Abstract

Background: In CR, there is an indicator called Exposure Index (EI) which displays how much exposure is used for a particular examination. The Exposure Index depends on the exposure factor, the total area of the detector exposed to radiation, and the object being exposed (Bontrager Lampignano, 2014). One of the exposure factors that is often regulated by the radiographer is mAs. Milliampere seconds (mAs) determine the number of X-rays produced. The purpose of this study was to determine the differences in the Exposure Index Values on Chest Radiography with Changes in the Combination of Different mA and s Values at the Same mAs Value.Methods: This research is a type of quantitative research with a pre-experimental design. The object of this research is the thorax phantom. Data were collected by exposing thorax the chest phantom, then recording the resulting Exposure Index value. mA and s value variations that are set are 25mA x 320 ms, 160mA x 50 ms and 400mA x 20ms. Data analysis was carried out using statistical tests.Results: From the statistical test results it is known that there is no significant difference in the Exposure Index Value on Thorax Radiography between the 25mA x 320 ms combination group, the 160mA x 50 ms combination and the 400mA x 20ms combination with a p-value 0.05 (p-value = 0.151) . The reason for the absence of a difference in the Exposure Index value is the variation in the combination of different mA and s values for the same mAs because with the same mAs, the quantity / intensity value of the X-ray radiation produced is also the same or there is no increase or decrease. With the same x-ray radiation intensity, the radiation received by the Imaging Plate is also the same so that the value of the Exposure index is also the same.Conclusions: There is no significant difference in the Exposure Index Value on Chest Radiography with Changes in the Combination of Different mA and s Values at the Same mAs Value

Abstract

Background: Organizational performance is something produced by an organization in a certain period with reference to established standards. Management performance measurement aims at work efficiency using balanced score card (BSC), which is a performance measurement solution. BSC consists of two  dimensions: financial and non-financial dimensions. The purpose of the study was to describe the measurement of the performance of the Madiun City general hospital using the Balance Scorecard method.Methods: Research design uses qualitative descriptive methods. The selection of informants was obtained from observations and interviews to employees of the Madiun Citygeneral hospital with 7 respondent. This study used purposive sampling techniques. Research instruments with observation sheets, interview guidelines and documentation studysheets.Results: The results of the study at the general hospital in Madiun City area showed that financial performance was considered good, the achievement of customer satisfaction levels in 2020 was quite good, the perspective of internal business processes could not be achieved at standard numbers due to the Covid-19 pandemic, the achievement of employee satisfaction levels in the good category even though there were 3 elements that were considered less good.Conclusions: The results of the research conducted are expected by the hospital should use performance assessment based on the concept of balance score card to implement and integrate the strategic planning of the hospital. Hospitals also need to improve employee capabilities by providing training to employees in turns, or organizing inhouse training to save costs. The perspective of benefit performance is that the poor must be served in Madiun City Hospital.

