Abstract

Standar Kompetensi Kerja Nasional Indonesia (SKKNI) adalah rumusan kemampuan kerja yang mencakup aspek pengetahuan, keterampilan dan keahlian serta sikap kerja yang relevan dengan pelaksanaan tugas dan syarat jabatan yang ditetapkan. Sebagai upaya mematuhi pedoman dan standar yang telah ditetapkan maka dilakukan penelitian ini untuk mengkaji kesesuaian kompetensi radiografer pada tindakan intervensi Coronary Angiography (CAG). Penelitian ini menggunakan metode penelitian kualitatif dengan melakukan observasi, wawancara dan dokumentasi terhadap peran radiografer. Didapatkan dari 25 kompetensi radiografer pada SKKNI terdapat 22 kompetensi yang sesuai dan 3 kompetensi yang tidak sesuai, kompetensi yang tidak sesuai yaitu radiografer tidak mempersiapkan pesawat sinar-x angiografi intervensi, tidak mempersiapkan troly emergensi beserta perangkatnya (obat-obatan dan lain-lain) serta tidak melakukan edukasi terhadap pasien. Kompetensi radiografer di dalam SKKNI meliputi prosedur sebelum pemeriksaan, persiapan peralatan dan perlengkapan, persiapan pasien, prosedur penatalaksanaan pemeriksaan dan prosedur setelah pemeriksaan.

Abstract

Risiko radiasi pada radiologi intervensi relatif lebih besar dibandingkan dengan radiologi diagnostik. Dengan potensi risiko radiasi yang besar pada radiologi intervensi, maka diperlukan sistem proteksi radiasi yang tepat bagi pekerja dan pasien di fasilitas radiologi intervensi. Tujuan penelitian ini adalah untuk evaluasi proteksi radiasi di ruang radiologi intervensi instalasi RIR RSUP Prof. Dr. I.G.N.G Ngoerah ditinjau dari Peraturan Menteri Kesehatan Republik Indonesia Nomor 24 Tahun 2020 dan Peraturan Bapeten Nomor 4 Tahun 2020. Jenis penelitian ini adalah deskriptif kualitatif dengan pendekatan observasional. Hasil penelitian menunjukkan bahwa pelaksanaan proteksi radiasi di ruang radiologi intervensi pada instalasi RIR RSUP Prof. Dr. IGNG Ngoerah belum sepenuhnya sesuai dengan Peraturan Menteri Kesehatan Republik Indonesia Nomor 24 Tahun 2020 dan Peraturan Bapeten Nomor 4 tahun 2020.

Abstract

Right heart catheterization is an invasive hemodynamic procedure that can be used to directly measure right heart pressure and calculate cardiac output. RHC is also considered to be the gold standard in the measurement of pulmonary artery pressure, in diagnosing increased heart filling pressure. Blood sampling in the intracardiac space measuring saturation among others: SVC (superior vena cava), MPA (main pulmonary artery), High and Low IVC (inferior vena cava), High and Low SVC (superior vena cava), High-Mid-Low RA (right atrium), Mid and Outflow RV (right ventricle), Right and Left PA (pulmonary artery), LV (left ventricle), and Distal Aorta. And additional samples are LA (left atrium) and PV (pulmonary vein). The measured saturation will be calculated using the formula by considering the value of flow ratio, PAR, mix vein, PVR, PARi, PVRi, Qs, Qp. The competency elements in SKKNI are very important to be applied so that radiographers can meet the standards of knowledge, skills and attitudes required by radiographers in carrying out their roles, functions and authorities when carrying out activities in the intervention room. Abstract

Percutaneous coronary interventional (PCI) is a minimally invasive blood vessel widening procedure using balloons and stents, the PCI procedure in CTO cases uses the double puncture technique and the C-arm tube angle position LAO 45 ? - Cranial 15 ? and Frontal 0 ? - Cranial 30 ?. The aim of using the double puncture technique is to see the entry point and exit point of chronic total occlusion. This study aims to find out the latest literature related to PCI procedures in CTO cases, including current developments and status. This research is a literature review (Summary Method) where literature exploration is carried out using various sources with the keyword "Percutaneous Coronary Interventional (PCI) Chronic Total Occlusion (CTO)". Reference sources used in preparing this article include books, as well as articles in scientific journals in English and Indonesian. Based on the results of a review of several literature procedures, PCI procedures with CTO cases consist of patient preparation, preparation of tools and materials, and examination techniques for selecting puncture access. radial artery and femoral artery, patient position, C-arm angle position. There is a role for radiographers that conforms to SKKNI radiographer Number 237 of 2020, and the role of radiographers in the actions taking place in the cathlab is to prepare the readiness of the C-arm aircraft and pay attention to radiation protection during the action.

