Abstract

From January to April 2021, 46 out of 200 medical record files were misplaced and misfiled, according to preliminary research conducted at the Kalijaga Health Center. Misplaced files and missfiles can interfere and cause delays in the patient care process, making new medical records other than missing medical records, making the contents of medical record files unsustainable. This study is designed to describe, prioritize causes, and schedule efforts to recover from erroneous file incidents using USG (Urgent, Severity, Growth) and brainstorming at the Kalijaga Health Center. This type of study uses qualitative and data collection through interviews, research, recordings, questionnaires, and brainstorming. The result is that the priority causes of the missfile problem are corrupt folders and stacked files that are not assigned to that folder. Efforts to overcome this problem are by replacing damaged folders, duplicating medical record folders, providing medical record folders sequentially, adding storage/filing shelves so that files do not pile up, separating inactive files.

Abstract

The implementation of coding done in the medical record should be done very thoroughly, precisely and accurately according to the diagnostic code that exists in ICD-10. If there is an error in coding, it will have a bad impact on the patient, puskesmas or hospital. But in the fact found in the field there are still problems in the implementation of the accuracy of the encoding of disease diagnosis based on ICD-10. The purpose of this research is to describe the accuracy of disease code in the medical record in Puskesmas Plumbon. The population in this study was the entire quarterly medical record document of year 2020 with a total of 1,098 medical record documents with a total sample of 92 documents using simple random sampling technique. This study was conducted in Plumbon Puskesmas Medical record unit on 20 June 2020. The method used is descriptive with a quantitative approach. The data collection procedures used are by observation and checklist sheet. The population in this study was the entire quarterly medical record document of year 2020 with a total of 1,098 medical record documents with a total sample of 92 documents using simple random sampling technique. This study was conducted in Plumbon Puskesmas Medical record unit on 20 June 2020. The results of the study obtained from 92 samples of medical record documents, the appropriate disease code in accordance with the ICD-10 as much as 39 (42.39%) and improper code of 53 (57.61%). There is still an imprecision of unsuitable disease code due to coding officers that do not include the 4th character. It is best to need a fixed procedure in accordance with WHO provisions for coding the disease to make the koder more thorough in determining the disease code.

Abstract

One of the qualities in health care facilities is inpatient services in the availability of a number of beds. The use of beds for inpatient services was assessed through indicators such as BOR, LOS, TOI, and BTO. The purpose of this study was to determine the description of the quality of inpatient services based on Barber Johnson indicators at Pertamina Cirebon Hospital.The research method used is quantitative descriptive. The sample used in this study are inpatient daily census recapitulation data for each room in January - March 2019 total 8 room. The variabel this study is the quality of inpatient servicesbased on the Barber Johnson indicator which includes BOR, LOS, TOI, and BTO with a checklist instrument and analyzed descriptively.The results of this study the rooms included in the Barber Johnson graphic efficiency values in January 2019 of the Tulip and Sakura rooms, in February of Tulip, Flamboyan, and Sakura rooms and in March of VIP, Tulip, and Flamboyan rooms.