Abstract

Background: Tube voltage (kVp) is one of the exposure factors on a head CT scan that can be varied to achieve a minimal radiation dose. Radiology installation of Bhakti Wira Tamtama hospital has a new CT scan modality that never variation its tube voltage. The variation of tube voltage is 80 kVp, 100 kVp, and 120 kVp. The aim of this research is to know and to analyse the most optimize tube voltage which can produce the best anatomy clarity and has the lowest radiation dose on a head CT scan examination of the base skull area that dominates CT scan examination.Methods: This research is descriptive analytic research conducted with an experimental approach. The study was conducted by varying the tube voltage to be used for head CT scan examination. There are 3 variations of tube voltage 80 kVp, 100 kVp, and 120 kVp where one variation contains 16 images for analyzing the clear anatomy of the base skull area. Then assessed by radiologists as the respondent. Data was analyzed by scoring where score 1 is not clear, score 2 is clear and score 3 is very clear. Radiation dose was obtained from CTDI and DLP records.Results: The results showed that there is no difference in anatomy clarity between variation of three tube voltage with p value 0,135. The anatomical clarity assessed includes anterior skull base, central skull base (fossa cranial middle, sphenoid, temporal bone, petrosous ridge), anterior clinoid processes, posterior margin of the lesser sphenoid wings, anterior and superior rim of the greater sphenoid wings, mastoid, and posterior skull base. All of them is very clear at 100 kVp and 120 kVp with value 100%. But at 80 kVp assessed clear at anterior and posterior skull base. So the value of anatomy clarity at 80 kVp is very clear only 71%. The effect of the difference in tube voltage on the radiation dose on a head CT scan of the base skull area showed a difference in the decrease in radiation dose from 120 kVp to 100 kVp for CTDI 39.9% and DLP 40.19%. There was a decrease in radiation dose from 100 kVp to 80 kVp for CTDI 52% and DLP 49.88%.Conclusion: The tube voltage setting on head CT scan examination of the base skull area can be applied with 100 kVp because this is evidenced by the same total score at 100 kVp and 120 kVp, which is 100% is very clear, while at 80 kVp tube voltage only 71% is very clear. And the radiation dose at 100 kVp is lower than 120 kVp.

Abstract

Backgroud: blurring artifact is an issue in axial cervical MRI using T2 Turbo Spin Echo (TSE) weighted Cartesian. T2W TSE BLADE is potential to substitute its shortcomings. This study aims to analyze the image quality and artifacts between T2W TSE Cartesian and T2W TSE BLADE on axial cervical MRI examination and to determine the better sequence in producing image quality.Methods: this is a quantitative research with descriptive approach. 7 volunteers were scanned for cervical axial examination with T2W TSE, T2W TSE Cartesian and T2W TSE Blade. Images were processed in MathlabTM. SNR of intervertebral disc, spinal cord and CSF were measured using ROI. Image resolution were find out from pixel spacing in DICOM info. Blurring artifact were analyzed by comparing the signal of blurred area to the background noise. Data were analyzed using descriptive statistics.Results: SNR of T2W TSE Cartesian and T2W TSE BLADE were 22,144 and 17,532 (intervertebral disc), 20,952 and 16,786 (spinal cord), and 16,717 and 12,883 (CSF) respectively. Image resolution (mean) were 0,391 pixel/mm (T2W TSE Cartesian) and 0,625 (T2W TSE BLADE). Blurring artifact were 8,161 (T2W TSE Cartesian) and 6,826 (T2W TSE Blade).Conclusion: T2W TSE Cartesian was superior in pixel intensity and SNR, while the blade technique was superior in terms of spatial resolution and artifacts reduction. Thus, it is recommended to be applied in clinical field if the SNR or image detail is the priority.

Abstract

Background: The implementation of health services is more focused on improving, maintaining, and protecting health, so it is not only focused on recovery or healing of diseases. Hospital as a referral level health service institution plays an important role in encouraging all stakeholders to commit to promotive and preventive efforts in preventing and reducing health risks faced by Patients, Patient Families, Hospital Human Resources, Hospital Visitors and the community, and to keep them in goodhealth. The purpose of the study to analyze the effectiveness of education in Covid 19 patients using SAJI techniques against changes in the knowledge, motivation and behavior of Covid 19 patients in the Special Isolation Room of Karsa Husada Batu General Hospital.Methods: Type of quantitative research with Pre-experimental design and One-Group Pretest-Posttest Research Design approach. The population of all covid 19 patients in isolation rooms amounted to 55 respondents. A sample of 46 respondents with Morgan's table, a total sampling technique. Independent variables of knowledge. Variable intervening motovation. Behavioral dependent variables.Results: There was a significant difference between knowledge before and after education (sig. = 0.000). There is a significant difference between motivation before and after education (sig. = 0.000). There is a significant difference between the behavior before and after education (sig. = 0.000). The motivation variable significantly serves as an intervening variable between the influence of knowledge variables on behavior (sig. = 0.01).Conclusions: It is hoped that by educating covid patients using present techniques can increase knowledge, motivation and behavior of Covid 19 recovery.

