Abstract

Banyak faktor yang menyebabkan kegagalan terapi endodontik antara lain pembersihan dan membentuk saluran akar yang tidak sempurna dan obturasi tidak hermetis sehingga menyebabkan kurangnya kemampuan untuk menghilangkan mikroorganisme yang ada. Saluran akar yang terinfeksi membutuhkan suatu medikamen untuk menunjang keberhasilan dalam perawatan saluran akar.Kalsium hidroksida merupakan salah satu bahan medikamen yang efektif karena memiliki sifat antibakteri dengan spektrum luas, pH tinggi, biokompatibilitas baik, mampu menetralkan endotoksin bakteri, memiliki sifat toksik yang paling rendah, serta menstimulasi pembentukan jaringan keras. Tujuan laporan kasus untuk menunjukan keberhasilan perawatan ulang saluran akar gigi insisivus lateralis kiri maksila dengan lesi periapikal menggunakan medikamen kalsium hidroksida- chlorhexidine. Pasien wanita umur 53 tahun, gigi insisivus lateralis kiri maksila dengan lesi periapikal.Radiografi tampak obturasi kurang hermetis dan radiolusen daerah periapikal. Perawatan ulang saluran akar,diikuti pemasangan pasak fiber frefabricated dan restorasi porselin fuse metal.Keseimpulan setelah evaluasi setelah enam bulan pasca perawatan ulang saluran akar, radiografi menunjukan radiolusen mengecil dan gigi dapat berfungsi dengan normal.ABSTRACT: Re-Treatment of Root Canal of Maxillary Left Lateral Incisor with Calcium Hydroxide-Chlorhexidine Medicament. There are many factors that cause failure of endodontic therapy. For instances, incomplete cleaning and shaping of root canal and inadequate obturation that results in difficulty to remove the microorganisms. Infected root canal requires a medicament for the success of the root canal treatment. Calcium hydroxide is one of the effective ingredients as medicament because it has broad spectrum antibacterial properties, high pH, good biocompatibility, and it is able to neutralize bacterial endotoxins, decrease tissue toxicity, and stimulate the formation of hard tissue. The purpose of this case report is to show the success of root canal treatment of the left maxillary lateral incisor with periapical lesions using calcium hydroxide-chlorhexidine medicaments. The patient was a woman aged 53, complaining about her left maxillary lateral incisor with periapical lesion. Based on the radiographic evaluation, there was less hermetic obturation and a radiolucent in the periapical. Root canal re-treatment was continued with fiber prefabricated post and porcelain fused to metal crown. After six months of evaluation and endodontic retreatment, it is found that there is a decrease of radiolucency periapical lesion, and her teeth are able to function normally.

Abstract

Perawatan saluran akar satu kali kunjungan memberikan keuntungan antara lain memperkecil resiko kontaminasi mikroorganisme dan menghemat waktu perawatan. Pasak fiber reinforced composite memiliki ikatan yang baik dengan dentin menggunakan semen resin dan inti dari resin. Penggunaan pasak bisa mengurangi risiko fraktur. Tujuan penulisan laporan kasus ini adalah untuk mengevaluasi hasil restorasi gigi 11 nekrosis pulpa pasca perawatan saluran akar disertai restorasi dengan pasak fiber reinforced composite. Pasien wanita, 22 tahun datang ke Klinik Konservasi RSGM FKG UGM untuk merawat gigi depan atas kanan yang berlubang. Berdasarkan pemeriksaan subjektif, objektif dan radiografis diperoleh diagnosis gigi 11 nekrosis pulpa. pasca perawatan saluran akar gigi Gigi direstorasi dengan resin komposit dan pasak fiber reinforced composite. Kesimpula dari hasil evaluasi klinis saat kontrol tidak ada keluhan rasa sakit dan pasien merasa puas.ABSTRACT: One Visit Treatment of Fiber Reinforced Compositerestoration in Maxillary Right First Incisivus. One visit root canal treatment is advantageous to minimize the risk of microorganism contamination. It saves time and more tolerable for the patients. Fiber reinforced composite post is fabricated, and it has been known to have a good bond with dentinal wall of root space, resin cement and composite resin core. The use of this post could decrease the risk of fracture. The purpose of this paper is to report the results of dental restoration 11 pulp necrosis after root canal treatment with resin composite restorations and post fiber reinforced composite. A 22 year-old female patient who came to Faculty of Dentistry UGM complained about her maxillary right incisor teeth which decayed and needed a treatment. Based on the subjective, objective and radiograph examinations, it was diagnosed that the pulp was necrotic. After one visit root canal treatment and based on clinical evaluation, it is concluded that the right upper incisor that was restored using fiber reinforced composite post and composite resin showed no pain, and patient was satisfied.

Abstract

Dental esthetic complex involves more than one caries with malformed shape and malposition. Dental trauma most commonly affects maxillary anterior teeth, and subsequently affects the esthetics, functions, and psychological well-being of the individual. Composite resins have become an integral part of contemporary restorative and minimally invasive dentistry. Objective: To summarize the successful use of direct composite resin restoration in the management of a 29-year old male patient. Case Report: A 29-year old male patient presenting with multiple secondary caries and crown fractures in the anterior maxillary teeth, resulting in an unaesthetic smile. Comprehensive esthetic dental treatment was carried out. The patient was highly satisfied with the results even after eleven months of follow-up. Conclusion: restorative success with composite resins, the dental treatment plan should be carefully executed with attention to factors such as dental convexities and contours, re‑establishment of function, and esthetics. Smile design should always be a multifactorial decision-making process that allows the clinician to treat patients with an individualized and interdisciplinary approach.

