Abstract

Background: Punctured Technique of Vasectomy is a simple and refined method of vasectomy. This method was introduced to make fewer complications and to remove the fear of surgical knife.Objective: This study was aimed to assess the ease of procedure, safety, and effectiveness of Punctured Technique of Vasectomy.Methods: A prospective study of 80 acceptors has been done at Dr. Sardjito Hospital Yogyakarta from January 1991 up to Jury 1991. These subjects were randomly distributed into two groups, Punctured Method (39) and Standard Method (41). Duration of operation and complication done during operation was recorded. The follow up for complication was recorded in the second week post-operative, and sperm analyses was done in the tenth week post-operative.Results: Duration of Punctured Method (9,7179 ± 2,9731 minutes) was significantly different (p-value =0,0006) compared to that of Standard Method (12,1951 ± 3,2421 minutes). There were two complications during Punctured Method operation compared to six complications during Standard Method operation (RR =1,54; 95%Cl: 0,97 - 2,46). In the second post-operative week, there was no complication in the Punctured Method group, while there was one wound infection in the Standard Method group. Sperm analysis both groups showed no active spermatozoon.Conclusion: Punctured Method is easier than Standard Method, and there was no difference of effectiveness in both methods.Key words: punctured vasectomy - complications - sperm - analysis

Abstract

Background: Previous studies showed that both systolic and diastolic blood pressures could be lowered to a modest degree with an aerobic type exercise.Objective: The present study was designed to explore the effect of resistance exercise training program on resting heart rate and blood pressure in males and females.Methods: Thirty-three normal healthy subjects participated in this study. Resting heart rate and resting blood pressure were measured before and after a weight-training program twice a week for eight weeks. Results: Data showed that resting heart rate was significantly (p) lower in males compared to that observed in females before and after the training program. A significant decrease in resting heart rate (p) was observed in both sexes as a result of the training program. Resting systolic and diastolic blood pressures were slightly (p = 0.08) lower in the female group compared to that observed in the male group before the training program. Resistance exercise training induced no significant change in resting systolic and diastolic blood pressures in both groups. After the training program, resting systolic and diastolic blood pressures were slightly (p=0.09) lower in the female group compared to that observed in the male group.Conclusion: Exercise stimulus in the form of circuit weight training was sufficient to elicit cardiovascular adaptations in response to training.Key words: resistance exercise training - heart rate - systolic and diastolic blood pressures -cardiovascular adaptations

Abstract

Background: Three issues about colorectal cancer are now blooming among surgeons and oncologists. The first issue is trend the younger the age of the patients suffering from colorectal cancer. The second is the younger the age of the patients the higher malignant of the cancer. The third issue is the delay of the management of colorectal cancer.Methods: A retrospective study on the colorectal cancer patients who were managed in Patmasuri Surgery Hospital, Yogyakarta, during a 10-year period (1989 - 1998).Results: The first and second issues were not proved, but the third issue was proved. The cause of delay came from the patient and the physician in establishing the diagnosis and in considering the operative procedure.Key words : colorectal cancer - younger age - higher malignant

Abstract

Background: Nonperiodic form of B. ma/ayi was recently reported from East Kalimantan by Sudjadi as a new sub-species of filarial worm. The parasite was highly prevalent among Dayak people living from shifting cultivation of paddy rice in deep forest. Wild mosquitoes exist in the highly endemic area of Krayan, Long Ikis district, Pasir regency.Objective: To show current transmission of filariasis and its main vector species in the area of indigenous inhabitants of Dayak.Method: Wild mosquitoes were collected using human bait by standard method at night and day time in different places in the Krayan area. Wild mosquitoes landing and feeding on him were collected, than killed in local laboratory, identified and dissected to find filarial larvae.Results: Seven genera or 21 species (amount of 22.942) of mosquitoes were collected from Krayan for dissection, and they were markly predominated by Mansonia (21.960 or 95.5%) mosquitoes. Transmission of filariasis was detected currently occured in the area with the main vector of Ma. bonneae. This mosquito species was not only discovered harbouring infective larvae of the disease, but was also found as markly predominant species among Mansonia collected, particulary in locations within the environment of thick forest.Conclusion: Current transmission of filariasis with Ma. bonneae, as the main vector recorded in the highly endemic area of indigenous Dayak people in Krayan, supports the nonperiodic form of B. malayi in East Kalimantan to be zoonotic, sylvanic and phyletically they are more primitive than the other B. malayi type.Key words: filariasis - nonperiodic form of B. malayi - transmission - main vector - Ma. bonneae