Abstract

Background: Health workers in a hospital environment are very at high risk for infection with diseases that can threaten their health and safety whileworking. Compliance in using PPE should take precedence to provide protection to yourself and to patients. The purpose of the study was to analyze the difference in levels (workspaces) related to the influence of knowledge, working life and supervision of compliance with the use of personal protective equipment in Nurses and Other Health Workers at Menur Surabaya Mental Hospital.Methods: Quantitative research design of analytical observational methods. The population of all paramedics at Menur Mental Hospital was 183 respondents. A sample of 126 respondents with proportional random sampling technique. Independent variables of knowledge, working life and supervision. Variable dependent compliance with the use of Personal Protective Equipment (PPE).Results: The majority of respondents' knowledge was more than the mean of 75 respondents (49.5%). The working period of the respondents was more than 1 year, which was 80 respondents (63.5%). Most of the surveillance on respondents was on the morning sift of 62 respondents (49.2). Compliance of 63 respondents (50.0%). Statistical test results  have  an influence of knowledge on compliance with the use of personal protective equipment in Nurses and Other Health Workers  (p = 0.000) and the effect of working life on compliance with the use of personal protective equipment on Nurses and Other Health Workers (p =0.000). And the influence of supervision on compliance with the use of personal protective equipment on Nurse and   (p =0.042). The results of the multivariate analysis  showed a significant relationship (p= 0.003). Astatistically significant difference between models without taking into account contextual influences and models that take into account contextual influences  (likelihoodratio = 0.0038).Conclusions: It is expected that by having goodknowledge, working life, and supervision of compliance with the use of PPE, paramedics will be more compliant with protection as a prevention of infection transmission.

Abstract

Background: Until now, Indonesia has become one of the countries with the highest health worker death rate in ASIA and the 3rd highest in the world due to exposure to COVID-19, reaching 647 people. Radiographers are one of the first lines of health professionals who are very likely to be exposed to COVID-19. The purpose of the study is to find out the adaptation of new radiographer habits during the COVID-19 pandemic including radiographer work experience, radiographer health conditions (including fisk and mental health) and radiographer resilience in pandemic times.Methods: Research design uses a qualitative approach with phenomenological types of research. The selection of informants is obtained from observations and interviews to radiographers. The study used purposive sampling techniques. Research instruments use observation sheets, interview guidelines and documentation study sheets. This research was conducted in the Radiology Unit at COVID-19 referral hospitals, public hospitals and private hospitals, around the city area of Surabaya, east Java. The data collection time is expected to be held in April - June 2021.Results: The results showed the work experience of radiographers before the pandemic period was very different from the experience during the pandemic. Radiographers experienced more convenience when working in the time before the pandemic. Much of the change lies in the use of personal protective equipment. The majority of respondents had high optimism about their health condition as well as a great sense of responsibility in caring for patients suspected of Covid-19. The majority of respondents continue to carry out the obligation to treat patients suspected of Covid-19 or who are confirmed positive for Covid-19 because it is an obligation for every radiographer. These differences include restrictions on the number of radiographers in the room, the use of PPE at level 3, and the screening process that must be done before examining patients.Conclusions: The role of radiographers is very important in carrying out radiological examinations, especially in pandemic times. Radiographers who are competent and comply with the SOP during the pandemic so as to minimize exposure to Covid 19.

