Abstract

Jaminan Kesehatan Nasional merupakan program pemerintah Indonesia yang mengamanatkan bahwa jaminan sosial bersifat wajib bagi seluruh penduduk Indonesia melalui Badan Penyelenggara Jaminan Sosial Kesehatan. Sampai November 2020, jumlah peserta JKN di Indonesia mencapai 223.470.668 jiwa atau 83% dari penduduk Indonesia. Padahal pada tahun 2019 diharapkan seluruh penduduk sudah terlindungi program JKN. Selain itu, masalah yang dihadapi BPJS Kesehatan adalah banyak peserta JKN yang menunggak pembayaran iuran terutama peserta segmen mandiri. Beberapa penelitian sebelumnya menyebutkan salah satu alasan banyak penduduk belum mendaftar menjadi peserta JKN adalah merasa tidak mampumembayar iuran. Demikian peserta JKN yang menunggak iuran karena tidak mampu membayar iuran. Sehingga tujuan penelitian ini adalah menghitung besar kemampuan dan kemauan membayar iuran JKN bagi pekerja informal di Kota Malang. Penelitian ini merupakan studi analitik kuantitatif yang dilakukan di Kota Malang. Berdasarkan perhitungan sampel rumus Slovin, sampel minimal penelitian ini adalah 100 sampel. Tetapi penelitian ini menggunakan 174 sampel yang diambil non randomized sampling, yaitu menggunakan quota sampling sampai tercapai 174 sampel. Uji yang digunakan adalah univariat untuk mendeskripsikan hasil penelitian serta uji analitik untuk membandingkan besaran ATP dan WTP iuran JKN pekerja informal di Kota Malang. Berdasarkan penelitian yang sudah dilakukan, kemampuan membayar iuran JKN bagi pekerja informal di Kota Malang rata-rata sebesar Rp. 131.661 tetapi kemauan membayarnya hanya berkisar rata-rata sebesar Rp. 35.490.

Abstract

Background: The National Health Insurance (Jaminan Kesehatan Nasional/JKN) uses a social insurance mechanism whose participation is mandatory and relies its financing from participant contributions. In JKN participation, informal workers are included in the segment of Non-Wage Earners (Pekerja Bukan Penerima Upah/PBPU) which is still a challenge in increasing their participation to achieve universal health coverage. Aims: To analyze the relationship between trust of PBPU towards participation of JKN. Methods: A Quantitative study using a cross-sectional approach. A total of 369 samples of PBPU in Malang were collected with purposive and proportional sampling. The data were collected through a questionnaire and were analyzed using chi-square test and logistic regression analysis. Results: There was a significant association between trust toward BPJS Kesehatan, FKTP, and FKRTL as health service providers for the JKN program and PBPU's participation in JKN p = 0 (<0.05). PBPU, which has low trust in BPJS Kesehatan, FKTP, and FKRTL, has a higher tendency not to participate in JKN than PBPU which has high trust (OR 2.76 95% CI 1.65 – 4.60 each; OR 5.69 95% CI 2.27 – 14.25; OR 5.29 95% CI 2.22 – 12.62). Conclusion: Trust in JKN’s health care providers has a significant relationship to PBPU's participation in the JKN program. Thus, it is highly recommended to BPJS Kesehatan to always evaluate the performance of health service providers in collaboration with BPJS Kesehatan as one of the efforts to accelerate universal health coverage.

Abstract

National Health Insurance (Jaminan Kesehatan Nasional: JKN) demonstrates government’s commitment providing health insurance to all Indonesians. Hoped that JKN can improve the health levels of its citizens, including the poor and vulnerable; i.e Premium Assistance Beneficiaries Of National Health Insurance (PBI JKN), so that the productivity of family economies increase and intergenerational cycles of poverty can be broken. Despite wider coverage of JKN, however, the poor were still disadvantaged in the healthcare market. Our preliminary study in Kediri shows that PBI beneficiaries complain about the service of healthcare they have received. This study aims to explore the healthcare experience of PBI JKN in Kediri through a qualitative case study approach using in-depth interviews with 10 informants of PBI who had previously been selected purposively. The findings support an understanding of PBI experience from the PBI’s perspective of accessibility, service procedures, and service quality of healthcare in Kediri. According to the informants’ perspective of accessibility to healthcare, all of them stated that the healthcare is accessible. From the informants’ perspective of service procedures, most of the informants stated that the flow of health services provided is quite detailed and complete, although some informants are not satisfied with the way the health service is served. The results suggest that there are several issues that must be addressed in order to enhance the quality of care for example therapeutic communication, and the PBI experience in relation to increase the quality care. In addition, policy-makers, managers and all healthcare workers must ensure that patients especially PBI beneficiaries are satisfied of the services provided regardless of their membership status.