Abstract

LATAR BELAKANG: Proses inflamasi periapikal yang berlanjut dapat merangsang sisa epitel malassez di ligamentum periodontal. Granuloma dapat terbentuk sebagai pertahanan pertama terhadap nekrosis pulpa yang selanjutnya dapat menjadi abses periapikal. Kista radikuler dapat terjadi jika kondisi sebelumnya tidak segera ditangani. TUJUAN: Studi kasus ini bertujuan untuk mengevaluasi apikoektomi yang diikuti dengan enukleasi kista radikuler. KASUS: Seorang pria berusia 24 tahun datang dengan rujukan ke Klinik Bagian Konservasi Gigi Universitas Gadjah Mada untuk memeriksakan giginya dan melanjutkan perawatan saluran akar gigi. Keluhan utama adalah perubahan warna gigi dan seringkali muncul benjolan besar pada gusi gigi dengan riwayat trauma (kecelakaan) kurang lebih 12 tahun yang lalu yang tidak segera dilakukan untuk diperiksa dan dirawat. MANAJEMEN: Enukleasi kista yang terlihat pada foto rontgen dan CBCT dilakukan setelah perawatan saluran akar selesai. Kapsul dan kuretase jaringan granulasi kista dikirim ke bagian patologi anatomi. Apikoektomi dilakukan sekitar 3 mm dari apeks dilanjutkan dengan preparasi kavitas, pengisian retrograde menggunakan mineral trioxide aggregate dan aplikasi bone graft. Evaluasi dilakukan satu minggu pasca operasi dan dilanjutkan pembuatan restorasi akhir berupa pasak dowel dan mahkota zirkonia. KESIMPULAN: Apikoektomi diikuti dengan enukleasi kista radikuler yang dilakukan setelah perawatan saluran akar menunjukkan keberhasilan dalam proses penyembuhan yang ditandai dengan pengurangan lesi periapikal pada foto rontgen.

Abstract

Latar Belakang: Proses inflamasi periapikal terus berlangsung dan akan menstilmulasi sisa epitel malassez yang ada di ligamen periodontal. Sebagai pertahanan pertama, pulpa akan membentuk granuloma, dimana granuloma akan menjadi kista radikuler ketika gigi tidak segera dirawat. Tujuan dari laporan kasus ini adalah untuk mengevaluasi perawatan lesi periapikal gigi dalam bentuk kista. Kasus: Seorang wanita usia 25 tahun datang ke klinik konservasi gigi Universitas Gadjah Mada dengan keluhan utama gigi depan bawah terkadang terasa sakit. Gigi tersebut telah dilakukan perawatan saluran akar. panatalaksanaan kasus ini dengan pendekatan non bedah, dan dilanjutkan pendekatan non bedah setelah dievaluasi. Penatalaksanaan: Bedah periradikuler dilakukan setelah retreatment dari gigi tidak menunjukan hasil penyembuhan di jaringan periodontal. Lesi periapikal kemudian dievaluasi dengan Cone Beam Computed Tomography (CBCT) kemudian dilakukan apikoektomi dan kuretase. Sebanyak 3mm dari apeks direseksi dan dilakukan dibawah anastesi lokal. Jaringan patoloigis kemudian dikirim ke departemen patologi setelah dikuretase. Bahan bonegraft dan apical seal dengan MTA diberikan dan diikuti penutupan flap. Evaluasi postoperatif dilakukan setelah satu minggu, tiga minggu, dan tujuh minggu. Gambaran radiograf menunjukan lesi periapikal mengecil dan pasien tidak ada keluhan. Kesimpulan: Bedah periradikuler dilakukan setelah retreatment endodotik konvensional tidak berhasil.

Abstract

Preparation of the root canal system is a crucial step in root canal treatment. Endodontically treated teeth differ structurally from healthy, untreated teeth. This can lead to root cracking by creating pressure on the canal wall, reducing the fracture resistance of the tooth. Different designs, including cross-sectional shape, tip, taper, flute, radial land, helix angle, rake angle, and pitch, can influence the outcome of the root canal preparation and the risk of root fracture. The purpose of this study was to evaluate the fracture resistance of teeth after root canal preparation using three different NiTi rotary files: ProTaper Next (PN), ProTaper Gold (PG), and HyFlex CM (HC). Thirty premolars with a single and straight root canal were decoronated at the cementoenamel junction, leaving 14 mm of the root. The subjects were divided into 3 groups. Group 1 (n = 10) was prepared using PN, group 2 (n = 10) was prepared using PG, and group 3 (n = 10) was prepared using HC. After instrumentation and irrigation, the specimens were subjected to a continuous vertical compressive force (crosshead-speed of 2.28 mm/sec) in order to record the force (in newtons) until root fracture. To describe the surface characteristic of the dentinal root after the preparation, a section fragment from the apical third of the specimens was observed using a scanning electron microscope (SEM) at 2500x magnification. The micrographs were analyzed according to the Hulsman’s method. The results demonstrated that there was a statistically significant difference in root fracture resistance among the three groups. (p = 0.043). The LSD post hoc test indicated that HC showed a higher root fracture resistance (p < 0.05) compared to both PN and PG. In conclusion, the different file systems of PN, PG, and HC instruments generate different root fracture resistance of teeth. In comparison to PN and PG, HC instruments tend to result in increased fracture resistance.
