Abstract

Hypertension is one of the main health problems in Indonesia, which if not controlled will result in health, psychological, social and economic burdens. A common clinical consequence of hypertension is impaired cognitive function, namely dementia and senility, which can be examined through the Indonesian version of the Montreal Cognitive Assessment (MoCA-Ina). The service program which is carried out by the Faculty of Medicine, Universitas Sumatera Utara (FK USU) is an examination of cognitive function and blood pressure in Padang Bulan Selayang II region. Examinations are carried out in accordance with the 2017 World Alzheimer Month and are followed by lay seminars on dementia, brain gym exercises (gerak latih otak/GLO) as well as brain teasers. The activity involved 26 participants from integrated development post (pos pembinaan terpadu/Posbindu) with a mean systolic blood pressure (TDS) 131.54 + 18.70 mmHg, mean diastolic blood pressure (TDD) 83.85 + 8.98 mmHg, mean mean arterial pressure 99, 69 + 9.47 mmHg and a mean MoCA-Ina score of22 + 5.03. Blood pressure monitoring, education of community awareness and cognitive stimulation at regular intervals are expected to prevent and reduce the incidence of hypertension, dementia and senility.

Abstract

Gangguan hormon tiroid merupakan salah satu gangguan endokrin yang utama mewakili 30% hingga 40% dari pasien yang datang dalam praktek endokrin. Gangguan hormon tiroid merupakan salah satu penyebab gangguan fungsi kognitif. Tujuan dari penelitian ini untuk mengetahui adanya hubungan tingkat TSH dengan fungsi kognitif pasien dengan gangguan fungsi tiroid. Penelitian ini merupakan studi analitik dengan desain potong lintang, pada pasien dengan gangguan fungsi tiroid yang berobat di Poliklinik Endokrin RSUP Haji Adam Malik Medan. Tingkat TSH dikelompokkan sebagai tinggi (>4,67?IU/ml), normal (0,49-4,67?IU/ml) dan rendah (<0,49?IU/ml). Fungsi kognitif dinilai dengan menggunakan Montreal Cognitive Assessment-Versi Indonesia (MoCA-INA) dengan nilai normal 26-30. Pada penelitian ini melibatkan 55 pasien yang terdiri dari 11 pasien laki-laki (20%) dan 44 pasien perempuan (80%). Rerata usia adalah 41,69±11,50 tahun. Terdapat 13 pasien tingkat TSH tinggi (23,6%), 15 pasien tingkat TSH normal (27,3%) dan 27 pasien tingkat TSH rendah (49,1%). Nilai median MoCA-INA adalah 24(9-30). Hasil uji Kruskal-Wallis menunjukkan hubungan yang signifikan antara tingkat TSH dengan fungsi kognitif (p=0,017).

Abstract

Chronic kidney disease (CKD) is an independent risk factor for cognitive impairment in all domains, especially delayed memory and executive function. The purpose of this study was to determine the correlation between chronic kidney disease severity and cognitive function. This study used a cross-sectional design in stage III, IV, and V CKD patients in the Nephrology Polyclinic of Haji Adam Malik Central General Hospital. Cognitive function tests were performed using the Montreal Cognitive Assessment (MoCA INA), digit span, and Trail Making Test A & B. The Spearman test was used to analyze the correlation between CKD severity and cognitive function. This study involved 45 chronic kidney disease patients consisting of 28 (62.2%) males and 17 (37.8%) females with a mean age of 49.67±12.18 years. The results of statistical analysis showed that there was a significant positive correlation between CKD on the MoCA-INA examination (r=0.618, p=<0.001), FDS (r=0.414, p=0.005), there was a significant negative correlation on the TMT A time examination (r=-0.425, p=0.004), TMT A error (r=-0.497, p=0.001), TMT B time (r=-0.618, p=<0.001), TMT B error (r=-0.370, p=0.012). The results of this study prove a significant correlation between the severity of CKD and cognitive function.
