Abstract

Introduction: Traumatized immature permanent teeth can cause pulp necrosis, leading to imperfect root formation, and thus resulting an open apex. The apical opening of the immature teeth is mostly large and thus may lead to difficulties in performing a root filling. In such cases, apexification is mandatory before doing root canal treatment. The aim of this case report was to inform that apexification and direct veneer was suitable to treat Ellis class IV fracture with necrotic pulpCase Report: 21-year-old female came to the Dental Conservation Clinic RSGM Prof. Soedomo Faculty of Dentistry UGM with chief complaint of right central incisive maxilla fracture and tooth discoloration. Radiographic examination showed an open apex of tooth 11 with a diffuse apical radiolucent area. Root canal treatment and apexification were carried out using circumferential technique and Mineral Trioxide Aggregate (MTA). Vertical warm obturation was performed, followed by intra coronal bleaching in the next visit. Final restoration was carried out using direct composite veneer. Abstract

ABSTRAKPendahuluan: Pemilihan bentuk restorasi setelah perawatan saluran akar harus dilakukan dengan cermat. Restorasi endocrown dapat mengoreksi dataran oklusal yang ekstrusi, memperbaiki kontak area, membentuk kontur gigi, embrasur serta memperbaiki inklinasi gigi. Tujuan laporan kasus menyampaikan keberhasilan restorasi endocrown fiber reinforced composite sebagai alternatif restorasi gigi setelah perawatan saluran akar dalam kondisi klinis khusus yaitu ekstrusi dan luksasi yang mampu mengatasi mobilitas dan inklinasi gigi sehingga gigi dapat berfungsi dengan baik. Laporan kasus: Kasus pasien wanita berusia 43 tahun dengan gigi molar mandibula kanan nekrosis pulpa, ekstrusi dan luksasi derajat 1. Perawatan saluran akar dilakukan dengan teknik crown down, multi kunjungan dan restorasi akhir menggunakan endocrown fiber reinforced composite yang dilakukan secara indirek. Hasil perawatan menunjukkan tidak adanya keluhan, luksasi terkoreksi dan gigi dapat berfungsi dengan baik. Simpulan: Perawatan saluran akar gigi molar satu mandibula dengan restorasi endocrown fiber reinforced composite dengan kondisi klinis ekstrusi dan luksasi menunjukkan tingkat keberhasilan perawatan yang baik ditandai dengan tidak adanya keluhan, luksasi terkoreksi dan gigi dapat berfungsi dengan baik Kata kunci: endocrown; fiber reinforced composite; perawatan saluran akar; luksasi. ABSTRACTIntroduction: The choice of restoration form after root canal treatment must be made carefully. Endocrown restorations can correct extruded occlusal planes, improve contact areas, contour teeth, embrasure and improve dental inclination. This case report aims to present the success of endocrown fiber reinforced composite restoration as an alternative to dental restoration after root canal treatment in special clinical conditions, namely extrusion and luxation, which can overcome the mobility and inclination of the teeth so that the teeth can function properly. Case report: A 43-year-old female patient with a right mandibular molar with pulp necrosis, extrusion, and grade 1 luxation. Root canal treatment was performed using crown down technique, multiple visits and indirect restoration using endocrown fiber reinforced composite. The treatment results showed no complaints, the luxation was corrected, and the teeth could function properly. Conclusion: Root canal treatment of mandibular first molars with endocrown fiber reinforced composite restoration with extrusion and luxation clinical conditions showed a good treatment success rate, indicated by no complaints, corrected luxation and functioning teeth. Keywords: endocrown; fiber-reinforced composite; root canal treatment; luxation
