercourse but in the Middle East and Mediterranean is predominantly due to masturbation. The other causes are unconscious nocturnal manipulation or fall onto the erecting penis. We present a penile fracture case, one of the less frequent urological traumas, referred to the Sardjito Hospital. The report illustrated a 23 year old circumcised male, who had suffered from penile fracture while his penis was erecting. He was referred to the hospital by a general practitioner two days after the incident. Immediate surgical repair was done with the indication of specific history and severe hematoma in the penis as the clinical presentations. The patient was discharged on day 8 postoperation. A mild hematom was present in the wound and no sign of infection.Discussion about penile fracture written in the literature by authors is presented.Key words: penile fracture, causes, urologist problem, surgical repair.

Abstract

Until now coronary heart disease is the number one killer in the world. Angina pectoris is one of the clinical manifestations of coronary heart disease. If there is an insufficient oxygen supply to meet the need ischaemic myocard will happen.Life style changing, drug treatment and operative procedure are applied for the management of pain caused by myocardial ischaemia. According to many big anti-thrombotic trials statin, ACE inhibitor, calcium inhibitor, beta blocker and nitrate are very helpful in the management of chronic coronary artery disease. Myocardial laser revascularization, enhanced external counter pulsation and spinal cord stimulation are alternative procedures in which we can handle it with common management. While conventional therapies are continued to be improved, strategies to increase myocardial circulation by stimulating formation of collateral vessels around the obstructed coronary arteries are also in development. These approaches include angiogenesis, protein transfer, gene therapy and cell therapy. It is hoped that those approaches will safely and effectively applied in the management of chronic coronary artery disease. Key words: Coronary heart disease - chronic stable angina - myocardial ischaemia - new approaches

Abstract

Background: Oxydative DNA damage, namely 8-OHdG, may occurred due to DNA oxydation by reactive oxygen species (ROS). Unrepaired 8-OHdG may initiate a carsinogenesis due to G to T transversion of gene point mutations. The 8-OHdG repair can be measured by determination of urinary 8-OHdG level. It is well known that ROS reaction with preexisting melanin may perform an immediate pigmentation darken-ing (IPD) and it can be measured by mexameter. The correlation between IPD and 8-OHdG repair capacity is still not known.Aim of Study: To know the correlation between IPD and 8-OHdG repair ability in order to predict the oxydative DNA repair capacity.Methods: Using of a quasi experimental design, 8-OHdG urine level alteration correlated with skin color alteration after oxydative stress among 30 healthy elementary-school children during sport activity pro-gram under sun exposure. 24 hours of skin color alterations as a IPD were measured by mexameter, and urinary-8- HdG- level were measured based on ELISA with 8-OHdG-Kit antibody. The correlation be-tween IPD and     8-OHdG urine level alteration was analyzed with Spearman test.Results: There was a very significant correlation between immediate pigmented darkening and urinary                      8- OHdG level alteration (r=0.8562; p<0.01).Conclusion: IPD represents 8-OHdG repair capacity after oxydative stressed under sun exposure Keyword: immediate pigmention darkening -  8-OHdG -mexameter - sun exposure

Abstract

Stroke is a disease caused by cerebral blood flow disturbance. Disturbance of cerebral blood flow could induce oxidative and nitrosative stres. Beside that, stroke is a also neurodegenerative disease that based on several risk factors. Some risk factors of stroke have causative link with oxidative and nitrosative stres. These risk factors include aging, hypertension, smoking, atherosclerosis and diabetes mellitus. Key words: stroke - risk factor -reactive oxygen species - reactive nitrogen species

Abstract

Background: Tuberous sclerosis complex (TSC) is an inherited syndrome in which affected individuals are at increased risk for developing benign tumors. Mutation of the TSC-2 gene encoding tuberin on chromo-some 16p13.3 give rise to the clinical disorder of tuberous sclerosis characterized by the development of hamartomas.Objectives: In the present study, I conducted to examine whether overexpression of TSC-2 can affect the growth of human oral squamous cell carcinoma (OSCC) which have different expression level of p27KP. Methods: I constructed an expression vector containing sense-oriented rat TSC-2 cDNA with pcDNA3.1 (Invitrogen), and transfected to oral squamous cell carcinoma (B88) to regulate the expression of TSC-2 gene in each transfectant. Western blot analysis was carried out to determine the protein level of TSC-2. Therefore, Tumor volume and body weight of nude mice was examined using tumorigenicity assay.Results: Overexpression of TSC-2 exerted the growth inhibitory effect of oral squamous cell carcinoma. Sense-oriented B88-TSC-2 cancer cells, have high expression of p27KP1. Moreover, tumor induced by TSC2 up-regulated transfectans become much smaller than those of control cells.Conclusion: overexpression of TSC-2 may exert the antitumor effect on oral squamous cell carcinoma through p27KP induction. Key word: TSC-2 - transfection - oral squamous cell carcinoma - pcDNA3.1 - mutation.

Abstract

Objective: To report the success of amniotic membrane transplantation combined with trabekulectomy ina patient with Pseudophakic bullous keratopaty.Case Report: Amniotic membrane transplantation and trabeculectomy followed by tarsoraphy had been successfully performed in an eye of male patient 67 years old with pseudophakic bullous keratopathy, with increased intraocular pressure, the visual acuity for both eyes was hand movement, intraocular pressure in the right eye was 25 mmHg and 18 mmHg in the left eye. Patient felt painful in both eyes also. Pre surgical and post surgical medication with oral and eye drop antibiotic, oral and eye drop steroid, and oral vitamin was given.Result: The painful of both eyes disappeared on the            111 day after amniotic membrane transplantation and no recurrence in 2 week observation. In the right eye amniotic membrane dissolves on the 41n day after surgery, an epithelization occurred on the 51" day after surgery. On the 7th day the visual acuity was 2/60. Amniotic membrane dissolves on the 31d day in the left eye, epithelization occurred on the 4th day. On the 71h day the visual acuity reached 3/60 and remained constant until the 2"tl week of observation.Conclusion: Amniotic membrane transplantation followed by tarsoraphy is effective for pseudophacic bullous keratopathy treatmentKey words: Pseudophacic - bullous keratopathy - amniotic membrane transplantation- trabeculectomy-intraocular pressure- visual acuity-

Abstract

Background: Several studies reported that vitamin-C can enhance immune response, including phagocytosis response on bacteria and virus. However at present there are only a few studies about the role of vitamin-C in phagocytosis response on fungi.Objectives: To determine the role of vitamin-C on macrophage phagocytosis on C. albicans.Methods: Ten ml blood was taken from healthy subject. Buffy coat containing monocytes was separated from the red-blood cells. After plating the mononuclear cells on the coverslip glass, the cells other than monocytes were washed out by tripsinization. The monocytes were treated with several doses of vitaminC i.e. 200 mg/mL, 100 mg/mL, 50 mg/mL, and 0 mg/mL as placebo. And then Candida suspension was added to the cells, stained PAS and HE counterstain. The result of macrophage phagocytosis activity was interpreted by counting intracellular Candida spores and was analyzed using one way ANOVA.Result: There is a high significant difference in the. number of intracellular candida spores, between vitamin-C 200 mg/mL compared to placebo (p<0.000); and there wasnt significant difference between 100mg/mL (p=0.064) and 50 mg/mL (p=0.709) compared to placebo.Conclusion: The dose of 200 mg/mL vitamin-C could increased macrophage phagocytosis activity on C.a/bicans significantly (p<0.01) compared to other doses. Key word: phagocytosis - macrophage - vitamin-C - C. a/bicans