Preparation tool involve table catheterization , fluoroscopy , catheter , substance contrast , equipment resuscitation , medicine pre-procedure , and hemostasis equipment . The medical team involves an interventional cardiologist, radiological technologist, catheterization nurse, catheterization technician, and anesthesia and resuscitation team. The CAG procedure involves patient preparation, vascular access, catheter insertion, coronary artery evaluation, and completion of the procedure. Conclusion : Congestive heart failure (CHF) is caused by the heart's inability to pump blood, especially coronary artery disease. Coronary angiography uses X-rays and a contrast agent to visualize the coronary arteries. Although symptoms of CHF include shortness of breath and swelling, mortality rates remain high, especially over 40 years of age. The role of Coronary angiography in assessment and therapy planning involves medical specialists. The results support effective treatment planning to improve patient prognosis by addressing the causes of CHF.

Abstract

Penyakit jantung koroner (PJK) adalah penyakit yang disebabkan oleh plak yang menumpuk di arteri koroner yang memasok oksigen ke otot-otot jantung. Tujuan dari penelitian ini untuk mengevaluasi dodsis radiasi pasien pada pemeriksaan Coronary Angiography dan Percutaneous Coronary Intervention di Rumah Sakit TK II Pelamonia Makassar tahun 2021. Penelitian ini menggunakan metode penelitian kuantitatif deskriptif dengan pendekatan survey untuk mengevaluasi nilai DAP dan Fluoro Time menggunakan jenis data retrospektif pada pemeriksaan Coronary Angiography (CAG) dan pemeriksaan Coronary Angiography (CAG) + Percutaneous Coronary Intervention (PCI) selama periode tahun 2021 di Rumah Sakit TK II Pelamonia Makassar. Hasil yang didapatkan dari perhitungan menggunakan SPSS Percentile 75 pada pemeriksaan Coronary Angiography (CAG) sebanyak 18 pasien dengan rata-rata dosis DAP sebesar 26.48 Gy.cm2 dan Fluoro Time sebesar 362.50 Second dan Percentile 75 dosis DAP sebesar 28.83 Gy.cm2 dan Fluoro Time sebesar 496.25 Second. Kemudian pada pemeriksaan Coronary Angiography (CAG) + Percutaneous Coronary Intervention (PCI) sebanyak 11 pasien dengan rata-rata dosis DAP sebesar 45.96 Gy.cm2 dan Fluoro Time sebesar 777.18 Second dan Percentile 75 dosis DAP sebesar 58.22 Gy.cm2 dan Fluoro Time sebesar 1065.00 Second.

Abstract

Background: Cerebral venous thrombosis (CVT) is a form of stroke when blood clots develop in the (venous sinus) of the brain. This condition causes the blood that flows into the brain to be unable to flow out. This can cause reduced blood flow to the brain and damage or kill brain cells if not treated properly. The research aims to determine the digital subtraction angiography (DSA) examination procedure of cerebral veins with trans-radial access in cases of cerebral venous thrombosis (CVT). Method: This type of research is qualitative research with a case study approach. Data was collected on July 20 2023.The research subjects were doctors, interventional radiology doctors, and scrub nurses. By conducting observations, interviews and documentation of the implementation team involved. Results: From the results of observations, documentation and interviews with the implementing team, results were obtained regarding the Cerebral Vein Digital Subtraction Angiography (DSA) examination procedure with transradial access in cases of Cerebral Venous Thrombosis (CVT), including several sub-topics. The Cerebral Digital Subtraction Angiography (DSA) examination procedure with Transradial access is the right action for patients regarding immobility and shorter hospital stays. Conclusion: The Cerebral Vein Digital Subtraction Angiography (DSA) examination procedure with transradial access in cases of Cerebral Venous Thrombosis (CVT) includes patient preparation, tools and materials, examination techniques and post-action care. The Cerebral Vein Digital Subtraction Angiography (DSA) examination procedure with Transradial access is the right action for patients regarding patient immobility and shorter duration of hospital stay. The implementation team consists of a radiology specialist, a scrub nurse and no radiographers involved in the DSA examination room.
