Abstract

Background: Glaucoma is a group of optic neuropathies characterized by progressive degeneration of ganglion cells in the retina irreversibly causing cupping of the optic disc and causing vision loss. Considering glaucoma as the second cause of blindness after cataracts in the world and its irreversible nature, so this study was conducted to find out the characteristics of glaucoma, especially primary glaucoma at Bali Mandara Eye Hospital period January 1, 2018- December 31, 2018. Methods: This research is a retrospective descriptive study using medical record data. The sampling technique of this research is total sampling. The data obtained according to the inclusion and exclusion criteria were 41 data which were then analyzed using SPSS version 25. Results: This study showed that 38.1% of patients were in the 51-60 years age group. Most of the patients were male (54.8%). The majority live in the Denpasar area (21.4%). POAG was the most common type of primary glaucoma (78.6%). As many as 54.8% of patients experienced an increase in IOP. The majority of patients experienced thinning of the RNFL both in the right eye (61.0%) and the left eye (58.5%). The patient's CDR in the left (35.1%) and right (48.8%) eyes were in a severe state (?0.8). A total of 18 patients (43.9%) suggested to perform central 10-2 treshold test both in right and left eyes. Conclusion: most of the characteristics of primary glaucoma at Bali Mandara Eye Hospital were between 51-60 years old, male, residing in Denpasar, POAG type, and came in advanced clinical appearance. Abstract

Granuloma piogenik konjungtiva merupakan tumor vaskular jinak pasca episode peradangan seperti pasca bedah eksisi pterigium. Kondisi ini diduga akibat penyembuhan luka yang tidak sempurna. Kasus : Laki-laki, 44 tahun mengeluh rasa mengganjal pada mata kiri disertai merah sejak 1 minggu. Pasien memiliki riwayat eksisi pterigium 1 bulan sebelumnya. Pada segmen anterior ditemukan massa granuloma pada konjungtiva bulbi dekat limbus bagian temporal. Terapi awal kortikosteroid topikal dan bedah eksisi sebagai pilihan terapi lanjut.Conjunctival pyogenic granuloma is a benign vascular tumor after inflammation, such as post pterygium excision. This condition is thought due to incomplete wound healing. Case: Male, 44 year-old feels a lump in his left eye accompanied by redness since 1 week. He had a history of pterygium excision 1 month before. A granulomatous mass was found on the conjunctiva bulbi near the temporal limbus. Therapy consist of topical corticosteroids as initial therapy and surgical excision as further treatment option.

Abstract

Background: Orbital cellulitis is an active infection of the orbital soft tissue posterior to the orbital septum, which can cause Tolosa-Hunt Syndrome (THS) complication. Purpose: We reported a child with orbital cellulitis with THS complication caused by methicillin-resistant Staphylococcus aureus (MRSA). Case: A two-year-old girl complained of swelling and pain in the left eye accompanied by fever. In nasal region and left eye showed multiple erythema patches with geographic shape, size 1x2cm - 4x5cm, unclear border, erosion with sizes 0.5x1cm - 1x1.5 cm size covered with blackish crusts. After several days of hospitalization, she complained of proptosis and pain in moving her eye. We did the magnetic resonance imaging (MRI) scan examination, which showed a mass size of 2.2 x1.1 x 0.9 cm in a left intraconal orbital and dilation of the left-sided cavernous sinus. The blood culture showed MRSA bacteria. She was diagnosed with orbital cellulitis with THS complication and showed a good response with Meropenem, Methylprednisolone, cendo lyteers eye drops, levofloxacin eye drops, and gentamicin eye ointment. Discussion: Orbital cellulitis presents as ill-defined erythema, edema, warmth, and pain around the nasal and the orbital region and is more often found in children. The THS complication is characterized by ophthalmoplegia, unilateral orbital or periorbital pain, unilateral headache, and leukocytosis. Orbital cellulitis management includes antibiotic intravenous, corticosteroid, and eye care. Conclusion: THS is a rare complication of orbital cellulitis. Early diagnosis and management of orbital cellulitis are essential to prevent THS complication.

