Abstract

BACKGROUND: Progression of laryngeal carcinoma can be classified with the clinical staging, however there are different patterns of progressions observed in the patient with the same clinical stage which also affects their prognoses. Therefore biomarkers should be used. Nuclear factor (NF)-ĸB, Cyclin-D1, vascular endothelial growth factor (VEGF), cyclooxygenase (Cox)-2 have been reported for laryngeal carcinoma. However, it is still unclear how these markers are expressed and correlated in advanced stage laryngeal carcinoma. Therefore current study was conducted to investigate the expressions of NF-ĸB, Cyclin-D1, VEGF and Cox-2 and their correlations in advanced stage laryngeal carcinoma.METHODS: Subjects were recruited and laryngeal biopsies were collected, fixed in formalin and prepared for immunohistochemistry. The immunohistochemistry was performed using mouse monoclonal anti-NF-kB p65, anti-Cyclin-D12 anti-VEGF, and anti-Cox-2 antibodies. The immunohistochemistry results were documented and measured using ImmunoRatio. Pearson or Spearman correlation test was used based on the results of Shapiro-Wilk test of normality. A p-value of less than 0.05 is considered statistically significant.RESULTS: Twelve male subjects were included in this study. Expressions of NF-ĸB, Cyclin-D1, VEGF dan Cox-2 were clearly observed. Mean of NF-ĸB, Cyclin-D1, VEGF dan Cox-2 IHC expression levels measured with ImmunoRatio were 57.50±20.06%, 45.00±24.31%, 43.33±17.23% and 40.42±16.98%, respectively. There was significant correlation between the expressions of VEGF dan Cox-2 (p=0.031, r=0.622).CONCLUSION: Since correlation between the VEGF and Cox-2 expressions was statistically significant, VEGF and Cox-2 might have important roles in the growth, invasion and metastasis of laryngeal carcinoma.KEYWORDS: advanced stage laryngeal carcinoma, immunohistochemistry, NF-ĸB, Cyclin-D1, VEGF, Cox-2

Abstract

BACKGROUND: Sinonasal inverted papilloma (SIP), a benign epithelial growth in the sinonasal region with epidermoid epithelial transformation, has been known for its invasiveness, recurrency, and its link with malignancy. Meanwhile sinonasal squamous cell carcinoma (SSCC) is an epithelial malignancy on squamous cells from the sinonasal region. Epidermal growth factor receptor (EGFR), Nuclear Factor kB (NF-kB), and Cyclin D1 are factors those might play important role in proliferation of SIP and SSCC. This research was conducted to investigate the expressions of EGFR, NF-kB and Cyclin D1 in SIP and SSCC.METHODS: A cross-sectional study by examining the EGFR, NF-kB, and Cyclin D1 immunohistochemical expressions of SIP and SSCC was conducted. Subjects whose blocks were used in this research, were diagnosed as SIP and SSCC at the Otorhinolaryngology-Head and Neck Surgery Clinic, Dr. Saiful Anwar General Hospital. Samples were selected, processed for inmmunohistochemistry, evaluated and statistical analyzed.RESULTS: Twenty-four SIP and 9 SSCC subjects with their paraffin blocks were selected. Clear immunohistochemical expressions of EGFR, NF-kB, and Cyclin D1 were observed for both SIP and SSCC. Significantly higher immunostaining levels of EGFR (45.6%, p=0.001) and NF-kB (42.2%, p=0.013) were observed in SSCC. Immunostaining levels of EGFR vs. NF-kB were moderately correlated (p=0.03, r=0.437), while the immunostaining levels of NF-kB vs. Cyclin D1 were strongly correlated (p=0.002, r=0.602).CONCLUSION: Expression of EGFR and NF-kB in SSCC were higher than the EGFR and NF-kB expression in SIP, suggesting that EGFR and NF-kB play important role in sinonasal malignancy.KEYWORDS: sinonasal, inverted papilloma, SCC, EGFR, NF-kB, Cyclin D1

Abstract

Background: Chronic Suppurative Otitis Media (CSOM) is a chronic infection in the middle ear mucosa with tympanic membrane perforation and causes complications in the form of intracranial and extracranial. In previous studies, CSOM mortality rates were different between intracranial and extracranial complications. That prompted the researchers to determine the middle ear postoperative prognosis in CSOM with intracranial complications compared to the extracranial. Postoperative prognosis is measured by Middle Ear Risk Index (MERI). This study aims to evaluate the comparison of MERI scores in CSOM patients with intracranial and extracranial complications. Methods: The research was conducted with an observational analytic design involving 40 research subjects from medical records of CSOM patients at RSUD dr. Saiful Anwar Malang in January 2017 – July 2021. The samples were divided into CSOM with intracranial and extracranial complications, including 12 and 28 subjects, respectively. Those samples are taken using consecutive sampling techniques and tested using an Independent T-test. Data were analyzed using SPSS version 25 for Windows.Results: The mean of MERI scores in CSOM patients with intracranial complications was 9.170±2.517, and that in extracranial complications was 7.570±1.854. The p-value of 0.031 (P<0.05) was obtained using an Independent T-test, indicating a significant difference of MERI scores mean between CSOM patients with intracranial and extracranial. Conclusion: There is a significant difference in MERI scores between CSOM with intracranial and extracranial complications. Latar Belakang: Otitis Media Supuratif Kronis (OMSK) adalah infeksi kronis yang terjadi pada mukosa telinga tengah yang ditandai dengan perforasi membran timpani dan dapat menyebabkan komplikasi intrakranial maupun ekstrakranial. Penatalaksanaan OMSK dengan komplikasi bertujuan untuk menyelesaikan komplikasi, infeksi di telinga tengah dan perbaikan fungsi. Perbaikan fungsi pendengaran bukan tujuan utama tapi tetap menjadi harapan dari pasien. Prognosis fungsi pendengaran pada operasi mastoid dapat dinilai dengan memperhatikan Middle Ear Risk Index (MERI). Penelitian ini bertujuan untuk mengevaluasi perbandingan skor MERI pada pasien OMSK dengan komplikasi intrakranial dan ekstrakranial.Metode: Penelitian dilakukan dengan desain penelitian observasional analitik yang melibatkan 40 subjek penelitian berupa rekam medis pasien OMSK di RSUD dr. Saiful Anwar Malang bulan Januari 2017 – Juli 2021. Sampel terbagi menjadi dua kategori yaitu OMSK dengan komplikasi intrakranial dan ekstrakranial dengan jumlah secara berurutan yaitu 12 dan 28 sampel. Sampel diambil menggunakan teknik konsekutif dan diuji menggunakan Independent T-Test. Data dianalisis dengan SPSS versi 25 untuk Windows.Hasil: Didapatkan rerata pasien OMSK dengan komplikasi intrakranial sebesar 9,170±2,517 dan ekstrakranial sebesar 7,570±1,854. Pada uji Independent T-Test didapatkan nilai p sebesar 0,031 (p<0,05) yang menunjukkan terdapat perbedaan rerata MERI pada pasien OMSK dengan komplikasi intrakranial dan ekstrakranial secara bermakna.Kesimpulan: Terdapat perbedaan skor MERI yang bermakna antara OMSK dengan komplikasi intrakranial dan ekstrakranial.
