Abstract

Nursing documentation is the primary source of clinical information for meeting legal and professional requirements. The problem that still often arises in the implementation of documentation is that it is not in accordance with the Standardized Nursing Language (SNL) standard of nursing language, especially uniformity in using the language of nursing diagnoses and interventions. The community service program aims to increase nurses' knowledge about how to formulate nursing diagnoses using the SDKI, how to formulate outcome criteria using the SLKI and how to formulate nursing activities based on the SIKI. The method of service implementation is screening nurses' knowledge about nursing documentation using 3S, counseling and discussions about nursing documentation using 3S and its application. The target of the activity is 35 nurses. Most of the service participants are 20-30 years old (54%), female (69%), have D3 Nursing education (91%) and have worked for more than 3 years (54%). Regarding knowledge about nursing documentation using 3S, before health education the level of knowledge was mostly in the poor category (68%) and after health education the level of knowledge was in the good category (60%). .

Abstract

Introduction: Diarrhea is a condition in which there is a disturbance in the human digestive tract, characterized by the consistency of watery or liquid stools or feces as well as the frequency of defecation more than three times a day. Many cases of diarrhea can heal on their own and are not dangerous, but diarrhea can disturb the sufferer and can cause complications of certain diseases. Therefore, diarrhea must be treated as soon as possible. Objective: The purpose of this service was to increase knowledge about diarrhea in adolescents. The purpose of this study is to find out the causes of diarrhea, prevent diarrhea, and know the six steps of good and correct hand washing. Method: This public service was conducted by conducting direct counseling on Saturday, July 15, 2023, from 20:00 to 21:30. The target of this counseling was the students of Al-Jamil Islamic Boarding School, with a total of 24 participants. Then, by using lecture, demonstration, redemonstration, and question and answer methods with the media used, namely PowerPoint and leaflets, about prevention of diarrhea and knowing the 6 steps of good and correct hand washing, Result: From this survey, the frequency of diarrhea that occurred in the students of Al-Jamil Islamic Boarding School in the last month was categorized by age. There were 4 students aged 13 years, 3 students aged 14 years, 3 students aged 16 years, and 1 student aged 17 years who experienced diarrhea in the last month. Conclusion: This counseling activity on diarrhea in adolescents runs well and smoothly. There is an increase in adolescent knowledge about diarrhea.

Abstract

Introduction: Upper Respiratory Tract Infection (ARI) is an acute respiratory infection caused by pathogens that are transmitted from human to human. Signs and symptoms usually appear quickly within a few hours to a few days. ARI is a very contagious disease and can occur due to decreased body resistance, for example due to fatigue or stress. Upper Respiratory Tract Infection (ARI) is one of the diseases that has the highest incidence rate and is the main cause of death and morbidity in infants and toddlers in Indonesia. Providing education to mothers of babies and toddlers regarding Upper Respiratory Tract Infections (ARI) is important. Objective: To provide counseling and education at toddler posyandu. Method: The strategy used is to carry out health education, where this strategy will be aimed directly at the target, namely mothers of babies and toddlers at Posyandu Balita Margosari RW. 05 Ledug Village. The implementation strategy that has been carried out is by providing health counseling on URTI, with lecture, discussion, and roleplay methods. Result: implementation of health education on Upper Respiratory Tract Infections at the Margosari Toddler Posyandu. Before being given counseling, many participants still did not know or hear about ISPA. After health education, participants already know about the meaning of ISPA infection, pathophysiology of ISPA, risk factors for ISPA, symptoms of ISPA, management of ISPA at home, prevention of ISPA, chest physiotherapy and massage for babies. Conclusion: The counseling material provided is about URTI which includes understanding, pathophysiology, signs and symptoms, risk factors for occurrence, management, and prevention, as well as chest physiotherapy in infants and toddlers. Counseling is carried out using leaflet media, ppt, and baby phantoms
