Abstract

Acute-onset dysphagia after stroke is frequently associated with an increased risk of aspiration pneumonia. Because most skrining tools are complex and biased toward fluid swallowing, we developed a simple, stepwise bedside screen that allows a graded rating with separate evaluations for nonfluid and fluid nutrition starting with nonfluid textures. The Gugging Swallowing Screen (GUSS) aims at reducing the risk of aspiration during the test to a minimum; it assesses the severity of aspiration risk and recommends a special diet accordingly. One important issue is the sequence of the subtests of a swallowing screen. Nearly every dysphagia screen reported starts with liquids. Clinical observation of acute-stroke patients shows that most of them have more problems swallowing liquids than semisolid textures. Studies of dysphagic patients during motion fluoroscopy found that penetration into the larynx was more likely when swallowing liquids than semisolid textures. On the basis of such findings, we developed a stepwise procedure of assessment aimed at reducing the risk of aspiration during the test to a minimum and enabling a graded rating with separate evaluations for nonfluid and fluid nutrition, starting with nonfluid textures. This would not result in an overall diagnosis of dysphagia based only on insufficient fluid swallowing and thus, in the recommendation to refrain from oral feeding altogether, but it might enable a considerable proportion of patients with acute stroke to continue with semisolid food while recommending that fluids should be applied via intravenous line or nasogastric tube.

Abstract

Background : The World Health Organization (WHO) in 2012 released that 26,4 % of world population suffer from hypertension. One of the contributory factors to blood pressure is stress. The level of blood pressure can be reduced by lowering down the stress level. A non-pharmacology effort to decrease stress level is through relaxation techniques such as back massage and dhikr. Objective : The aim of this research was to reveal the influence of back massage and dhikr to the level of stress of hypertensive patient. Method : This research applied quasi experiment design by using pretest-posttest intervention with control group. The samples of this research were 60 respondents divided to 30 respondents as intervention group and 30 respondents as control group. The intervention group given was a back massage and dhikr as well as anti-hypertension therapy for 3 days consecutively while the control group was only given anti-hypertension therapy. Mann-Whitney test was used to analyze the influence of combination therapy. Result : Test normality using Shapiro Wilk with p=0,012, which means the data is not normally distributed.Wilcoxon test for the influence of the intervention group with p value of 0,008. Back massage and dhikr was proven to decrease the level of stress (p=0,011) by Mann Whitney test in two groups. Conclusion : Back massage and dhikr was proven to significant decrease the level of stress. Decrease stress that occurs is a positive effect of the combination of relaxation . In the next step is expected to decrease stress will affect also to a decrease in blood pressure in patients with hypertension.

Abstract

Background : Hypertensive patients increases every year. Studies on efforts to lower blood pressure in patients with hypertension continue to be developed, one of them with non-pharmacological therapy back massage and dhikr. The aim of this research was to reveal the influence of back massage and dhikr to blood pressure of hypertensive patient. Method : This research applied quasi experiment design by using pretest-posttest intervention with control group. The samples size were 60 respondents divided to 30 respondents as intervention group and 30 respondents as control group. The intervention group given was a back massage and dhikr as well as anti-hypertension therapy for 3 days consecutively while the control group was only given anti-hypertension therapy. Statistic Mann-Whitney test was used to analyze the influence of combination back massage and dhikr intergroup and Kruskal Wallis test was used to test the influence difference of combination of back massage and dhikr to blood pressure in the intervention group. Result : On the Mann-Whitney test showed back massage and dhikr was proven to decrease systole blood pressure (p=0,004) and did not influence the diastole blood pressure (p=0,987). Conclusion : Back massage and dhikr was proven to statistically significant decrease systole blood pressure. The combination, however, reduced diastole blood pressure statistically yet insignificant.

Abstract

Background: World health agency (WHO) in 2012 showed 26.4% of people suffering from hypertension. One of the things that affect blood pressure is stress. Blood pressure can be lowered by lowering stress levels. Non- pharmacological efforts to reduce stress is with relaxation techniques, including aromatherapy lavender and dhikr. The purpose of this research is to know the influence of combination of aromatherapy lavender and dzikir to stress level and blood pressure of hypertension patient. Method: This research uses quasi experimental design with pretest-posttest intervention with control group. The sample was 30 respondents divided into 15 people of the intervention group and 15 control group. The intervention group received lavender aromatherapy and dhikr and antihypertensive therapy for 3 consecutive days while control group only received antihypertensive therapy. The analysis used paired t-test to determine the effect of intervention in the intervention group and independent t-test to see the difference between the influence between groups. Result: lavender and dhikr aromatherapy did not decrease stress score significantly with p = 0,925, mean also in decrease of systole blood pressure with p-value = 0,014 and not significant at decrease of diastole blood pressure with value p = 0,265. Conclusion: The combination of lavender aromatherapy and dhikr significantly decreased systole blood pressure. Whereas for stress and diastolic blood pressure, such combination therapy may be decreased but statistically insignificant.
