Abstract

Life expectancy increases along with improving health. A good healthy life as an impact that there are still many workers in old age who are still needed. The effect of age and work environment will affect the normal range of time-domain heart rate variability (HRV) in older worker. Heart rate variability test might detect autonomic nervous system influenced by blood pressure level, stress, and working environment. A low HRV value is considered an independent marker of risk of death. However, age-related decline in HRV may limit its predictive value, especially in older workers. It was found that the decrease in HRV decreased with age and length of service. The elevation of HRV at specific wavelengths was correlated with the blood pressure values of hypertension patients; however, in the uncontrolled hypertensive group, the values of SDNN, SDANN, ASDNN, VLF, LF (indicating sympathovagal balance), and HF were significantly lower.

Abstract

Peripheral Arterial Disease (PAP) requires good evaluation to reduce amputation rate. The aim of this study is to assess non-invasive detection methods in evaluating microvascular conditions in PAP. The potential use of Near Infrared Spectroscopy (NIRS) in identifying disorders in patients with PAP, either with or without DM, was also assessed. A systematic literature search was conducted through PUBMED until October 2023. Based on the literature review, it can be concluded that the use of NIRS in evaluating microvascular status in PAP is a non-invasive testing method that has expanded its role. Evaluation with NIRS provides metrics such as recovery time, deoxygenation, oxygen consumption (VO2), tissue oxygen saturation (StO2), total haemoglobin (HbT), and oxyhemoglobin area under the curve (O2 HbAUC). NIRS also has the potential to assist multidisciplinary teams in treating high-risk patients, especially in diabetic patients whose symptoms are compensated by peripheral neuropathy. This study shows that NIRS can be used as a useful tool in evaluating microvascular status in PAP, by providing important information that can be used to optimise patient care and prevent more serious complications. It is concluded that NIRS can be used as a tool to evaluate the effectiveness of treatment and prevent worsening of the condition of patients with PAP in diabetic foot.

Abstract

Fibrosis lung diseases such as chronic obstructive pulmonary disease, acute lung injury and post-Covid 19 fibrosis lung disease are considered as major health problems in the last decade. Cellular therapy with Mesenchymal Stem Cells (MSC) offers a novel therapeutic approach for fibrosis lung disease associated with anti-inflammatory, immunomodulatory, regenerative, pro-angiogenic and anti-fibrotic properties. Such therapeutic effects may be attributed to the MSC-secretome, which is made of free soluble proteins and extracellular vesicles (EVs). This review summarises some recent literature findings related to the efficacy and safety of MSC-derived products in pre-clinical models of lung diseases, demonstrating the biologically active substances contained in the MSC-secretome and its mechanisms involved in tissue regeneration. A perspective view is given on the secretome being a high-quality, safe and effective medicinal product,

Abstract

Background: Coronavirus (Covid-19) has spread rapidly to more than 200 countries, including Indonesia. Adjuvant therapy of Vitamin C, D, Zinc has become beneficial for Covid-19 treatment to improve the immune system, while Curcumin is still not standard therapy. Curcumin can stimulate specific immune cells and can reduce oxidative stress. There is currently no scientific evidence showing that the addition of Curcumin can protect COVID-19, especially as an anti-inflammation and anticoagulant effect. Methods: This cross-sectional study was conducted at Ukrida Hospital from January -June 2021. The inclusion criteria are Mild-Moderate Covid-19 use on Curcumin and Supplement therapy ( Vitamin C, Vitamin D, and zinc). Data analyses with SPSS v.20 use Anova test and Kruskall Walis test. This study had been approved by The Institutional Board of the Faculty of Medicine Universitas Krida Wacana, Indonesia (No. 1053/SLKE-IM/UKKW/FKIK/KE/II/2021). Research Objectives: To see the effect of Curcumin as an additional therapy from Covid-19 by clinical and laboratory inflammation indicators of the usefulness of Curcumin are such as ratio neutrophil/lymphocyte, ratio platelet/lymphocyte, erythrocyte sedimentation rate (ESR), C-Reactive Protein ( CRP), and D Dimer. Results: There were 246 cases, with 62.6% male and 56.5% female taking Curcumin. Curcumin has a good effect in reducing inflammation and coagulation in mild-moderate Covid-19. Patient who got Curcumin or without Curcumin as additional supplement therapy shows patient that got Curcumin has decreased inflammatory markers N/L ratio (p = 0.041), P/L ratio (p = 0.040), ESR (p = 0.013 ), and CRP ( p= 0.045). Curcumin also has an anticoagulant effect that shows a decrease of D-Dimer ( p= 0.020). Conclusion: Curcumin is beneficial in mild-moderate Covid-19 as an anti-inflammatory and anticoagulant.
