Abstract

The paper describes various methods of problem-solving during the history of mankind and puts special emphasis on discovering and identification of psychiatric problems. Research methodology in general is explained.Then it reviewed similarities between psychiatric research -- and the general, methodology. Abstract

Plasma lipid spectrum studied included total lipid plasma, plasma triglyceride, plasma phospholipid and plasma cholesterol concentration in rats.1-listologically earlier hepatotoxicosis showed hyperplasia and metaplasia, and metaplasia of bile duct epithelial cells.The two groups of rats, the control group and the treatment group (each group comprised 10 rats). 3•month old, healthy, and 12 —16 g % haemoglobin concentration, were used as experimental animals. -All of them were fed and given water ad libitum, and 0.2 ml of propylen glycol per oral was also provided by sonde for 60 days. The treatment group was supplemented 10 mg of aflatoxin B, (AFB1) diluted in 0.2 ml propylen glycol.At the end of the treatment, histologically epithelial cells of bile duct showed extensive hyperplasia and metaplasia in rats supplemented with 10 rag AFB1. The plasma lipid spectrum between the control group and the treatment group was:plasma total lipid concentration (X ± 1 SD): 496.0 ± 12.61 mg/I00 ml and SO4M ± 10.46 mg/100 ml, plasma triglyceride concentration (X * 1 SD) 93.4 ± 14.83 mg/ l00 ml and 101.2 12.62 mg/100 ml. plasma phospholipid concentration (X ± 1 SD) 9.3 ± 4.21 mg/ l00 ml and 8.9 ± 3.92 mg/100 ml, plasma cholesterol concentration (X ± 1 SD) 106.9 t 17.56 mg/100 ml and 104.2 ± 12.53 mg/100 ml. The analysis test showed that there were no significant differences between blood plasma parameter of the two groups (p > 0.05).The result suggests that AFB, chronic toxicity on plasma lipid spectrum of rats is not proven despite the occurrence of earlier hepatotoxicosis. -Key Words: aflatoxin Bt -- hepatotoxicosis -- bile duct epithelial cells -- plasma lipid spectrum -- rat

Abstract

More than 40 years ago T. Duckett Jones published a set of guidelines for the diagnosis of acute rheumatic fever that became known as the Jones criteria. The criteria became widely used throughout the world and provided a uniform diagnostic reference for national and international epidemiologic, prevention and treatment studies.Although the basic concepts proposed by Jones have been retained, modifications of the original criteria should be done to simplify the criteria in order to be used as a diagnostic tool at the primary health care, type D and C hospitals and for surveillance. These new simple diagnostic instruments should have good sensitivity, specificity, positive and negative predictive values using Jones criteria as gold standard.Eight criteria have been considered as a new simple diagnostic instrument, with various sensitivity, specificity, positive predictive and negative predictive values. The optimum result of this diagnostic test is in the eight criteria, i. e. the criteria of specific murmur.Key Words: rheumatic fever -- cardiomegaly -- heart failure -- Jones criteria -- specific murmur

Abstract

A discussion of the impact -of abnormal labor on the neonatal morbidity and mortality with special reference to birth trauma and perinatal mortality has been carried out. This report is a literature review of cases found in Sardjito Hospital and an illustration of obstetric cases during the first semester of 1987. The aim of this paper is to find out how far the negative impact of abhor.. LI labor on birth trauma and perinatal mortality is, as well as preventive measures to be taken.It is obvious that abnormal vaginal deliveries give rise to an increase incidence of birth trauma compared to spontaneous deliveries. The higher the birth weight, the higher the incidence of birth trauma. Caput succedaneum is the most frequent type of birth trauma. whereas broken bone is the least frequent.Transverse position is the most unfavorable position of the fetus, giving the perinatal mortality rate (PMR) 775/1000 deliveries. The second one is breech presentation, having the PMR 129/1000 deliveries. The very unfavorable obstetric operatives are vacuum and forcipal extraction and internal podalic version which altogether bring about the PMR 210.5/1000 deliveries. Prolonged labor gives rise to the PMR 21/2 times higher compared to non-prolonged labor. Although the Cesarean rate is quite high (12.5%), its PMR is still higher. This signifies that Cesarean section has not been adequately accomplished.Some efforts to reduce the PMR in this hospital are (1) expecting the high risk pregnant women especially during labor to be referred as soon as possible, (2) fast and appropriate management of such cases with sophisticated instruments provided, and (3) providing an intensive neonatal care.Key Words: high risk pregnancy -- abnormal labor and delivery birth trauma -- perinatal mortality rate -- neonatal care
