The making of investigating report on the factory of tobacco products (cigarette and tobacco factories) in middling KPPBC Pabean B at Surakarta was still done semi-manually with Microsoft Office. Even though an investigating report was computerized but it was still not effective. It is due to the fact that the officers had to manually record of investigation results on the forms and input the data one section at a time in Microsoft Excel. Semi-manual data processing is a problem in completing work. These problems include lag in processing data. The investigating report system on web-based in middling KPPBC Pabean B at Surakarta is used to complete the work in making the investigating report to be faster and more accurate. It is because the large number of supervision and work areas of middling KPPBC Pabean B at Surakarta. This study used the waterfall method as a system design while data collection techniques used interviews, observation, literature studies and questionnaires. Whereas, the system testing used the webqual method. This research produces a system of investigating report on web-based in middling KPPBC Pabean B at Surakarta. System testing shows that usability quality has a positive effect on user satisfaction because it has a significance value of 0.03 0.05 and interaction service quality has a positive effect on user satisfaction because it has a significance value of 0.001

The purpose of this study was to describe the internalization of Pancasila democratic values for the students dean administrators. The method used in this study is a quantitative descriptive approach. The sample in this study were 39 respondents. Data analysis was done by using Chi squares and data collection techniques are questionnaires and supporting techniques using interviews and documentation. The results of this study indicate that some of the board members of SMP Negeri 19 Bandar Lampung academic year 2018/2019 in the process of internalizing the values of Pancasila democracy can be categorized with the percentage of 25.64% in the inappropriate category, 38.46% of the categories are quite appropriate, and 35.89% is in the appropriate category. Looking at the analysis and the data shows that the number 0.44 can be categorized as moderate, indicating that the internalization of the values of Pancasila democracy has a moderate closeness to the Minister of National Education Regulation No.39 of 2008 for the students dean administrators.Tujuan penelitian ini adalah untuk mendeskripsikan tentang Internalisasi nilai demokrasi Pancasila bagi pengurus OSIS. Metode yang digunakan dalam penelitian ini adalah adalah pendekatan deskriptif kuantitatif. Sampel dalam penelitian ini sebanyak 39 responden. Analisis data menggunakan Chi kuadrat dan teknik pengumpulan data menggunakan angket dan teknik penunjang menggunakan wawancara dan dokumentasi. Hasil penelitian ini menunjukkan bahwa sebagian pengurus OSIS SMP Negeri 19 Bandar Lampung Tahun Pelajaran 2018/2019 dalam proses internalisasi nilai demokrasi Pancasila dapat dikategorikan cukup sesuai. Dengan persentase 25,64% kategori kurang sesuai, 38,46% kategori cukup sesuai, dan 35,89% kategori sesuai. Melihat dari analisis dan olah data menunjukkan pada angka 0,44 dapat dikategorikan sedang, menunjukkan bahwa internalisasi nilai demokrasi Pancasila memiliki keeratan sedang pada Permendiknas No.39 Tahun 2008 bagi pengurus OSIS.Kata kunci: Internalisasi, Demokrasi, Pancasila

Background: Surgery is one of the services carried out by the hospital. The surgical report sheet must be completely filled out and signed by the doctor who performed the operation and must be made immediately after surgery, then entered into the patient's medical record. The incompleteness of filling out the surgical report can result in unclear sequence of procedures, this can cause serious problems, especially if it goes to court. The purpose of this study was to determine the completeness of the surgery report and the factor of incompleteness of the surgery report sheet at RSU PKU Muhammadiyah Bantul. Subjects and Method: This type of research is mixed methodology, data collection using observation checklist tables, and interview sheets. The population in this study was a surgical patient visit report sheet in April 2022 with a sample of 100 operating report sheets. The research subjects were 2 surgeons and 2 nurses. Results: The completeness of the surgical report for the identification section was 88.75%, the important report section was 95.58%, the authentication section was 100%, and the documentation section was good at 98%. The factor causing the incomplete surgical report is because of the doctors and nurses are busy, causing the operation report sheet to be incomplete. Conclusion: It is recommended that there be course to doctors and nurses regarding the importance of completeness of medical records, especially operating report sheets, and collaboration between medical record officers, nurses, and doctors who treat patients. Latar Belakang: Tindakan operasi adalah salah satu pelayanan yang dilaksanakan oleh rumah sakit. Lembar laporan operasi harus terisi lengkap dan ditandatangani oleh dokter yang melakukan operasi dan harus segera dibuat setelah pembedahan, kemudian dimasukkan kedalam rekam medis milik pasien. Ketidaklengkapan pengisian lembar laporan operasi dapat mengakibatkan ketidakjelasan urutan prosedur, hal ini dapat menimbulkan permalasahan serius terutama jika sampai pengadilan. Tujuan dari penelitian ini adalah untuk mengetahui kelengkapan lembar laporan operasi dan faktor ketidaklengkapan lembar laporan operasi di RSU PKU Muhammadiyah Bantul. Subjek dan Metode: Jenis penelitian ini adalah mixed methodology, pengumpulan data menggunakan tabel checklist observasi, dan lembar wawancara. Populasi dalam penelitian ini adalah lembar laporan operasi kunjungan pasien bedah di bulan April 2022 dengan sampel 100 lembar laporan operasi. Subjek penelitian 2 dokter bedah dan 2 perawat. Hasil: Hasil penelitian secara umum menunjukkan kelengkapan lembar laporan operasi bagian identifikasi sebesar 88,75%, bagian laporan penting sebesar 95,58%, bagian autentifikasi sebesar 100%, dan bagian pendokumentasian yang baik sebesar 98%. Faktor penyebab ketidaklengkapan lembar laporan operasi yaitu karena kesibukan dokter dan perawat, sehingga menyebabkan lembar laporan operasi tidak terisi dengan lengkap. Kesimpulan: Sebaiknya diadakan sosialisasi kepada dokter dan perawat mengenai pentingnya kelengkapan rekam medis khususnya lembar laporan operasi, serta diperlukan kerjasama antara petugas rekam medis, perawat, dengan dokter yang menangani pasien.