Abstract

Child development happens at the golden age when children are at the age of 24 until 36 months. At this age, stimulation treatment will be more effective, as has been done in PAUD or Early Childhood Education. The purpose of this study was to determine the effect of developmental stimulation given by teacher to paud students of the age group 24 until 36 months. The teacher who will provide developmental stimulation to the students of the paud child, has previously been given education and skills training related to developmental stimulation for the students. The Quasy experimental research method is used by providing stimulation treatment to a group of students aged 24 until 36 totaling 30 children, and choosing 30 aged 24 until 36 months as a control group without being given developmental stimulation. The sample selected in this study was 60 students of PAUD from PAUD cluster IX, Pandanwangi Urban Village. The results showed that 68 percent of the group specifically stimulated by the teacher were in normal development, and 72 percent in the group without special stimulation were in normal development. Both of them did not show any significant difference, with a value of 0.833, which was greater than 0.05, so H1 was rejected. There are many factors that can affect a child development at the age of 24 until 36 months. Either external and internal environmental of each child play a role. Paud as the education centre and its facilities are also factors that contribute to the development of early childhood students of cluster IX in Pandanwangi Village, Blimbing District, Malang City. For this reason, the existence of Paud as a place for children from the age of 24 months to participate in learning and playing activity becomes effective as a media for stimulating the development as well. Therefore, parents and teachers in paud are suggested to provide stimulation for children according to the material guidelines in KPSP that are trusted and easy to do

Abstract

JKN applies the performance baxed capitation payment or called KBKP or Kapitasi Berbasis Komitmen Pelayanan in Primary health Care or FKTP, with 3 indicators, namely Contact Rate, Non Specialistic Referral Ratio, and Routine Visiting Prolanis Participant Ratio. The preliminary study for January until July 2019, the KBKP of Malang City is still low. The purpose of this research is to explore the application of KBKP from the point of view of policy implementation theory.The research method is a mix method with informants 8 FKTP, Malang Health Office, and BPJS Kesehatan KC Malang.The results showed that no one FKTP had received full capitation in 2019. The implementation of the socialization and evaluation policy has been running. Constraints in the availability of resources related to the operation of the p care application. The implementation of budgeting is adequate both from capitation, BOK and BLUD. The FKTP heads play a role in encouraging the fulfillment of the KBKP by motivating them to achieve targets, but there are FKTP heads who do not care much about the implementation of the KBKP. The Health Office has not implemented a reward and punishment policy in encouraging efforts to fulfill the KBKP. The fulfillment of the 2019 KBK at the FKTP in Malang City is still not optimal, competence in the FKTP is still an obstacle, and the Health Office has not implemented incentive and disincentive policies in fulfilling the KBKP indicators.

Abstract

One of the challenges faced by BPJS Kesehatan in implementing the JKN program is that there are still many JKN participants who are in arrears in paying their monthly JKN contributions. Community service activities aim to increase community compliance in paying National Health Insurance contributions, increase public knowledge about the importance of paying JKN contributions on time, and increase public understanding of the mechanisms and procedures for paying JKN contributions and paying JKN fines. The implementation method is carried out by applying pre-test, socialization, mentoring, question and answer and post-test. The target of the activity is 59 community members who are in arrears in paying JKN contributions in Tumpang District, Malang Regency. Community service activities have a positive impact on the community in Tumpang District, Malang Regency. The implementation of socialization and mentoring activities is able to increase public knowledge about the JKN program and increase public awareness to pay JKN contributions on time.
