Abstract

Patient safety is the absence of the danger posed to the patient can be prevented during the process of health care. One of the indicators of patient safety in pharmacy practice is the principle of the drug, this using the 6 principles of drug administration. Research using a mix method, this research occured in Puskesmas of Kasihan II on October 5-28, 2015. The respondents in this study were 2 pharmacists, patient and the head of the health center. Application of patient safety in drug delivery based on 6 principles correctly, showing 100% true with principles and expired, 78,5% true principles patient, 63,7% right drug, right dose of 94,1% and 25,5% correct information. Based on qualitative analysis, the application of patient safety based on 6 principles of drug administration were obtained, the officer checks the identity of the patient, adjust the dose, adjusting the recipe, provide drug-related information provided to the patient. Problems derived from the application of the 6 principles of drug delivery was 6 reciper identified were given much more than the actual dose, no places to store drugs that have expired, an imbalance between the number of patients pharmacist. Advice and hope that can be given is the addition of human resources in pharmacy and provide room for storing medicines that have expired.

Abstract

Merapi eruption in 2010 in Yogyakarta caused people losing their homes and triggering traumatic effect. Elderly is one of victims that vulnerable to psychological problem. The common psychological late respons due to disaster is insomnia. The impacts of insomnia are decrease the level of immunity, increase mortality and decrease quality of life, thus it needs to be solved. One of the solution is by listening murotal Al-Qur’an that will give relaxation effect. The study was aimed to know the effectiveness of listening murotal Al-Qur’an to the changing of elderly insomnia level in shelter Dongkelsari, Sleman, Yogyakarta.The design of study was a quasy experimental pre post test with control group. Technique of taking sample was probability sampling and it was found that there were 37 elderly who live in shelter Dongkelsari Sleman Yogyakarta that fulfilled the inclusion criteria. Research instrument which was used to measure insomnia level is KSPBJ questionnaire rating scale and to listen for murotal Al-Qur’an (Ar-Rahmaan) was used laptop and speaker. The duration of listening murotal Al-Qur’an was 8 days with 12 minutes for each day. Data analysis was used paired samples t test and independent samples t test. The level of insomnia for control group pre test and post test is in moderate insomnia by the mean ± SD each of them is 9.3 ± 3.7 and 10.3 ± 3.4 (p>0.05) and for intervention group there was a changing from moderate insomnia to mild insomnia by the mean ± SD each of them is 10.9 ± 3.6 and 8.2 ± 4.2 (p<0.05). There was a significant differences between the intervention group and control group by the mean ± SD each of them is -2.8 ± 3.0 and 0.9 ± 3.5 (p<0.05). In conclusion, listening murotal Al-Qur’an was effective to reduce elderly insomnia level in shelter Dongkelsari Sleman Yogyakarta.

Abstract

Health services provided in the hospital are likely to cause patient safety incidents which are adverse patient. The most incidents occurred in Indonesia are errors drug administration. Incidents can be prevented, one of them by training nurses on patient safety with S-BAR approach communications. The study aimed to determine the effectiveness of patient safety training; S-BAR communication in reducing injection drug administration errors in PKU Muhammadiyah Hospital in Yogyakarta Unit II. The type of research pre-experiment with the design of one group pretest-posttest design. Respondents were taken by purposive sampling technique, which total of 32 nurses who fulfilled the inclusion criteria. This study used the observation sheet instruments of 10 true principle of injection drug delivery. Analyzed by Wilcoxon test. After the training of patient safety: the SBAR communication given to nurses, there were signifi cant differences in injection drug administration errors based on the principle really patient, route, medicine, time, assessment, information and evaluation (p<0.05), but no signifi cant differences in principle correct dose, expired and documentation (p> 0.05). Conclusions and suggestions: patient safety training; SBAR communication on nurses wasc reduced injection drug administration errors effectively in PKU Muhammadiyah Hospital Unit II Yogyakarta based on the principle of right of patients, service, medicine, time, assessment, information and evaluation. Suggested Hospital to provide periodic training to all health workers, considering the creation of a patient safety culture is the responsibility of all parties in the Hospital. Thus increase the productivity of health workers as well as the creation of a patient safety culture.
