Abstract

Scabies is an infectious disease caused by infestation of Sarcoptes scabiei hominis variants and their products. Scabies is transmitted through direct skin-to-skin contact or indirect contact through shared objects, such as towels, clothing, bed linen. Lack of knowledge and personal hygiene can lead to scabies. This study is a case report based on Evidence Base Medicine in male patients aged 42 years by identifying risk factors and clinical problems and management of patients based on patient problems as well as the patient centered approach and family approach. Primary data were obtained through history taking (autoanamnesis), physical examination and home visits. Secondary data was obtained from the patient's medical record. Assessment is based on a holistic diagnosis from the beginning, process, and end of study in a qualitative and quantitative manner. Tn.Y patient aged 42 years was diagnosed with scabies, has an internal risk aspect in the form of the habit of rarely washing his hands after the activity and habit of using toiletries together. Aspects of external risks in the form of a dense residential environment, slums, and irregular arrangement of goods and there are similar complaints in the families of patients who live at home. Holistic management with a family doctor approach can improve knowledge, attitudes, and behavior in patients. In addition, the role of the family is very important in the care and treatment of sick family members.

Abstract

Fever is an acute infectious disease caused by Salmonella typhi bacteria. Fever often occurs in several countries in the world and generally occurs in countries with low levels of cleanliness. Based on data from WHO (World Health Organization) estimates the incidence rate in the world around 17 million people per year, the death rate from typhoid fever reaches 600,000 and 70% occur in Asia. To identify risk factors and problems with patient management based on the patient's problem by seeking family approval. This study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. Assessment is based on a preliminary, process, and final diagnosis of a qualitative and quantitative study. Patients have functional degrees 2 with typhoid fever which has internal risk factors, namely lack of knowledge related to typhoid fever and the process of infection of the surrounding environment. External risk factors are unclean home environment. Then educating patients and families about typhoid fever for a clean and healthy life. Clinical problems in patients need attention to changes in clean and healthy living behavior. Health workers support not only solving clinical problems, but also finding and providing solutions to problems in the environment that affect the health of patients and families

Abstract

A holistic and comprehensive family medicine approach in detecting internal-external factors, prioritizing family-approached and patient centers is hoped to be able to improve the quality of life of patients. In this case, diagnosis and management were carried out with the concept of family medicine. Primary data were obtained through history taking, physical examination and home visits. Secondary data comes from patient medical records. The case of the patient, Mrs. 26 years old, had drug withdrawal TB (ICD X A15.0). The complaint was felt to have reappeared since three months ago, but the patient still had functional degree 2 in carrying out daily activities. Patients have internal risk factors, namely the patient's lack of knowledge of the disease, inappropriate lifestyle, curative treatment behavior and lack of medication adherence. External factors possessed by the patient are a family with low income (ICD10 – Z59.6), poor lighting, poor ventilation in the house, a densely populated home environment, inadequate cleanliness of the home environment and contact with patients with prolonged coughs. Handling and treatment efforts in a holistic and comprehensive manner using a family medicine approach in the form of education on causes, transmission, treatment and prevention of disease.