Abstract

Background: Intraocular surgery releases prostaglandin. Prostaglandin caused miosis and inflammatory reaction. Flurbiprofen and Indomethacin are antiprostaglandin agents.Objectives: This study was aimed to compare the effects of flurbiprofen and indomethacin eye drop and flurbiprofen tablet orally in preventing surgically induced miosis to dilated pupils during extracapsular cataract surgery.Methods: Measurement of the diameter of dilated pupil before and during extracapsular cataract surgery. Analysis variance and Tukey Studentized Rest Test were used to analyse the result.Results: No statitical significant difference was found in pupil diameter among the three groups except the stage of capsulotomy and inspiration-aspiration among those who received flurbiprofen and indomethacin eye drops.Conclusion: Flurbiprofen 0.03% eye drop was significantly stronger than Indomethacine eye drop 0.1% in preventing surgically induced miosis at extracapsular cataract surgery especially in both stage of capsulotomy and inspiration-aspiration and type manipulation.Keywords: cataract surgery - anterior capsulotomy - lens extraction - insiparition-aspiration - miosis -antiprostaglandin

Abstract

Background: Acute bacterial meningitis is one of paediatric emergency cases which results in high morbidity and mortality. Diagnostic procedures using cerebrospinal fluid (CSF) are often unsatisfactory. A simple and rapid procedure is required for early diagnosis and prompt treatment.Objectives: The aims of this study were to assess total cerebrospinal fluid creatine-kinase (CSF-CK) level in childhood acute bacterial meningitis and to assess the diagnostic values of CSF-CK level compare with routine CSF analysis (i.e. cell count, morphology, chemical, C reactive protein and culture) which is accepted as diagnostic gold standard for acute meningitis.Materials and methods: All patients hospitalized in the Pediatric Department Sardjito Hospital for suspected acute bacterial meningitis in July-December 1998, underwent lumbar puncture. The level of CSF-CK and routine analyses were examined for diagnosis acute bacterial meningitis.Results: Forty five children with suspected meningitis were included in this study. CSF-CK 16 U/I had sensitivity 50%, specificity 80%, positive predictive value 42%, negative predictive value 85% and likelihood ratio 2,75. Mean of CSF-CK in childhood acute bacterial meningitis was 82,84 U/I.Conclusion : CSF-CK level was not sensitive, but it is a specific diagnostic test for childhood bacterial meningitis.Key words: acute bacterial meningitis - CK-CSF - diagnostic test

Abstract

Background: Dengue hemorrhagic fever (DHF) is still a problem in Indonesia. Early diagnosis is important in the management. The cytoplasm of activated lymphocytes in dengue infection contains ribonucleic acid (RNA). RNA can be specifically shown by pyronin staining.Objective: To know the diagnostic value of lymphocyte pyroninophilic in DHF in children.Methods: Subjects were patients hospitalized in the Pediatric Department of Faculty of Medicine/Pediatric Ward Sardjito General Hospital, from September 1995 to December 1996, with 2-7 days of fever, positive tourniquet test, and 0-14 years of age. Clinical diagnosis of DHF was based on WHO 1986 criteria. Hemagglutination inhibition (HI) test and/or antibody antidengue were used to confirm the diagnosis of DHF. Peripheral blood smear was stained with pyronin stain according to Sutaryo modification. Lymphocyte percentage was counted per 100 leukocyte under light microscope.Results: Means of pyroninophilic lymphocyte numbers were increased from day-3 to day-5 of fever and then decreased. There were significant difference between the means in non-dengue infection and in dengue infection from day-3 to day-7 of fever, between dengue fever (DF) and DHF from day-3 to day-6 of fever, and between non shock (DHF-I and DHF-II) and shock cases (DHF-III and DHF-IV) from day-3 to day-9 of fever. The number of pyroninophilic lymphocytes correlated with the degree of DHF. Using cut-off point of 4 at day-5 of fever, the sensitivity was 86,2%, specificity was 84,3%, positiv predictive value was 92,6%, and negative predictive value was 65,1%.Conclusion: Pyroninophilic lymphocyte has a high diagnostic value in DHF in children.Key words: Dengue hemorrhage fever - ribonucleic acid - pyroninophilic lymphocyte - diagnosis

Abstract

Background: Macrophage represents one of the cellular component of the immune system which plays an important role during malarial infection. Both the number and functional activities including phagocyte activity of these cells increase during the infection.Objectives: This study was carried out to investigate the phagocyte activity of peritoneal macrophages from immunized and non-immunized mice during P. berghei infection.Methods: Swiss mice were divided into two groups, one experimental group was immunized by crude vaccine P. berghei, one control group was not immunized. Phagocyte activity was measured by the ability of mouse peritoneal macrophages to phagocytes latex particles in vitro.Results: In non-immunized mice the percentage of macrophages which were phagocyte latex particles was increased during early infection, reached a peak of about 9 times of the normal level then declined until the mice died. In the immunized mice this activity was increased to reach a peak of about 11 times of the normal level and remained high until recovery.Conclusion: Phagocyte activity of immunized-mice peritoneal macrophages was significantly higher than those of non immunized. The increase of the phagocyte activity seemed to be correlated with the ability of mice to overcome the infection.Key words : Immunization - P. berghei - Effector cells - Macrophages - Phagocytosis