Abstract

Introduction: Scleritis is an inflammation of the sclera’s lining, characterized by cellular infiltration, collagen damage, and vascular changes. The incidence rate of scleritis accompanied by systemic disease generally is around 39% to 50%. Scleritis can be divided into two, namely anterior and posterior scleritis. Anterior scleritis can be divided into four, diffuse anterior scleritis, nodular anterior scleritis, necrotizing anterior scleritis with inflammation and without inflammation. One of the causes of nodular anterior scleritis is systemic sclerosis which is an autoimmune disease. Nodular anterior scleritis in systemic sclerosis is very rare, with an estimated prevalence rate of 4.4%.Case Description: A man, 46 years old, complaining of red, watery, painful, and visible white membrane on his right eye since two weeks ago and was referred from an ophthalmologist with scleritis on his right eye. The patient had a history of asthma and cataract surgery. Physical examination showed right eye VA was 6/45 pinhole (PH) 6/18, conjunctival and scleral injection, nodule on superior conjunctiva with yellowish-white in color. Laboratory examination showed a clinical representation of autoimmune disease, consulted to the rheumatology division, and was diagnosed with systemic sclerosis. The patient has been prescribed corticosteroids and immunosuppressant’s for the therapy, the patient's eye condition improves after.Conclusion: Systemic sclerosis with a presenting ophthalmologic complaint is a rare disease. This case illustrates that nodular anterior scleritis needs comprehensive intervention to seek the etiology and provide the treatment. This case has shown that nodular anterior scleritis with systemic sclerosis improved with corticosteroids and immunosuppressants, a typical treatment for autoimmune disease.

Abstract

Background: Chorioretinitis is uveitis that causes choroidal and retinal inflammation. Chorioretinitis in general due to cytomegalovirus (CMV) infection has been recognized as a major cause of congenital virus infections. A common complication in patients with uveitis is retinal detachment. In addition, high myopia can also cause retinal detachment.Case Illustration: We reported a 33-year-old male patient who came with chief complaints of a sudden blurred left eye the couple of weeks before admission. The patient had a history of high myopia -5D in both eyes and was HIV positive. One week before the complaint, patient underwent laser therapy because of retinal detachment and was also given Neomicin and Potassium Iodide eye drops. Patients presented with vision 1/60. In funduscopy examination, we found vasculitis, retinal detachment, bleeding and exudate. The patient was then diagnosed with OS Rhegmatogenous Retinal Detachment (RRD) caused by CMV chorioretinitis and high myopia ODS. The patient was treated with valganciclovir 1x900mg, methylprednisolone 2x32mg, prednisone eye drops 6x1 OD and underwent pars plana vitrectomy (VPP). The patient had complicated cataracts, therefore phacoemulsification, evacuation and reinjection of silicon oil and endolaser were performed. Unfortunately, the results obtained are not optimal where the patient's vision becomes the patient's no light perception (NLP).Discussion: Retinal detachment is a frequent complication in patients with CMV chorioretinitis. The main therapy is HAART, anti-CMV and operative management. This therapy regimen is proven to improve overall patients’ prognosis.Conclusion: Early diagnosis and appropriate treatment have a very important role in determining the prognosis of patients with CMV chorioretinitis.

Abstract

Background: Ocular leptospirosis is a rare condition. Transmission of leptospirosis is caused by the bacterium leptospira interrogans transmitted directly or indirectly from animals to humans (zoonosis). Ocular manifestations often caused by leptospirosis are conjunctivitis, keratitis, uveitis (anterior, intermediate, posterior, and panuveitis), complicated cataracts, choroiditis, papillitis, and macular edema. Uveitis is a latent manifestation of leptospirosis. Proper management can lead to a good prognosis of visual.Case Presentation: Male patient, 55 years old, with complaints of both red and blurry eyes since 5 days. Blurring comes slowly with pain, glare, and seeing shadows like spider webs. The patient's previous medical history was hospitalized because of Weil's disease. Examination of the anterior segment of both eyes revealed eyelid spasm, conjunctival injection, corneal infiltrate, keratic precipitate, and posterior synechiae on both eyes. While in the posterior segment found cells 4+ in the vitreous. The patient was treated with antibiotics and topical steroids, and visual acuity was improved.Conclusion: Leptospirosis uveitis can have various presentations during the acute and chronic phases of the disease. Even when ocular involvement is extensive and severe, most patients have a good visual prognosis with appropriate therapy. Timely diagnosis is essential to maximize the potential for visual acuity improvement and appropriate systemic monitoring and treatment of extraocular involvement in this potentially fatal condition. Good sanitation in the community plays an important role in preventing exposure to leptospirosis.

