Abstract

IntroductionSkin hydration decreases with aging. Aquaporin-3 (AQP3) is a major protein that plays a role in skin hydration, therefore it is a novel target for skin moisturizing treatment. Retinoic acid (RA) as a well-known active agent in antiaging treatment increases AQP3 expression, but frequently causes harmful side effects. Asiaticoside, a saponin compound isolated from Centella asiatica (CA) is also known as an antiaging cosmetic and plays a role in wound healing. The aim of this study was to evaluate and compare the effect of asiaticoside isolated from CA and the effect of RA on the AQP3 expression in normal human epidermal keratinocytes (NHEKs).Methods An experimental laboratory study was performed using primary NHEKs that were derived from the foreskin of a boy. AQP3 expression in NHEKs was examined in vitro after the cells were incubated for 24 hours with asiaticoside or with RA at several concentrations. The AQP3 expression was evaluated by immunocytochemistry and quantitatively analyzed by Image-J software. Independent t-test and one-way ANOVA were used to analyze the data, followed by post-hoc Tukey test.ResultsThere was an increasing trend of AQP3 expression upon exposure to asiaticoside at all concentrations compared to the control group. However, RA exposure seemed to induce a higher level of AQP3 expression. Asiaticoside effected a lower increase in AQP3 expression in NHEKs than did RA (p=0.042). Optimal results were achieved at 1 mg/mL concentration of asiaticoside.ConclusionsAsiaticoside isolated from CA can enhance the AQP3 expression in NHEKs. Therefore it can be used as an active ingredient in cosmetic moisturizer formulation for dry skin treatment.

Abstract

Syphilis is a sexually transmitted infection caused by bacterium Treponema pallidum which can be transmitted through sexual intercourse, blood transfusion, and vertically from mother to fetus. If pregnant woman suffers from syphilis, transplacental infection can occur to the fetus, causing abortion, prematurity, low birth weight, stillbirth, or congenital syphilis. The diagnosis of syphilis in pregnancy is established based on history, clinical manifestations, laboratory and serologic examination. Screening in the first trimester with non-treponema tests such as rapid plasma reagin (RPR) or venereal disease research laboratory (VDRL) combined with a treponema test such as the treponema pallidum hemagglutination assay (TPHA) is important for every pregnant woman. Clinical manifestations of syphilis to the fetus depend by gestational age and stage of maternal syphilis also fetal immune response. Early detection and adequate management are important to prevent the transmission of syphilis infection from mother to fetus.

Abstract

Eksantema merupakan manifestasi erupsi kulit makulopapular eritem difus disebabkan virus atau bakteri. Erupsi kulit terjadi karena kerusakan sel akibat mikroorganisme, toksin, dan respon imun pejamu. Morbili, rubela, roseola, demam skarlatina, dan hand-foot and mouth disease (HFMD) adalah lima penyakit eksantema terbanyak. Kelainan ini sering sulit dibedakan satu sama lain dan dengan erupsi obat tipe makulopapular. Diagnosis berdasarkan anamnesis, gejala prodromal, gambaran erupsi kulit, dan manifestasi klinis khas. Pemeriksaan laboratorik dan serologik membantu diagnosis. Sebagian besar eksantema bersifat self-limiting sehingga terapi hanya suportif. Exanthema is maculopapular skin eruption of diffuse erythematous caused by viral or bacteria. Skin eruptions occur due to cell damage caused by microorganisms, toxin, and host immune response. Morbili, rubella, roseola, scarlet fever, and hand-foot and mouth disease (HFMD) are the five most common exanthema diseases. The disease is difficult to distinguish from each other and with drug eruption. Diagnosis is based on history, prodromal symptoms, skin eruption, and typical clinical manifestation. Laboratory and serologic examinations help establish diagnosis. Most exanthemas are self-limiting and only need supportive therapy.

Abstract

Sindrom Stevens-Johnson (SSJ) merupakan suatu kumpulan gejala klinis erupsi mukokutaneus yang ditandai trias kelainan pada kulit berupa eritema, vesikel/bula, dan dapat disertai purpura yang mengenai kulit, selaput lendir orifisium, serta mata. Sindrom ini dapat bermanifestasi mulai dari gejala ringan hingga gejala berat yang dapat mengancam nyawa. Dilaporkan satu kasus SSJ diduga akibat siprofloksasin pada laki-laki usia 48 tahun. Pada pemeriksaan fisik, didapatkan tekanan darah, denyut nadi dan frekuensi pernapasan dalam batas normal namun terdapat demam (suhu 38oC), vesikel dan bula berdinding kendur pada regio femur dan lumbal yang mengalami erosi dan ekskoriasi, krusta kehitaman pada regio labialis dan genital, serta dry eye. Tes Nikolsky positif. SCORTEN pada kasus ini 1. Luas daerah yang terlibat 6 %.Stevens-Johnson syndrome (SJS) is a group of clinical symptoms of mucocutaneous eruption, characterized by triad signs: erythema, vesicle/bullae, and can be followed with purpura which affects skin, orificial mucous membrane and eyes. This syndrome consists of various symptoms from low-risk to life-threatening. We report a case of SJS in a 48-years old male, suspected to be induced by ciprofloxacin. The blood pressure, arterial pulse, and respiratory rate were normal but there was fever (38oC); vesicle and bullae with flaccid wall were found in femoral and lumbal regions and already had been eroded and excoriated, and with dark crusts in labial and genital regions. Nikolsky’s sign was positive. SCORTEN in this case was 1. The body surface area involved was 6%.Â