Abstract

Background: Hospitals as institutions providing health services must keep abreast of technological developments, so as to be able to compete  in a healthy manner with other hospitals to create the best service quality for customers. The purpose of the study was to determine the evaluation of linen management at Madiun hospital city in 2020.Methods: Observational research design using descriptive qualitative method. The selection of informants through primary data and secondary data. The number of main informants is 4 people and triangulation informants are 3 people. This research uses interview, documentation and triangulation techniques. Collecting data using in-depth interviews, observation and documentation studies. The research was conducted at the Madiun hospital city in March-April 2021. The data analysis techniques used data reduction, data presentation and conclusion drawing.Results: The results of research that has been carried out at the Madiun hospital city regarding evaluation of linen management. In general, The input components (human resources, infrastructure, guidelines and Standard Operating Procedure) in the management of linen at Madiun hospital city are not in accordance with the Hospital Linen Management Guidelines. The process components (planning, implementation of activities, and control) in the management of linen are generally not in accordance with the Hospital Linen Management Guidelines. The output component of linen management is the implementation of linen management that is not in accordance with the SPM (Minimum Service Standards) indicator standard.Conclusions: The obstacles that arise are due to constraints in input aspects such as human resources, policies and infrastructure that are not appropriate, causing the linen management process flow to be not optimal. As in the implementation process, there are still things that are not running according to Standard Operating Procedure. There has not been strong coordination between linen managers in the laundry and in the room so that it proves the weak control of facilities and infrastructure, linen inventory, quality and age of linen.

Abstract

Background: The leader is a real symbol of power. The success of an organization in achieving its goals is largely determined by the ability of its leaders. Every workforce is entitled to protection for occupational safety and health (K3) in doing work. The purpose of the study is to analyze the influence between transformational leadership and health worker participation on the improvement of K3 implementation commitment at Mardi Waluyo Blitar Hospital.Methods: Design observational research with a cross-sectional approach. The population of all health workers in Mardi Waluyo Hospital in Blitar City amounted to 834 people, while sampling techniques were conducted by probability sampling technique using Proportionate Stratified Random Sampling. The samples were obtained from Kerjcie and Morgan's table of 260 people with total sampling techniques. Independent variables are transformational leadership and the role of healthcare workers. The dependent variable is the K3 implementation commitment. The analyses used are univariate, bivariate and multivariate.Results: The results showed that there was a co-influence between transformational leadership (p=0.000) and health worker participation (p=0.021) on the increased commitment to K3 implementation at Mardi Waluyo Blitar Hospital. Multivariate analysis shows the most dominant variables are transformational leadership with significance levels p=0.000 and B=0.274.Conclusions: By participating in the research, it is expected that respondents can improve the quality of health services in hospitals by improving the implementation of K3 as a guideline or standard in providing quality health services and prioritizing patient safety and preventing occupational accidents in health workers.

Abstract

Background: With the times, Convention Radiography (RK) was replaced by Digital Radiography (RD) because film printing is more efficient, archiving can be done digitally, reducing the risk of losing film due to file backups, and is more environmentally friendly because it does not require chemicals. The purpose of the study was to analyze the perceived ease of use influence on the radiographer's confidence from the digital radiography tool so that word of mouth was achieved to other radiographers at the Covid-19 Referral Hospital.Methods: Design observational research with a cross-sectional approach. The population of all radiographers from 180 Covid 19 referral hospitals has used Digital Radiography. A sample of 170 respondents with cluster random sampling technique. The variables in the study were perceived ease of use, quality of service, user confidence and word of mounth. Research instruments use questionnaire sheets. The analysis used is bivariate analysis and path analysis with SPSS 23 and STATA 13.Results: Design observational research with a cross-sectional approach. The population of all radiographers from 180 Covid 19 referral hospitals has used Digital Radiography. A sample of 170 respondents with cluster random sampling technique. The variables in the study were perceived ease of use, quality of service, user confidence and word of mounth. Research instruments use questionnaire sheets. The analysis used is bivariate analysis and path analysis with SPSS 23 and STATA 13.Conclusions: The development of this technology is used as an excuse that facilitates the subscription of transportation services in fulfilling the quality of their services and makes it easier for consumers or the public to access health services. Healthcare providers mostly achieve success because they are influenced by word of mouth communication or word of mouth marketing.
