Abstrak - Sistem Peminjaman Berkas Rekam Medis ini dibuat karena berdasarkan dari pencatatan pencatatan peminjaman dan berkas-berkas medis. Sistem peminjman rekam medis ini terkomputerisasi, medis peminjaman dan berkas rekam medis tidak lagi dikukan secara manual. Tujuan dari sistem ini adalah untuk memudahkan petugas dalam melakukan pencatatan pemijaman dan mengontrol rekam medis. Peminjaman dan berkas berkas medis ini harus dikontrol dan dikendalikan dengan baik untuk mengurangi rekam medis dan miss file. Penelitian kualitatif deskriftif meliputi pengumpulan data atau observasi dan dokumentasi di RSUD Majalengka. Metode yang digunakan dalam penelitian ini yaitu metode waterfall(Pers, 2012). Tahapan Yang dilakukan Yaitu definisi persyaratan, sistem dan perangkat lunak desain, implementasi dan unit testing, Dan integrasi dan pengujian unit. Sistem informasi ini menggunakan design Flowchart, diagram konteks, data flow diagram (DFD) dan entity relation diagram (ERD) kemudian mengimplementasikan hasil desain ke dalam bahasa pemograman Visual Basic menggunakan aplikasi Microsoft Visual Studio 2010 . Hasil penelitian adalah sistem peminjaman dan kebijakan Berkas Rekam Medis di RSUD Majalengka. Kata Kunci : Peminjaman, Air Terjun dan Rekam Medis

Distribusi dokumen rekam medis sangatlah penting untuk kegiatan penyelenggaraan rekam medis, semakin cepat dokumen rekam medis sampai ke unit pelayanan makan semakin cepat juga pasien mendapatkan pelayanan kesehatan. pendistribusian dituntut untuk memenuhi standar pelayanan minimal ?10 menit, tujuan penelitian ini adalah untuk menganalisa waktu pendistribusian dokumen rekam medis di RSJ provinsi Jawa barat, jenis penelitian ini menggunakan pendekatan kualitatif, pengumpulan data melalui observasi petugas distribusi di unit rekam medis RSJ provinsi Jawa barat. Persentase ketepatan waktu sesuai standar pelayanan minimal adalah 88 berkas (88%) dan yang belum memenuhi standar 12 berkas (12%), rata rata waktu yang di perlukan untuk dokumen yang memenuhi standar pelayanan minimal adalah 4,5 menit per dokumen rekam medis, Kesimpulan dari penelitian ini adalah masih ditemukan adanya keterlambatan waktu pendistribusian rekam medis di RSJ provinsi Jawa barat, dengan menambah petugas distribusi dapat meningkatkan jumlah berkas yang memenuhi standar pelayanan minimal ?10 menit..

The research conducted was motivated by retention activities in the hospital dr. M.Salamun where the activity is still used manually by way of officers sorting one by one medical records files that will be destroyed. The retention process that is done affects the performance of the officers because it requires energy and a long time. This research aims to create a retention information system so that it will greatly help medical records officers in processing medical records that will be destroyed and make retention reports. This research uses qualitative methods with descriptive approaches and for development methods used, namely waterfall methods with stages of system needs analysis, design, code, testing and maintenance. Observations, interviews and literature studies are conducted to collect data. For the software design stage starting from the flowmap creation stage, Data Flow Diagram level 0, Entity Relationship Diagram. Visual basic.Net is the programming language used while for databases using Microsoft access. The results of the research conducted showed that the retention information system created can make it easier for medical record personnel to sort out active and inactive medical records. This information system is also becoming more well integrated and effective and efficient.

Medical Resume is a record/summary of results of entire period of patient care and treatment carried out by health personnel concerned. Medical Resume used as a requirement for one of BPJS claims at hospital. The use of information technology at this time must continue to grow, to create quality information. Method used is quantitative descriptive analysis, data collection used observation techniques, and software development method used Waterfall method through stages of needs analysis, system design, code writing and implementation. Purpose of this study was to determine the Design of a Medical Resume Completeness Information System for BPJS Claims at Sartika Asih Hospital Bandung. Medical Resume Completeness System at Sartika Asih Hospital is still done manually with Microsoft Excel so it is less effective, it is necessary to have an efficient information system to make it easier to check the completeness of the medical resume if there is a medical resume that staff can return it immediately to doctor who is responsible for replenishment. By the information system created, it is hoped that it will make it easier for staff to check the completeness of medical resumes quickly and efficiently so that the BPJS claim process is not obstructed.

Penelitian ini bertujuan untuk mengetahui pengaruh kelengkapan formulir resume medis rawat inap terhadap mutu rekam medis di RSUD Kabupaten Sumedang. Metode penelitian yang digunakan adalah kuantitatif dengan pendekatan deskriptif. Teknik pengumpulan data yang digunakan adalah dengan cara penyebaran kuisioner, observasi, dan kajian pustaka. Dari hasil uji hipotesis maka diketahui bahwa t hitung 7,140 dan t tabel = 1,666 maka H0 ditolak dan H1 diterima, sehingga jika dinyatakan dalam perentasi maka pengaruh kelengkapan formulir resume medis terhadap mutu rekam medis di RSUD Kabupaten Sumedang adalah sebesar 41,1%, sisanya 58,9% dipengaruhi variabel lain yang tidak diteliti dalam penelitian ini. Dari penelitian yang dilakukan, permasalahan yang dihadapi antara lain: (1) Masih adanya formulir resume medis rawat inap yang tidak diisi lengkap, (2) Mutu rekam medis di RSUD Kabupaten Sumedang belum berkualitas dikarenakan kurangnya sosialiasi antara petugas rekam medis dengan dokter akan pentingnya mengisi rekam medis, dan (3) Masih ditemukan pengisian resume medis yang tidak sesuai dengan standar operasional prosedur. Saran yang diberikan untuk permasalahan tersebut adalah (1) petugas rekam medis segera menghubungi dokter untuk melengkapi. (2) petugas rekam medis harus mensosialisasikan kepada dokter akan pentingnya pengisian rekam medis yang lengkap dan jelas khususnya bagian resume medis (3) Dibentuknya tim Review rekam medis yang beranggotakan wakilan dari setiap petugas medis.

This study aims to determine the influence of borrowing outpatient medical record files to quality of medical record installation at UPT RSUD Lembang The method used is a quantitative research method with a descriptive approach. Data collection techniques used are the method of observation, questionnaires and are equipped with a literature review. The sampling technique uses saturated sampling technique with a sample of 11 Medical Record Officers. The results of the calculations performed using a questionnaire with a simple linear test model, the number of samples N = 11 and significance α = 5% obtained tcount = 7.384 and t table = 1.833 the results is t count> t table then H0 is rejected and H1 is accepted, that means borrowing medical records influential outpatient on the quality of medical record installations at the UPT RSUD Lembang. The effect is 86% and 14% is influenced by other variables or errors (e). From the research conducted, problems found include: (1) The absence of outguide instructions when borrowing outpatient medical records at the medical record storage rack, (2) There is no handover at the time of return of medical record, (3) There are still delays in returning medical records and (4) There are still medical records that have not been stored in the medical record storage rack. Based on research conducted, researchers provide advice to the hospital as follows: (1) The existence of the SOP socialization Sso that medical record return in a timely manner, (2) Efforts to arrange a good medical record storage system, accelerate the service so that it does not cause patients to wait a long time (3) Imposing strict sanctions against officers so there is no misunderstanding of the existence of borrowed medical records, (4) The hospital shall use an outguide so that the medical record is clear of its existence