Abstract

Dry Eye Syndrome (DES) is a multifactorial disease of the tears and ocular surface that results in symptoms of discomfort, visual disturbance, and tear film instability with potential damage to the ocular surface. This research purposes are to identify the prevalence and the characteristics of DES in medical student, Program Studi Sarjana Kedokteran dan Profesi Dokter (PSSKPD), Fakultas Kedokteran, Universitas Udayana (FK Unud). This research is a descriptive cross-sectional. Ocular Surface Disease Index (OSDI) questionnaire is used to identify the prevalence and risk factors questionnaire such as gender, BMI, history of allergy, frequent use of over the counter (OTC) eye drops, frequent use of contact lens, and sleep quality, is used to identify the characteristics. This research conducted from March until September 2020. This research use total sampling method managed to collect 199 sample. The results of the study show the prevalence of DES in Preclinical Student of PSSKPD FK Unud is 106 students (52.7%) with 21% on mild severity, 15% on moderate severity, and 18% on severe severity. The characteristics of DES based on gender is 54% for male and 52% for female; on BMI are 40% for <18 kg/m2, 55% for 18-25 kg/m2, 47% for >25 kg/m2; on history of allergy is 47%; on history of OTC drug is 70%; on history of contact lens utilization is 64%; based abnormal quality of sleep is 64%.

Abstract

Gadgets with Video Display Terminals (VDT) are being used daily by students all over the world. One of the effects that may arise from VDT gadgets usage for a long duration of time is Dry Eye Syndrome (DES). Dry Eye Syndrome itself can cause discomfort and may hinder productivity. OSDI is one of the few diagnosis methods for DES which uses several sets of questions in the form of questionnaire (discomfort symptoms, visual acuity, and environmental factors). The purpose of this study is to observe DES prevalence and VDT gadgets usage behaviour among students of Udayana University, Faculty of Medicine. This study is a cross-sectional descriptive study. Research data is obtained from OSDI questionnaire that was made using online form and filled through the period of July to September 2020. The data was then processed using computer software. Sample amount that is being included in this study is 228 people. The prevalence of DES among Udayana University Faculty of Medicine students is 62,3% (142 people). DES prevalence by gender is 61,6% in male students and 62,7% in female students. Usage behaviour of VDT gadgets varies by duration per day; below two hours of usage (2 sample), two to four hours of usage (21 sample), four to six hours of usage (93 sample), and above six hours of usage (112 sample) with above six hours having the average of 9,9±2,4 hours. The most common gadget that is being used among the sample is smartphone (70,6%), and the main purpose of gadget usage is for doing study assignments (36,8%). The results of this study are useful for increasing information about prevalence data of DES among students. Further research is needed to conclude a more in-depth relationship between VDT gadgets usage and DES occurrence. Keywords: Dry Eye Synrome, Video Display Terminal, OSDI.

Abstract

ABSTRAK Lens induced glaucoma (LIG) merupakan salah satu jenis glaukoma sekunder yang diakibatkan oleh kelainan pada lensa mata. Menurut data, kasus LIG menjadi kasus glaukoma sekunder terbanyak yang ada di rumah sakit. Namun hingga saat ini masih jarang ditemukan penelitian epidemiologi mengenai LIG di Indonesia maupun di Bali. Penelitian ini bertujuan untuk mengetahui karakterisik LIG berdasarkan usia, jenis kelamin, pekerjaan, tipe LIG, status mata, manifestasi klinis, tekanan intraokuler (TIO), tajam pengelihatan, dan terapi. Penelitian ini dilakukan di RSUP Sanglah tahun 2019-2020 dengan metode penelitian cross sectional dengan metode pengambilan sampel consecutive sampling. Berdasarkan hasil penelitian didapatkan 23 pasien LIG di RSUP Sanglah tahun 2019-2020 yang memenuhi kriteria inklusi. Hasil penelitian menunjukkan pasien usia >59 tahun dengan presentase terbanyak 47,8%. 73,9% berjenis kelamin laki-laki dan 34,8% bekerja sebagai petani. Tipe LIG fakomorfik (56,5%) menjadi tipe yang paling sering dijumpai yang didominasi status fellow eye katarak imatur (43,5%). Seluruh pasien (100%) datang dengan manifestasi pandangan buram. TIO sebelum terapi terbanyak pada rentang >39 mmHg (52,5%) dan pasca terapi pada rentang <21 mmHg (65,2%). Didapatkan hasil tajam pengelihatan sebelum terapi dan sesudah terapi terbanyak yaitu hand movement masing-masing 30,4% dan 26,1%. Terapi yang paling sering diberikan adalah fakoemulsifikasi dengan implan intraocular lens sebanyak 60,9%. Dapat disimpulkan pentingnya pemberian terapi yang tepat di awal perjalanan penyakit agar bisa mendapatkan prognosis yang baik. Kata kunci : lens induced glaucoma, karakteristik, tekanan intraokular
