Abstract

Background: COPD is a pulmonary disease characterized by air obstruction in respiratory tract from bronchi to the bronchioles, which continuously occur progressively, and irreversible, and related with the inflammatory response due to toxic gases (cigarette smoke, factory pollution and vehicle pollution) so it can cause a desaturation oxygen (SpO2). This impact will caused in concentration decreased, functional activity reduction, hypoxemia, hypoxia until cyanosis. To prevent this impact, there are two non-pharmacological methods that can be applied, a tripod position and pursed lips breathing exercise. Purpose: To determine the effectiveness of tripod position and pursed lips breathing to enhance oxygen saturation in patients with COPD.Methods: The research used queasy experimental with pre-posttest design. This research used total sampling that have 34 samples COPD patient which is include in inclusion criteria in working area of Puskesmas Padangsari Semarang. The samples are divided into 2 treatment groups (tripod position groups and pursed lips breathing groups) that gives 15 minutes therapy 3 times within 3 continuous days. This research will begins from February until March 2019. Statistical test to determine the affectivity of each groups using non-parametric test (Mann-Whitney test). Results: the result of this research shows differences effectiveness between tripod position and pursed lips breathing with p value 0.00 with mean deviation before and after therapy of tripod position 0.29 and pursed lips breathing 1.23.Conclusion: in this research shows that pursed lips breathing is most effective to enhance oxygen saturation in COPD patient compare with tripod position.

Abstract

Background: Chronic Kidney Failure (CKD) is a kidney function disorder that occurs gradually, cannot be recovered, and describes a decrease in urine production which results in an accumulation of fluid in the body. This can disrupt their fluid and electrolyte balance. In this regard, in Indonesia there are still many phenomena of non-compliance with fluid monitoring that occur. If this is left unchecked, it will result in a condition of overload or excess fluid in the patient. Fluid management is needed to fulfill the patient's fluid and electrolyte needs, so that changes in body weight and the results of the calculation of fluid balance can be controlled and become an illustration of meeting these needs.Purpose: to explain nursing care to meet fluid and electrolyte needs in chronic kidney disease patients.Methods: nursing care was carried out on patients with a diagnosis of chronic kidney disease who have impaired fluid and electrolyte requirements. The nursing intervention used to overcome this problem was fluid management. Data were collected from interviews and observations and further analysis was carried out.Results: After three days nursing intervention, the results showed that the client's level of shortness of breath decreased, the blood pressure level was close to normal, fluid calculation close to balance, and the client's weight seemed stable.Conclusion: fluid management was effective for monitoring fluid intake and output of chronic kidney disease patient.

Abstract

Tirah baring dalam waktu yang lama beresiko memperbesar resiko decubitus pada penderita stroke sehingga harus dilakukan pencegahan agar tidak menyebabkan kenaikan grade pada luka dan menimbulkan komplikasi. Upaya untuk mencegah dan menjaga integritas kulit yang mengalami immobilisasi lama dengan cara melakukan massage effleurage secara berlahan yang diselingi dengan Virgin Coconut Oil (VCO). Penelitian ini bertujuan untuk mengetahui adakah pengaruh massage dengan Virgin Coconut Oil (VCO) terhadap pencegahan decubitus pada pasien stroke non hemoragik. Desain penelitian menggunakan quasi-experinmental dengan desain penelitian pretest dan post test with cintrol group. Dengan metode total sampling dan masing-masing kelompok intervensi dan kontrol berjumlah 15 responden. Variabel independent penelitian ini adalah Massage Effleurage dan Virgin Coconut Oil (VCO) Variabel dependen penelitian ini adalah Dekubitus dan Stroke Non Hemoragik. Pengukuran risiko dekubitus dengan skala braden Data diperoleh dengan cara wawancara, pemberian Massage effleurage dengan Virgin Coconut Oil (VCO) diberikan setiap 1-2 kali sehari selama 7 hari. hasil penelitian menunjukan bahwa terdapat pengaruh massage effleurage dengan Virgin Coconut Oil (VCO) terhadap pencegahan risiko dekubitus pada pasien stroke non hemoragik dengan hasil uji dependent t test pada kelompok intervensi menunjukan p volue 0,000 ( p<0,05). Serta hasil uji independent t test pada post test kelompok intervensi dan kontrol menunjukan p volue 0,000 (p< 0,05) kesimpulan pada penelitian ini adalah ada pengaruh massage effleurage dengan Virgin Coconut Oil (VCO) terhadap pencegahan risiko dekubitus pada pasien stroke non hemoragik.
