Abstract

Health Human Resources in health services is a form of efforts to plan, train, and develop health workers to support the achievement of the highest degree of health. The availability of quality health human resources, distributed fairly and evenly can ensure the implementation of health development. Puskesmas became the first health service facility for the community with promotive and preventive efforts. The lack of manpower and the uneven distribution of health workers in puskesmas will affect the degree of public health. This study aims to see how the placement of duties and work functions of health workers in UPTD Puskesmas Binjai Kota based on aspects of procurement, development, and maintenance. The type of research used is qualitative research with a descriptive approach and literature studies, namely observation, in-depth interviews and documentation. The results of the study obtained that the UPTD Puskesmas Binjai Kota has an excessive number of health workers with task placements that are not in accordance with their expertise and the low capacity of their human resources for computer operations. Therefore, proper planning and self-awareness of each individual are needed so that services at puskesmas can run optimally.

Abstract

Currently, there are still many maternal and child health problems, especially for vulnerable groups, such as groups of pregnant women, breastfeeding, postpartum women and newborns. This is a priority for the government, where the progress of a country is also calculated from the health of mothers and children. A healthy mother will give birth to the next generation of a healthy and productive nation. This study aims to analyze the quality of health services through the role of cadres on maternal and child health at Posyandu Mawar, Tuntungan I Village, Medan, North Sumatra. The method used in this study is descriptive qualitative, which describes the data that has been obtained and uses interview techniques and compares with theories in previous studies obtained from the Google Scholar database. In this study, the results showed that there were 3 roles of cadres in improving maternal and child health, namely socialization, counseling and assistance. The role of cadres in the village has been running smoothly, but researchers found obstacles experienced by cadres in improving the welfare and health of mothers and children. This is due to the lack of community participation in posyandu activities in the village. Thus, more in-depth socialization or counseling is needed so that people's interest can increase and increase their knowledge of maternal and child health.

Abstract

Jaminan Kesehatan Nasional (JKN) diperkenalkan pada tanggal 1 Januari 2014 sebagai upaya untuk menjamin akses kesehatan bagi seluruh masyarakat. Namun, sejak implementasinya, JKN menghadapi tantangan dalam mencapai keuangan yang seimbang. Penelitian ini bertujuan untuk mengevaluasi kinerja JKN dan menganalisis faktor-faktor yang berkontribusi pada defisit keuangan, dengan fokus pada Kota Medan. Data klaim JKN dari tahun 2014 hingga 2019 dianalisis untuk menghitung rasio klaim, yang merupakan pembagian antara biaya iuran dengan biaya klaim. Hasil analisis menunjukkan bahwa rasio klaim JKN secara konsisten berada di atas 100% selama periode tersebut, mengindikasikan adanya defisit keuangan. Pada tahun 2016, Menteri Hukum dan Hak Asasi Manusia mencatat rasio klaim sebesar 101%, menandakan bahwa pendapatan dari iuran masih perlu ditingkatkan untuk membiayai layanan kesehatan. Selanjutnya, dalam konteks Kota Medan, BPJS Kesehatan menghadapi tunggakan sebesar 395 miliar rupiah di beberapa rumah sakit hingga Agustus 2019. Salah satu faktor yang berkontribusi pada defisit di Kota Medan adalah rendahnya tingkat kepesertaan, yang hanya mencapai 80%. Kepesertaan yang rendah menyebabkan pendapatan iuran tidak mencukupi untuk mengcover biaya layanan kesehatan yang diajukan oleh peserta. Penelitian ini menggaris bawahi pentingnya memperhatikan defisit keuangan dalam implementasi JKN. Diperlukan upaya yang komprehensif, termasuk peningkatan pendapatan melalui iuran dan peningkatan kepesertaan, untuk memastikan keberlanjutan program ini. Temuan ini dapat memberikan masukan berharga bagi pemerintah dan BPJS Kesehatan dalam mengoptimalkan kinerja JKN dan mengatasi tantangan keuangan yang dihadapi.
