Abstract

Work from home (WFH) mandate is one of the major changes known during this pandemic, aimed as a preventive way to mitigate the spread of the COVID-19 virus. This study aimed to observe the characteristics of pediatric burn injury during COVID-19 pandemic and WFH mandate's impact on pediatric burn injury admission at some Hospital burn centers in Malang. Every patient’s age, gender, clinical characteristics, parent's background, and other variables such as the possession of siblings, response time using our burn registry form, and comparative analysis of the incident in WFH housewife mother were assessed. The majority were in the group age of under five years old group age (70%) with a mean of 5.5 years. The most frequent part of the burn injured is extremity 36.7%, and hot liquid dominates as the cause of the injury 73.3% with the total body surface area of burn injury group >10% is the most common 56.7%. The burn injury incident happened more frequently in mothers with children less than two in both groups. This study showed that the increase in increasement of the pediatric burn injury during COVID-19 pandemic between housewife mother and WFH mother has no significant difference also showed that parent especially mother unable to supervise the children during WFH. Strategies to mitigate pediatric burn injuries during WFH should be thoughtfully implemented.

Abstract

Background: Cleft lip is a congenital birth defect caused by many proteins. Transforming growth factor (TGF)-β1, p38 mitogen-activated protein kinase (MAPK) and extracellular signal-regulated protein kinase (ERK)-1 are proteins which regulate proliferation and apoptosis role during intrauterine period. This study aimed to observe the expression of these proteins in cleft affected tissue of the lip.Materials and Methods: A descriptive study by examining the TGF-β1, p38 MAPK, and ERK-1 immunohistochemical expression of cleft affected tissue of the lip was conducted. Subjects were patients that were participating for the social event held by Plastic Surgery Department, Faculty of Medicine, Univesitas Brawijaya, on December 3-12, 2012 in Nusa Tenggara Timur. Excess lip mucosa (waste tissue) during the operation were stored in 10% formalin then stained by immunohistochemistry for TGF-β1, p38 MAPK, and ERK-1. We counted the average protein expression under the light microscope with 1000x magnification for 20 different fields of view, randomly.Results: Paraffin blocks from 30 subjects were selected. The mean p38 MAPK expression was found to be highest, with the average of 8 per field of view; followed by the mean TGF-β1 expression, with the average of 5 per field of view; and the mean ERK-1 expression was found to be the lowest, with the average of 2 per field of view. Conclusion: Expression of p38 MAPK and TGF-β1 are higher than ERK-1, suggesting that p38 MAPK is in the same signalling pathway as TGF-β1, while ERK-1 is lower, as its role as anti-apoptotic. This is consistent with several previous studies showing that all proteins took part in the development of cleft lip or craniofacial development. Further study needs to be conducted to determine which protein plays the bigger role.Keywords: cleft lip, TGF-β1, p38 MAPK, ERK-1

Abstract

Introduction : Hypertrophic scarring is a complication that occurs in post-surgical wounds. There are many ways to prevent scarring, but there have been no satisfactory methods yet. Moreover, no studies investigated the effectiveness of intradermal sutures. Polypropylene thread can be used to repair a scar after surgery. This study aimed to prove the effectiveness of intradermal sutures using polypropylene thread on surgical scar quality.Method : The study was conducted using a randomized, controlled trial, and post-test only with 20 rats (Rattus norvegicus) as animal subjects. The wounds were made on the back, approximately 6 mm x 2 cm. Then, the rats were divided into two groups: a control group without intradermal sutures (K) and treatment group with intradermal sutures (I). Histopathological examination using Hematoxylin and Eosin (H&E) staining was used to identify the fibroblast number on the 21st day. The fibrocytes number were also observed on the 12th week and the Vancouver Scar Scale (VSS) was used to assess the scar quality.Result : The number of fibroblasts and fibrocytes in the control group (K) was significantly smaller (53.60 ± 14.571; 3.20 ± 0.447) than the treatment (I) (243.20 ± 75.334; 171.40 ± 13.221). The VSS value was significantly greater in the control than the treatment.Conclusion: The intradermal sutures using polypropylene thread produced a better-quality scar after surgery compared to wounds without intradermal sutures.
